Perspectives on the extracellular matrix in inflammatory bowel disease and bowel decellularization protocols.

The extracellular matrix (ECM) is a non-cellular three-dimensional structure present in all tissues that is essential for the intestinal maintenance, function and structure, as well as for providing physical support for tissue integrity and elasticity. ECM enables the regulation of various processes involved in tissue homeostasis, being vital for healing, growth, migration and cell differentiation. Structurally, ECM is composed of water, polysaccharides and proteins, such as collagen fibers and proteoglycans, which are specifically arranged for each tissue. In pathological scenarios, such as i

Deletion of Smad7 Ameliorates Intestinal Inflammation and Contributes to Fibrosis.

BACKGROUND: Patients suffering from inflammatory bowel diseases (IBDs) express increased mucosal levels of transforming growth factor (TGF)-β compared with non-IBD controls. SMAD7 negatively regulates TGF-β signaling. An earlier study aiming to target Smad7 showed a lack of clinical benefit. It remains unknown whether inhibition of SMAD7 is beneficial in specific settings of IBD. We evaluated the effect of Smad7 deficiency on inflammation, fibrogenesis, and wound healing. METHODS: For the initiation of fibrosis in Smad7-/- (Smad7Δex-I) CD-1 mice, the dextran sodium sulfate-induced chronic col

Early histological findings quantified by histomorphometry allow prediction of clinical phenotypes in Crohn's colitis patients.

OBJECTIVE: To evaluate the histomorphometric features of early colonic biopsies from patients with Crohn's disease (CD) and their relationship to clinical phenotypes. The clinical course of Crohn's disease is variable and relevant for treatment selection. Early aggressive treatment may change the course of disease but should be balanced by safety considerations. Currently, prediction of disease course is suboptimal. STUDY DESIGN: Colonic biopsies from CD colitis patients with different phenotypes were analyzed using histomorphometry. The quantitative results were used to predict postbiopsy cl

Long-lasting extracellular matrix modifications reshape intestinal stem cell fate and promote chronic inflammation.

Tissue regeneration is viewed as a return to homeostasis, but whether the extracellular matrix (ECM) reverts during recovery from gut inflammation is unclear. Using temporal multi-omics, biomechanical profiling, and spatial fate mapping in colitis models, we showed that colonic ECM underwent lasting pathological reprogramming following inflammation, which we termed modified (mod)ECM. Characterized by collagen XVIII accumulation and immune-driven proteolysis, modECM redirected intestinal stem cells (ISCs) toward a wound-associated epithelial state with a pro-inflammatory transcriptional program

Glucocorticoid receptor modulators CpdX and CpdX-D3 exhibit the same in vivo antiinflammatory activities as synthetic glucocorticoids.

We previously reported that the nonsteroidal compound CpdX, which was initially characterized 20 y ago as a possible gestagen and, shortly afterward, as a possible drug for treatments of inflammatory diseases, selectively triggers the NFκB/AP1-mediated tethered indirect transrepression function of the glucocorticoid receptor (GR), and could therefore be a selective glucocorticoid receptor agonistic modulator (SEGRAM). We now demonstrate that, upon administration to the mouse, CpdX and one of its deuterated derivatives, CpdX-D3, repress as efficiently as a synthetic glucocorticoid (e.g., Dexame

Epidermolysis bullosa acquisita and inflammatory bowel disease: a review of the literature.

We present a review of all previously reported cases of epidermolysis bullosa acquisita (EBA) and inflammatory bowel disease (IBD). We found 42 cases of coincident EBA and IBD in the literature: 35 cases of Crohn disease (CD) and 7 of ulcerative colitis (UC). The clinical and immunopathological features of the cases are described and the demographics collected. In the majority of cases, the diagnosis of IBD predated the development of the skin condition. The association between EBA and IBD was more common for CD than for UC. We discuss the immunopathogenesis of IBD and EBA, and also the link b

Boswellic Acids and Their Role in Chronic Inflammatory Diseases.

UNLABELLED: Boswellic acids, which are pentacyclic triterpenes belong to the active pharmacological compounds of the oleogum resin of different Boswellia species. In the resin, more than 12 different boswellic acids have been identified but only KBA and AKBA received significant pharmacological interest. Biological Activity: In an extract of the resin of Boswellia species multiple factors are responsible for the final outcome of a therapeutic effect, be it synergistic or antagonistic. Moreover, the anti-inflammatory actions of BAs are caused by different mechanisms of action. They include inhi

Gastrointestinal Manifestations of Yao Syndrome (NOD2-Associated Autoinflammatory Disease).

BACKGROUND: Yao syndrome (YAOS) is a NOD2-associated autoinflammatory disease characterized by periodic fevers, dermatitis, polyarthritis, swelling of the distal extremities, and gastrointestinal symptoms in patients with specific NOD2 variants. While gastrointestinal symptoms are a hallmark of the disease, little is known about specific symptoms and their underlying mechanisms. METHODS: In this study, we identified 24 patients with YAOS from an institutional YAOS databse. We evaluated their presenting gastrointestinal symptoms at initial presentation via the validated Gastrointestinal Sympto

Refractory coeliac disease: a window between coeliac disease and enteropathy associated T cell lymphoma.

The treatment of coeliac disease (CD) is straightforward and simple: life-long adherence to a gluten-free diet. However, in a small subgroup of patients, the clinical and histological abnormalities persist or recur. This non-responsiveness leaves a poorly understood syndrome known as refractory coeliac disease (RCD). A specific definition of RCD is lacking in the literature. We speculate that RCD may appear in a subgroup of coeliacs with persisting histologic abnormalities. In all patients screened for RCD we look for DQ2 and DQ8. In non-DQ2/DQ8 patients we reconsider the diagnosis of CD and o

CFTR dysfunction predisposes to fibrotic liver disease in a murine model.

Cystic fibrosis liver disease (CFLD) is a rapidly progressive biliary fibrosis, resembling primary sclerosing cholangitis that develops in 5-10% of patients with cystic fibrosis. Further research and evaluation of therapies are hampered by the lack of a mouse model for CFLD. Although primary sclerosing cholangitis is linked to both ulcerative colitis and loss of cystic fibrosis transmembrane conductance regulator (CFTR) ion channel function, induction of colitis with dextran sodium sulfate (DSS) in cftr(-/-) mice causes bile duct injury but no fibrosis. Since profibrogenic modifier genes are l

[Functional and inflammatory bowel disorders].

BACKGROUND: Current investigations suggest that postinflammatory mechanisms might induce subtle changes in the mucosa and enteric nervous system which cannot be readily detected by routine diagnostic measures in subgroups of functional bowel disorders, especially in postinfectious irritable bowel syndrome (PI-IBS). DEFINITION OF PI-IBS: Characteristic of IBS is a symptom cluster with abdominal pain/discomfort and altered bowel movement without pathologic findings during routine work-up. PI-IBS is characterized by acute onset of symptoms, fever, vomiting, diarrhea and/or positive stool culture.

β-Glucan-rich Hericium erinaceus hot-water extract ameliorates acute colitis by suppressing inflammation, preserving the epithelial barrier, and modulating gut microbiota ecosystem.

Inflammatory bowel disease (IBD) is a chronic disorder marked by mucosal barrier disruption, immune imbalance, and gut dysbiosis. Polysaccharide-enriched extracts of Hericium erinaceus (HE) have shown anti-colitic effects, but the role of its β-glucan-rich water extract (HE-HW) remains unclear. By investigating a commercially relevant β-glucan-rich extract rather than isolated polysaccharides, this work addresses a key translational gap in HE research. HE-HW, rich in 1,3:1,6-β-glucan (286.4 mg/g), was evaluated using cells and mouse colitis models. In RAW 264.7 macrophages, HE-HW (1-10 μg/mL)

Fecal microbiota transplantation mitigates cardiac remodeling and functional impairment in mice with chronic colitis.

Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the intestines accompanied by profound extraintestinal manifestations. Although IBD shows a clear clinical association with cardiovascular derangements, whether and how chronic colitis impairs heart function remains unclear. To address this gap, we investigated the impact of chronic colitis on cardiac performance and the cardiac transcriptome using two mouse models: dextran sodium sulfate (DSS)-treated and Il10-/- mice. Heart function was assessed by echocardiography and molecular characterization was performed using RNA-se

Administration of neuritin as a novel therapeutic strategy for autoimmune and inflammatory diseases.

Autoimmune and inflammatory diseases are characterized by dysregulated T cell-mediated immune responses leading to tissue damage. Despite therapeutic advancements, patients remain resistant to treatment, highlighting the urgent need for alternative therapeutic strategies. Here, we identified neuritin (NRN), an immunosuppressive molecule, capable of restraining effector CD4+ T cell responses and mitigating autoimmune and inflammatory diseases. NRN is downregulated in CD4+ T cells of rheumatoid arthritis (RA) patients, driving T cell-mediated autoimmune pathology. Nrnfl/flCD4Cre mice exacerbate

Novel Biomarker of Collagen Degradation Can Identify Patients Affected With Both Axial Spondyloarthritis and Crohn Disease.

OBJECTIVE: Chronic inflammatory arthritis is a hallmark of axial spondyloarthritis (axSpA), where coexistence of Crohn disease (CD) is prominent. We investigated the association between biomarkers of collagen degradation in healthy controls (HCs) and in patients with axSpA, CD, and CD and axSpA overlap (CD-axSpA), with the aim to investigate the ability of the biomarkers to identify patients with CD-axSpA. METHODS: Patients with axSpA who fulfilled Assessment of Spondyloarthritis international Society criteria (n = 13), had biopsy-proven CD (n = 14), had CD-axSpA (n = 10), and HCs (n = 11) un

[A Case of Triple-Negative Breast Cancer with irAE-Induced Enteritis Caused by a PD-1 Inhibitor].

The patient, a 70-year-old post-menopausal woman diagnosed with cT2N0M0, Stage ⅡA, triple-negative breast cancer (TNBC), received neoadjuvant chemotherapy consisting of carboplatin, paclitaxel, and pembrolizumab. After 3 courses of chemotherapy, the patient experienced diarrhea over 10 times per day. The stool culture was negative for CD and cytomegalovirus antigens. A histological examination of the sigmoid colon and rectum using needle biopsy revealed lymphocytic infiltration of epithelial cells, apoptosis, and collagen band thickening, consistent with irAE-induced enteritis. We diagnosed di

Prevalence of collagen VII-specific autoantibodies in patients with autoimmune and inflammatory diseases.

BACKGROUND: Autoimmunity to collagen VII is typically associated with the skin blistering disease epidermolysis bullosa acquisita (EBA), but also occurs occasionally in patients with systemic lupus erythematosus or inflammatory bowel disease. The aim of our present study was to develop an accurate immunoassay for assessing the presence of autoantibodies against collagen VII in large cohorts of patients and healthy donors. METHODS: Based on in silico antigenic analysis and previous wetlab epitope mapping data, we designed a chimeric collagen VII construct containing all collagen VII epitopes w

Microscopic Colitis After Fecal Microbiota Transplant.

Microscopic colitis (MC) is an inflammatory condition of the large bowel that is associated with chronic, nonbloody diarrhea. Colonoscopy usually demonstrates normal mucosa, while tissue biopsy reveals intraepithelial lymphocytes or a subepithelial collagen band. Although no specific antibody has been discovered, MC is associated with several autoimmune disorders such as celiac disease, Hashimoto's thyroiditis, and rheumatoid arthritis. There are only a small number of case reports documenting possible hereditary MC cases, but up to 12% of patients with MC have a family history of inflammatory

Inflammatory bowel disease in children and adolescents.

Ulcerative colitis and Crohn's disease are significant childhood illnesses. With their many extraintestinal manifestations, they may masquerade as fever of unknown etiology, arthritis, anorexia nervosa, growth hormone deficiency, collagen-vascular disease, idiopathic growth retardation and even irritable bowel syndrome of childhood. In any child who presents with growth failure and/or chronic abdominal pain with fever or weight loss, the diagnosis of inflammatory bowel disease must be considered. As in any other chronic disease of childhood, long-term management will often challenge the physic
