We report the occurrence of a cerebral ischemic infarct in a young normotensive woman suffering from ulcerative colitis. One week later, clinical neurological deterioration corresponded with hemorrhagic transformation of the previously ischemic region and white matter edema. Extensive laboratory investigation excluded other possible causes for stroke mechanism and rather supported a cerebral vasculitic process, while significant improvement both on neurological and bowel function was observed under prednisone therapy. This report provides some additional support to the possible association bet

OBJECTIVE: This Review provides an overview of the pathophysiology, epidemiology, histopathology, clinical characteristics of non-IBD forms … to confirm the diagnosis and to exclude IBD-associated colitis. These diseases include microscopic colitis, ischemic colitis, segmental colitis associated with diverticula, radiation colitis, diversion colitis, eosinophilic colitis and Behcet's colitis.
TREATMENT: In many cases the treatment is empirical and often the therapy and outcome depend on the severity of …

Toxic megacolon is a complication that usually presents in inflammatory bowel disease such as ulcerative colitis or Crohn's disease. It also occurs in typhoid fever, acute bacillary dysentery, amebic colitis, ischemic colitis, pseudomembranous colitis and severe salmonellosis. This report describes the occurrence of toxic megacolon in two young children with Salmonella colitis manifesting as abdominal distension, diarrhea and fever. The clinical course of these two cases suggests that toxic megacolon is a severe complication of salmonellosis in children, irrespective of the use of antibiotics.…

We present six cases to illustrate the protean clinical and sigmoidoscopic features of the amebic colitis. Three cases presented with a clinical picture resembling ulcerative colitis. In one fatal case, diverticular disease, ischemic colitis and toxic megacolon of uncertain origin were considered before arriving at the proper diagnosis. Two cases were believed to have pseudomembranous colitis on sigmoidoscopic examination. Given the multiple expressions of this entity, amebic colitis should be considered a diagnostic possibility in any protracted inflammatory condition of the colon. From the c

The spontaneous passage of colon cast from a 76-year-old Japanese female patient is reported. Macroscopically, the colon case was shaped like the airbladder of a fish. Histopathologically, the cast consisted of degenerated colonal mucosa, including glands. No inflammatory reaction was apparent. The patient lacked any evidence of abdominal aneurysm. Since there have been only five reported cases of colon cast in the literature, and since in all of those association with abdominal aneurysms was always described, the present study represents the first report demonstrating the formation of a colon

The case of a 45 year old man is reported who developed an acute hemorrhagic colitis after the administration of a soapsud enema. Clinical, laboratory, radiologic, endoscopic and histologic findings are presented and compared with reports from the literature. Differential diagnosis of soap colitis has to include ischemic and drug-induced colitis. Because of the possible complication soapsud enemas should not be used any longer in medical practice.

Ischemic colitis constitutes about half of mesenteric vasculopathies, and it shares a similar pathophysiologic process with acute and chronic mesenteric ischemia. These, in turn, can be divided into occlusive and nonocclusive mesenteric ischemia (NOMI). We present this case of a patient presenting with abdominal pain secondary to right-sided colitis, initially presumed to be infectious, but later attributed to localized NOMI. The correlation between localized NOMI, specifically right-sided colitis, and right ventricular (RV) failure has not been well described in the literature. NOMI, in gener

BACKGROUND: The purpose of this study was to investigate the characteristic sonographic features, such as focal disappearance of the wall stratification sign (FD sign), of longitudinal ulcers in patients with Crohn disease (CD).
METHODS: A total of 545 sonographic examinations of patients with Crohn disease (n = 166), ulcerative colitis (n = 196), bacterial colitis (n = 78), ischemic colitis (n = 63), pseudomembranous colitis (n = 32), Behçet's disease (n = 7), and collagenous colitis (n = 3) were extracted. The sonographic findings were compared with those of a barium contrast study, colonos

During the past 10 years, 122 patients with ulcerative colitis were diagnosed by double-contrast barium enema and colonofiberscopy with endoscopic biopsy. Among them, five patients (4%) had longitudinal ulcers and eccentric deformities in the colon. Other radiologic findings included thumbprinting (two cases), sacculations (two cases), and inflammatory polyps (four cases). The possibility of the concomitance of ischemic colitis in cases of ulcerative colitis is discussed, due to their radiographic similarities.

BACKGROUND: Inferior mesenteric arteriovenous fistula (IMAVF) is an extremely rare condition characterized by abnormal communication between the inferior mesenteric artery and vein. IMAVF often mimics ischemic bowel disease and ulcerative colitis (UC), leading to diagnostic challenges and treatment failures.
CASE SUMMARY: Three consecutive cases presented with non-specific symptoms including perianal pain and bloody diarrhea were reported. Initial diagnosis included acute severe UC and ischemic colitis. Pathology suggested ischemic changes, and with further contrast-enhanced computed tomograp

This article is a review of the side-effects of drugs affecting the small and large intestines. Pseudomembranous colitis is caused by antibiotics facilitating an overgrowth of Clostridium difficile. A hemorrhagic colitis, generally self-limiting, can be caused by penicillin, amoxycillin and ampicillin. Toxicity of NSAID may induce intestinal ulcers, diaphragm-like strictures, perforation, colitis and relapse of inflammatory bowel disease. Drug-induced lymphocytic colitis has been reported due to ticlopidine, Cyclo 3 Fort, and occasionally by ranitidine, carbamazepine, vinburnine, tardyferon, a

Stercoral colitis is a rare inflammatory colitis caused by the accumulation of impacted fecal material. Left untreated, stercoral colitis can progress to fatal complications including ischemic necrosis, ulceration and perforation of the bowel, peritonitis, septic shock, and even death. We describe FDG PET/CT findings of stercoral colitis in the rectum incidentally found on FDG PET/CT in staging a patient with suspected renal cell carcinoma. FDG PET/CT showed fecal impaction in the dilated rectum, asymmetrical rectal wall thickening, perirectal fat stranding, and diffusely inhomogeneous activit

Fiberoptic endoscopy is well established, safe and accurate for diagnosis and management of inflammatory bowel disease (IBD). Direct visualization of the mucosa and biopsy of suspected abnormalities are possible. The principal applications of endoscopy in IBD include discrimination of Crohn's disease from ulcerative colitis of o other inflammatory conditions (acute infectious colitis, chronic infections, microscopic colitis, ischemic/radiation colitis, colitis induced by NSAID), delineation of the sites of disease or extent, grading of endoscopic disease severity, and confirmation of complicat…

We report a case of drug-related segmental colitis in a 70-year-old woman with no thromboembolic risk factor. Mesenteric angiography showed multiple stenoses and occlusions of the distal arterial circulation in the sigmoid and descending colon. These observations support the hypothesis of an ischemic origin for this type of colitis.

Colonic ischemia encompasses a wide clinical spectrum from mild, reversible disease to severe, irreversible injury. It is a frequent disorder of the large bowel in the elderly, and can mimic certain diseases such as inflammatory bowel disease and neoplasms. The clinical course is variable, but often includes crampy, lower abdominal pain and the passage of red or maroon blood mixed with stool. In most cases, management is expectant, with supportive care and attention for signs of complicated disease. Prognosis typically is favorable, with a majority of patients completely resolving their illnes

Colitis in patients with systemic lupus erythematosus (SLE) is quite rare. It can be caused by intestinal vasculitis, mesenteric vascular thrombosis, concomitant inflammatory bowel disease or infectious colitis. It is important to make an accurate and early diagnosis as the treatments for each condition differ and a delayed diagnosis can result in life-threatening complications. However, non-specific gastrointestinal symptoms make a timely diagnosis challenging. Amoebic colitis is a rare condition in patients with SLE. Here we present a case of fulminant amoebic colitis in a patient with SLE w

The endoscopic features of cholesterol atheroembolism affecting the colon have not been extensively described in the literature, owing to the rarity of this entity. We report a middle-aged man who presented with hematochezia after recent coronary artery bypass graft surgery. Colonoscopy revealed ulcerative skip lesions with overlying slough resembling pseudomembranes distal to the transverse colon, inconsistent with the initial clinical impression of ischemic colitis. As a consequence of continued bleeding with hemodynamic instability, the patient underwent an extended low anterior resection w

INTRODUCTION: Idiopathic myointimal hyperplasia of the mesenteric veins (IMHMV) is a rare, non-thrombotic ischemic bowel disease caused by the proliferation of smooth muscle cells in the venous intima. Most patients are initially diagnosed with typical ischemic colitis or inflammatory bowel disease (IBD) and are treated nonsurgically. There is no established treatment for IMHMV, and surgery is the mainstay of treatment, although the optimal surgical approach remains unclear.
CASE PRESENTATION: A 66-year-old man visited the previous hospital with abdominal pain and was clinically diagnosed wit
