Mothers' Experience of Febrile Convulsions in Their Children.

Background Febrile convulsion is the most common convulsive disorder in under five children. About 1 in 25 children have suffered from febrile convulsion in at least one episode. It is a very difficult condition for parents to handle their children during convulsion and they worry about the future health of the child. It is one of the frightening, emotionally traumatic, and anxiety conditions experienced by parents. Objective This study intended to explore the experience of mothers on febrile convulsion in their children. Method The qualitative research design was used for the study. A purposi

Treatment of pediatric convulsive status epilepticus.

Status epilepticus is one of the most common life-threatening neurological emergencies in childhood with the highest incidence in the first 5 years of life and high mortality and morbidity rates. Although it is known that a delayed treatment and a prolonged seizure can cause permanent brain damage, there is evidence that current treatments may be delayed and the medication doses administered are insufficient. Here, we summarize current knowledge on treatment of convulsive status epilepticus in childhood and propose a treatment algorithm. We performed a structured literature search via PubMed a

The role of metabolic diseases in neonatal convulsions.

OBJECTIVE: The neonatal period is the most vulnerable time for the development of seizures, particularly in the first weeks after birth. These seizures often signify serious malfunction or damage to the immature brain and constitute a neurological emergency, necessitating urgent diagnosis and management. This study was performed to identify the etiology of convulsions during the neonatal period and to determine the rate of congenital metabolic disease. PATIENTS AND METHODS: A total of 107 term and preterm infants 0-28 days old who were treated and followed-up in the neonatal intensive care un

Pharmacotherapy for Pediatric Convulsive Status Epilepticus.

Convulsive status epilepticus (CSE) is one of the most common pediatric neurological emergencies. Ongoing seizure activity is a dynamic process and may be associated with progressive impairment of gamma-aminobutyric acid (GABA)-mediated inhibition due to rapid internalization of GABAA receptors. Further hyperexcitability may be caused by AMPA (alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid) and NMDA (N-methyl-D-aspartic acid) receptors moving from subsynaptic sites to the synaptic membrane. Receptor trafficking during prolonged seizures may contribute to difficulties treating seizure

Non convulsive status epilepticus in the elderly.

Non-convulsive status epilepticus (NCSE) is common in the elderly. It most often corresponds to prolonged focal seizures with impaired contact ("complex partial status epilepticus"). A form of de novo absence status epilepticus, much rarer, can also meet. The identified risk factors for NCSE onset are: a precession by a generalized tonic-clonic seizure, a known history of epilepsy, female gender, and known brain injury (especially a stroke sequelae). The presence of one of these risk factors combined with a confusional picture of unknown origin should lead us to think of the diagnosis of NCSE.

Recognition and management of febrile convulsions in children.

Febrile convulsions (FCs) are characterised by convulsions associated with fever in children aged between 6 months and 6 years. FCs are relatively common and affect 3-4% of children in western countries. This is the most common seizure disorder seen in children. The cause of febrile illness in FC is usually benign and most frequently due to acute viral infection. Convulsions secondary to an intracranial infection (e.g. meningitis, encephalitis) or from acute electrolyte imbalance should not be labelled as FCs. The diagnosis is based mainly on clinical history, and further investigations are ge

Benign convulsions in children with mild gastroenteritis.

BACKGROUND: Benign convulsions with mild gastroenteritis (CwG) is a clinical condition characterized by convulsions occurring in otherwise healthy children, usually in the absence of fever and in the presence of mild acute gastroenteritis. Until now, CwG had not been fully recognized as an epileptic syndrome, and several aspects of this condition are not clearly defined, especially its pathogenesis. METHODS: The main aim of this paper is to discuss after the review of the literature what is known about CwG to facilitate its recognition and treatment. RESULTS: CwG is a benign condition that h

[Diagnosis, treatment and prevention of febrile convulsions].

In cases involving a febrile child having convulsions, the possibility of infections of the central nervous system should be considered and the necessary diagnostic investigations performed. If the febrile convulsion does not end by itself in five minutes, it will be treated as an epileptic seizure. Recurrences cannot be prevented by current treatments. Antiepileptic drugs should not be used, because their adverse effects are larger than the benefit. Antipyretics are not helpful in the prevention of seizures. An essential part of the treatment is informing and reassuring the parents.

Role of apolipoprotein E in febrile convulsion.

Apolipoprotein E is consistently associated with the progression of some common human neurodegenerative diseases, e.g., epilepsy. We hypothesized that genetic variations in the apolipoprotein E gene have implications for susceptibility to, and prognoses in, febrile convulsion, which plays an apparent role in the development of epilepsy. We used the polymerase chain reaction and restriction enzyme digestion to characterize variations of the apolipoprotein E gene. Sixty-nine patients with febrile convulsion (simple/complex) and a corresponding cohort of healthy patients (n = 75) were used. There

Convulsive ergotism: epidemics of the serotonin syndrome?

Between 1085 and 1927, epidemics of "convulsive ergotism" were widespread east of the Rhine in Europe due to consumption of grain contaminated with ergot, which is produced by the fungus Claviceps purpurea. West of the Rhine, consumption of ergot-contaminated food caused epidemics of gangrenous ergotism. The clinical features of convulsive ergotism--muscle twitching and spasms, changes in mental state, hallucinations, sweating, and fever lasting for several weeks--suggest serotonergic overstimulation of the CNS (ie, the serotonin syndrome). The ergot alkaloids are serotonin agonists. Dihydroer

Convulsive-like movements in brainstem stroke.

BACKGROUND: Involuntary convulsive-like movements sometimes occur in patients with brainstem strokes. These movements vary in nature, frequency, and trigger, including fasciculation-like, shivering, jerky, tonic-clonic, and intermittent shaking movements. Some are interpreted as decerebrate postures or seizures. It is important to recognize this type of motor phenomenon since it may be a diagnostic clue for early diagnosis and treatment of brainstem strokes. DESIGN: Case report and review of the literature OBSERVATION: A 72-year-old-man presented with impaired consciousness and jerks of the

Convulsant activity of nonanesthetic gas combinations.

Most nonanesthetics (inhaled compounds that neither cause anesthesia when given alone nor decrease the partial pressure of a known inhaled anesthetic required to produce anesthesia) and transitional compounds (inhaled compounds that are less potent than would be predicted by the Meyer-Overton hypothesis) cause convulsions. A possible exception is the perfluoroalkane series of nonanesthetics. The present study tested whether perfluoroalkanes do provide an exception. Further, we tested whether the convulsant effects of nonanesthetic and transitional compounds were additive. The nonanesthetic per

Trigeminally triggered epileptic hemifacial convulsions.

We present whole-head magnetoencephalographic recordings from a patient suffering from trigeminally triggered left-sided hemifacial convulsions. The patient was a candidate for surgical treatment, but regardless of extensive scalp EEG, videotelemetry and PET recordings, an epileptic focus could not be identified. Magnetic signal distribution during a seizure suggested focal epileptic activity in the face area of the right primary motor cortex. A secondary focus was activated 22 ms later in the left hemisphere. Discharges could be triggered by sensory stimulation of the left lower gum. The simi

Tonic-clonic convulsions in meadow voles.

Some meadow voles (Microtus pennsylvanicus) exhibit prolonged tonic-clonic convulsions, possibly epileptiform seizures, when handled or exposed to a strange environment. These convulsions are often preceded by a period of slow head shaking and/or stiff-legged hopping, but never by the explosively wild running bouts that characterize convulsions in some mammals. Convulsions occasionally occur in meadow voles in response to mild disturbance, as when an individual in its home cage is carried from one room to another. In contrast, they can not be elicited by some of the auditory or olfactory insul
