Evaluation of the WHO Haemoglobin Colour Scale for diagnosis of anaemia in children and pregnant women as used by primary health care nurses and community health workers in western Kenya.

OBJECTIVES: To evaluate the diagnostic accuracy of the WHO Haemoglobin Colour Scale (HCS) for anaemia in three groups of children aged 2 months to 2 years (sick children, those visiting an immunization clinic and a community-based random sample of children) and a sample of pregnant women. METHODS: Finger-prick blood samples were taken from all consenting participants. Haemoglobin (Hb) levels from the HCS were compared with results from a HemoCue portable haemoglobinometer. Sensitivity, specificity and positive and negative predictive values for the HCS were calculated. RESULTS: A total of 45

Equity impact and cost-effectiveness of a community health worker breast cancer educational programme in rural South Africa: a modelling study.

INTRODUCTION: Breast cancer is a leading cause of cancer-related death among women. Women with lower income, those living in rural areas and women of Black ethnicity are more likely to be diagnosed at advanced stages and have poorer survival outcomes. Reducing these inequities is an important public health priority. This study aimed to identify a cost-effective strategy for reducing breast cancer-related inequities and to evaluate the equity impact of the intervention across population subgroups. METHODS: We developed a novel individual-level microsimulation model to assess both the equity im

Anti-Rabies Vaccine Compliance and Knowledge of Community Health Worker Regarding Animal Bite Management in Rural Area of Eastern India.

Background Rabies is a public health problem in developing countries like India accounting for the second-highest number of rabies-related deaths worldwide. Anti-rabies vaccine (ARV) is the only proven and effective way of preventing death in this 100% fatal disease. However, compliance is a real concern. This study aims to assess the compliance of ARV and rabies immunoglobulin (RIG) among the ARV-clinic beneficiaries and also the knowledge of the health workers regarding animal bite management. Methods A cross-sectional study was conducted in an ARV clinic of a community health centre in a ru

Community health workers and early detection of breast cancer in low-income and middle-income countries: a systematic scoping review of the literature.

BACKGROUND: Breast cancer is the leading cause of female mortality in low-income and middle-income countries (LMICs). Early detection of breast cancer, either through screening or early diagnosis initiatives, led by community health workers (CHWs) has been proposed as a potential way to address the unjustly high mortality rates. We therefore document: (1) where and how CHWs are currently deployed in this role; (2) how CHWs are trained, including the content, duration and outcomes of training; and (3) the evidence on costs associated with deploying CHWs in breast cancer early detection. METHOD…

Community health worker interventions are key to optimal infant immunization coverage, evidence from a pretest-posttest experiment in Mwingi, Kenya.

INTRODUCTION: Immunization is a powerful and cost-effective health intervention which averts an estimated 2 to 3 million deaths every year. Kenya has a high infant and under five mortality and morbidity rates. Increasing routine child immunization coverage is one way of reducing child morbidity and mortality rates in Kenya. Community Health Workers (CHWs) have emerged as critical human resources for health in developing countries. The Community Strategy (CS) is one of the CHW led interventions promoting Maternal and Child Health (MCH) in Kenya. This study sought to establish the effect of CS o…

Rational use of antibiotics by community health workers and caregivers for children with suspected pneumonia in Zambia: a cross-sectional mixed methods study.

BACKGROUND: Antibiotic resistance is an issue of growing global concern. One key strategy to minimise further development of resistance is t … e spread of resistance in resource-poor contexts. This study provides evidence on antibiotic use by community health workers (CHWs) and caregivers to inform iCCM programmes, safeguarding current treatments whilst maximising access to care. METHODS: 1497 CHW consultations were directly observed by non-clinical researchers, with measurement of respiratory rate by CHWs reco …

Design of Heart Well: A randomized pilot study of a community health worker-delivered nutrition intervention for low-income adults with hypertension.

BACKGROUND: Suboptimal diet and food insecurity contribute to hypertension disparities experienced by individuals with low-income. Clinic-based nutrition counseling is limited by time constraints and low access to dietitians. Integrating community health workers (CHWs) with basic nutrition training into care teams may be an effective strategy to promote sustainable dietary modifications. OBJECTIVE: Describe the design of Heart Well, a study to determine whether adding nutrition counseling to a primary care-based CHW hypertension program is feasible and acceptable and improves diet and blood p…

Community health worker involvement across domains and national infectious disease programmes in rural Madagascar: evidence of fragmentation and determinants of engagement.

INTRODUCTION: Community health workers (CHWs) are central to primary healthcare (PHC) delivery and infectious disease control in low-income settings. In Madagascar, CHWs are mandated to contribute across multiple national disease programmes, yet evidence on their actual involvement and its determinants remains limited, particularly for HIV/AIDS and tuberculosis (TB). METHODS: We conducted a cross-sectional survey between March and June 2024 among CHWs in four regions (Analamanga, Atsimo Andrefana, Androy and Anosy). Using two-stage cluster sampling, districts and public primary health centres…

Optimizing community health workers and nonbiomedical approaches to assess the burden of soil-transmitted helminthiasis in a school-aged population in rural Rwanda.

BACKGROUND: Soil-transmitted helminths (STHs) remain a public health problem for children in low- and middle-income countries. Conventional methods for STH diagnosis require well-equipped laboratories and trained personnel, which are often scarce or inaccessible in rural and resource-limited settings. Algorithmic non-biomedical screening tools are a potential strategy for community health workers (CHWs) to ensure rapid STH detection and referral of children. This exploratory feasibility study aimed at assessing whether a CHW-led, algorithm-based screening program could identify school-aged chi…

Pilot Implementation of a Nutrition-Focused Community-Health-Worker Intervention among Formerly Chronically Homeless Adults in Permanent Supportive Housing.

Food insecurity is a known health equity threat for formerly chronically homeless populations even after they transition into permanent housing. This project utilized a human-centered design methodology to plan and implement a nutrition-focused community-health-worker (CHW) intervention in permanent supportive housing (PSH). The project aimed to increase access to healthy foods, improve nutritional literacy, healthy cooking/eating practices, and build community/social connectedness among 140 PSH residents. Validated food-security screening conducted by CHWs identified low or very low food secu

Novel Program of Using Village Health Workers in Early Detection and Awareness of Head and Neck Cancers: Audit of a Community Screening Program.

UNLABELLED: Among the side effects of diabetes mellitus is diabetic nephropathy, the main reason for end-stage kidney disease linked to increased mortality and morbidity. Early diagnostic biomarkers for diabetic nephropathy are required to stop or even slow down the progression of the disease and to administer the most appropriate protective treatments on time. Therefore, it is essential to study additional potential biomarkers for the early diagnosis of diabetic nephropathy. This study aims to evaluate elabela, FABP1, and FABP2 levels synergistically as a new diagnostic tool for the early det
