The hereditary angioneurotic oedema is an exceptional disease of deadly risk from glottic oedema. The diagnosis is based on a very low serum C4, with collapsed C1 INH for a patient who has repeated attacks of Quincke's oedema or pseudo occlusive digestive crisis. These crisis are prevented by taking anabolic steroids and must be treated when they are serious by intravenous C1 INH. A 36-year-old woman was stung in the right wrist by a bee, suffered typical anaphylaxis, and was transferred to a local hospital. After a few hours, which corresponded to late-onset reaction, she developed shortness of breath and weakness and was transferred to the emergency department, where the diagnosis of anaphylaxis was confirmed. Serum complement levels, components C3 and C4, were undetectable. Flexible bronchoscopy excluded lung hemorrhage. She was on the ventilator for 4 days and was fully awake during that time. After the treatment, her improvement was rapid, and she fully recovered.

C-reactive protein (CRP)-mediated complement activation can be tested on cryostat sections of the rat kidney by indirect immunofluorescence (C3-IFT). In contrast to a positive C3-IFT regularly obtained with sera of patients with polymyalgia rheumatica, we found unexpectedly negative C3-IFT results in 5/18 patients with seropositive rheumatoid arthritis and in 11/14 patients with bacterial endocarditis in spite of elevated CRP and normal hemolytic complement in these sera. Differential diagnostic and pathophysiological aspects of these findings are addressed.

Fresh sera from patients with active polymyalgia rheumatica and/or giant cell arteritis are fixing the complement components C4 and C3 to structures of the medulla of rat kidney tissue as demonstrated by the indirect immunofluorescent technique. This PMR-associated reaction is also a useful tool in monitoring disease activity, as it becomes negative under effective treatment with steroids.
