Bioactive microneedles enabled by Ca-MOF-regulated collagenase for synergistic hypertrophic scar remodeling.

Hypertrophic scars resulting from pathological fibrosis and excessive collagen deposition cause significant functional and aesthetic impairment. Conventional therapies often fail to address the underlying molecular mechanisms of collagen overproduction. To overcome these limitations, we developed a novel layered dissolving microneedle system (MOF-CCH-MN) capable of delivering a synergistic therapeutic payload. This system incorporates Clostridium histolyticum collagenase (CCH) loaded within a calcium-based metal-organic framework (Ca-MOF), embedded in a γ-polyglutamic acid (γ-PGA) matrix. Char

Perihematomal changes in glucose metabolism and cell counts in the rat collagenase model for intracerebral hemorrhage.

While the perihematomal region of a spontaneous intracerebral hemorrhage (ICH) provides a potential therapeutic target, the pathophysiological processes in that region remain unclear. To evaluate the nature, extent and localization of perihematomal changes, we investigated glucose uptake, water diffusion, and cell counts in the acute phase of collagenase-induced ICH. Sprague-Dawley rats were intrastriatally injected with collagenase or saline (control group). 2-deoxy-2-[18F]fluoro-D-glucose ([18F]FDG) PET, T2-weighted and diffusion-weighted MRI were performed at baseline (n = 30) and day 1 (n

Oxygenated collagenase nanoliposomes for deep photodynamic therapy via remodelling the tumor extracellular matrix.

Melanoma is highly aggressive and remains difficult to treat. Traditional treatment options often result in damage to normal tissues and a high rate of recurrence. Conventional photodynamic therapy (PDT) is limited by poor light penetration, oxygen depletion, and extracellular matrix (ECM) barriers. In this study, we developed an acid-responsive liposome system that co-loads indocyanine green (ICG, a photosensitizer) and 1-bromoperfluorooctane (PFOB) modified with collagenase (Col-LIP-(ICG + PFOB)) to facilitate the degradation of the tumor extracellular matrix and enable near-infrared (NIR)-t

Revisiting rabbit models for keratoconus: A long-term study on collagenase-induced disease progression.

Keratoconus (KC) is a degenerative disorder resulting from the degradation of the stromal collagen fibril network in the cornea, leading to its thinning and conical deformation. Various studies have established animal models of KC by using the collagenase type II enzyme to gain a better understanding of the pathogenesis, however, long-term monitoring or follow-up of the models have not been reported so far. This study evaluates the long-term stability of collagenase type II-induced KC in a rabbit model. Six New Zealand rabbits were divided into 4 study groups with 3 eyes per group. The groups

In vitro efficacy of intralesional Collagenase Clostridium Histolyticum for the treatment of calcified Peyronie's disease plaques.

Peyronie's disease (PD) is defined by penile plaque formation and curvature causing sexual dysfunction. The only FDA-approved intralesional treatment is Collagenase Clostridium histolyticum (CCh). CCh contains two collagenases, AUX1 and AUXII, that break down the type I and type III collagen contained in plaques, leading to plaque dissolution and reduction in penile curvature. Peyronie's plaques, however, also contain fibrin and calcium, which CCh cannot digest. It is unclear if plaque calcification prevents CCh from breaking down plaques. We collected ten tissue samples: five calcified penile…

Changes in Point of Maximal Curvature During Collagenase Clostridium Histolyticum Injections for Peyronie's Disease.

OBJECTIVE: To determine change in the point of maximal curvature (POMC) during Collagenase Clostridium histolyticum (CCH) injections for Peyronie's disease (PD). METHODS: A prospective database has been maintained of all men undergoing CCH injections since March 2014. For the current study, data were abstracted on the POMC with each curve assessment and correlated with demographic and clinical factors. Maximal changes were defined as the largest change in POMC from baseline. RESULTS: Six hundred and eighteen men underwent ≥1 series of CCH, with 313 having a baseline and subsequent POMC measu

Peyronie's disease response to intralesional collagenase clostridium histolyticum therapy is independent of baseline testosterone.

BACKGROUND: Testosterone plays an important role in collagen metabolism, transforming growth factor-β1 expression, and wound healing, which are all critical factors in pathogenesis of Peyronie's disease. Some clinical studies have suggested an association between Peyronie's disease and hypogonadism. OBJECTIVE: We sought to investigate whether baseline total testosterone levels influence response to intralesional collagenase clostridium histolyticum in Peyronie's disease. METHODS: A retrospective review of patients receiving collagenase clostridium histolyticum injections with available total

Insurance approval rates for collagenase clostridium histolyticum prior to discontinuation: a Canada-wide analysis.

Collagenase Clostridium histolyticum (CCh), the first approved non-surgical treatment for Peyronie's disease (PD), was withdrawn from the European, Canadian, and Asian markets due to poor demand and lack of government reimbursement options. We sought to assess insurance approval rates and usage of CCh across Canada to understand the factors that led to its withdrawal. Data on patients prescribed CCh for PD or Dupuytren's contracture was obtained through collaboration with BioScript Solutions to assess the association of variables with insurance approval and prescription filling. We identified

Dupuytren Disease: A Retrospective Cohort Study Comparing Collagenase Injection and Percutaneous Needle Fasciotomy.

UNLABELLED: Collagenase Clostridium histolyticum (CCH) injection and percutaneous needle fasciotomy (PNF) are minimally invasive procedures aiming to relieve Dupuytren disease (DD) by disrupting the cord and restoring the normal functionality of the hand. The purpose of this study is to compare the outcomes and recurrence rates for treatment of DD in the proximal interphalangeal (PIP) and metacarpophalangeal (MCP) joints with either collagenase or percutaneous needle at 3-year follow-up. Moreover, we aim to determine the role of these therapeutic modalities and their impact on hand functionali

Phosphonate as a Stable Zinc-Binding Group for "Pathoblocker" Inhibitors of Clostridial Collagenase H (ColH).

Microbial infections are a significant threat to public health, and resistance is on the rise, so new antibiotics with novel modes of action are urgently needed. The extracellular zinc metalloprotease collagenase H (ColH) from Clostridium histolyticum is a virulence factor that catalyses tissue damage, leading to improved host invasion and colonisation. Besides the major role of ColH in pathogenicity, its extracellular localisation makes it a highly attractive target for the development of new antivirulence agents. Previously, we had found that a highly selective and potent thiol prodrug (with

Efficacy and safety of collagenase Clostridium histolyticum in Peyronie's disease following a modified treatment protocol.

INTRODUCTION: Collagenase Clostridium histolyticum (CCH) is the only approved treatment for conservative management of Peyronie's disease (PD) that has demonstrated efficacy and safety in clinical trials. However, as the standard treatment protocol is time and resource consuming, we are introducing a new CCH treatment protocol with a more cost-effective profile. Our goal is to evaluate its efficacy and safety. MATERIALS AND METHODS: We included patients with PD in stable phase, with curvatures of 30-90degrees. Ventral curvatures and complex deformities were excluded. The treatment protocol co

The successful use of collagenase for Ledderhose disease (plantar fibromatosis) in a paediatric patient: a case report.

Ledderhose disease is a connective tissue disorder involving proliferation of fibrous tissue in the plantar fascia of the foot. Histologically identical manifestations exist in the hand (Dupuytren's contracture) and penis (Peyronie's disease), and collagenase injections are approved as a treatment in both, however not in Ledderhose, where the treatment of choice remains surgical resection. Surgery is associated with high rates of recurrence and need for further surgery, so alternative therapies should be sought. Due to their histological and physiological similarities, it is likely that therap
