The impact of changing provider remuneration on clinical activity and quality of care: Evaluation of a pilot NHS contract in Northern Ireland.

OBJECTIVES: A pilot NHS dental contract was introduced in Northern Ireland between 2015 and 2016, which involved changing the method for paying general dental practitioners working in the NHS from fee-for-service (FFS) to capitation-based payments, providing an opportunity for a robust evaluation. We investigated the impact of a change in payment methods on clinical activity and the quality of care provided. DESIGN: A difference-in-difference (DiD) evaluation was applied to clinical activity data from pilot NHS dental practices in Northern Ireland compared to matched control NHS practices and

Evaluation of an Academic-Community Partnership to Implement MTM Services in Rural Communities to Improve Pharmaceutical Care for Patients with Diabetes and/or Hypertension.

BACKGROUND: Although the current methods of medication therapy management (MTM) delivery have demonstrably improved therapeutic, safety, economic, and humanistic health outcomes, patient- and prescriber-level barriers persist, limiting its reach and effectiveness. OBJECTIVE: To assess telephonic- and community-based clinical pharmacy services in improving health indicators for rural, underserved patients. METHODS: In 2014, an established MTM provider created a novel, collaborative pilot program with independent retail and community health center pharmacies to provide comprehensive, telephoni

Occupations at risk of contracting zoonoses of public health significance in Québec.

INTRODUCTION: Climate change plays an important role in the geographic spread of zoonotic diseases. Knowing which populations are at risk of contracting these diseases is critical to informing public health policies and practices. In Québec, 14 zoonoses have been identified as important for public health to guide the climate change adaptation efforts of decision-makers and researchers. A great deal has been learned about these diseases in recent years, but information on at-risk workplaces remains incomplete. The objective of this study is to paint a portrait of the occupations and sectors of

Aid cuts, health financing fragility, and maladaptive resilience in Sierra Leone: a rapid mixed-methods study.

Sierra Leone's health system remains highly dependent on external aid, with donor funding accounting for approximately 47% of total health expenditure between 2020 and 2024, compared with a government contribution of around 25-26%. Recent reductions in development assistance for health, including a sharp in-year contraction of USAID-supported health programming during 2024-25 and an approximately 15% reduction in Global Fund allocations, have exposed major structural vulnerabilities in the health financing system. We conducted a rapid mixed-methods study to examine the effects of recent and an

Prevalence and availability of specialty dental care sites for children covered by Medicaid and CHIP in the United States.

INTRODUCTION: Access to dental care is a critical aspect of children's healthcare. For these reasons, Medicaid and Children's Health Insurance Program (CHIP), administered by states, cover medically necessary services. Specialty providers can enroll and participate in state or state-contracted plans, but availability varies widely. Our research objective was to evaluate the availability of non-general dentist specialists in endodontics, oral and maxillofacial surgery, orthodontics/dentofacial orthopedics on the InsureKidsNow website for Medicaid/CHIP enrollees, on a state-by-state basis, and t

Understanding HIV vulnerability among women who inject drugs in Mozambique, 2023.

BACKGROUND: Women who inject drugs (WWIDs) belong to a group of people with high-risk behaviors for contracting HIV. In Mozambique there is still limited coverage of combined HIV prevention strategies among WWIDs. These actions, however, are hampered by the scarcity of data. This study offers a better opportunity to inform the wide range of social, contextual, and individual factors that play a role in the HIV epidemic. At the same time, it will provide crucial explanations about how these diverse factors affect the vulnerability of WWID to HIV in Mozambique. METHODS: A qualitative study was

[Good and cost-effective palliative care in Westphalia-Lippe - but why? Results of a mixed-methods study].

BACKGROUND: The organization of palliative care in Germany varies from region to region. Previous studies show that palliative care in Westphalia-Lippe (WL) - measured by quality and cost indicators - performs better than in other regions. The Westphalian model for outpatient care emphasizes a close cooperation between general practitioners (GPs) and palliative care consultation services (PCS). The basis is a contractual regulation of general and specialized care within a joint outpatient palliative care contract according to Sect. 140a SGB V. The VESPAL study (on the Quality of care in outpat

Pre-Incarceration Healthcare Use and Access Barriers among Men with HIV and those at-Risk for Contracting HIV: A Case Study of an Urban Jail.

Approximately 1.5% of incarcerated people live with HIV. Limited information on their pre-incarceration healthcare use which could inform discharge planning efforts to link them to treatment is available. We investigate factors associated with pre-incarceration healthcare use and access barriers for a status-neutral HIV cohort. We collected data via self-report from men with HIV (n = 22) or at-risk of contracting HIV (n = 77) who entered the Washington, DC, Department of Corrections jail from November 2020 to June 2021. We analyzed pre-incarceration hospital, emergency department (ED), physici

Exceptional Prices of Medical and Other Supplies during the COVID-19 Pandemic in Ecuador.

Shortages of essential supplies used to prevent, diagnose, and treat COVID-19 have been a global concern, and price speculation and hikes may have negatively influenced access. This study identifies variability in prices of products acquired through government-driven contracts in Ecuador during the early pandemic response, when the highest mortality rates were registered in a single day. Data were obtained from the National Public Procurement Service (SERCOP) database between March 1 and July 31, 2020. A statistical descriptive analysis was conducted to extract relevant measures for commonly p

Any-Willing-Provider laws: their financial effect on HMOs.

Any-Willing-Provider (AWP) legislation requires that health plans accept any health care provider who agrees to conform to the plan's conditions, terms, and reimbursement rates. Many states have adopted such legislation, raising questions about its effect on the managed care market. Those favoring this legislation argue that it will reduce restrictions on choice of provider, while opponents argue that it will reduce competition by increasing administrative and medical costs for managed care plans. Using cross-sectional time-series data for the period 1992-1995 (the period during which many of

Dengue as stress test for the resilience of health systems: An ecological analysis of selected public health functions in Brazil.

OBJECTIVES: Health system resilience is defined as the capacity to maintain essential functions under recurrent or episodic stress. In dengue-endemic settings, seasonal outbreaks represent predictable shocks that simultaneously strain multiple Essential Public Health Functions, providing an empirical lens to assess functional continuity. STUDY DESIGN: We conducted an ecological municipality-year study from 2014 to 2024, including 67 Brazilian municipalities with at least 400,000 inhabitants. METHODS: Functional stress years were defined using extreme deviations in municipality-specific stand
