Osteoporosis Treatments Affect Bone Matrix Maturation in a Rat Model of Induced Cortical Remodeling.

To test how osteoporosis drugs affect bone matrix maturation during cortical bone remodeling, 72 pregnant rats were switched from a 0.4% to a 0.01% calcium diet at parturition for a 23-day lactation period. At weaning, eight dams were sacrificed to establish baseline values, while the remaining dams were returned to 0.4% calcium and treated with vehicle (saline), sodium fluoride (NaF), zoledronic acid (ZA), or sclerostin antibody (Scl-Ab) for either 7 or 28 days (eight animals per group per time point). Femora were examined by μCT, dynamic histomorphometry, Fourier transform infrared imaging,

Anatomy and Imaging Studies on Cortical Bone Screw Freehand Placement Applying Anatomical Targeting Technology.

OBJECTIVES: A series of constant anatomical structures were used as guide targets for screw placement to improve the accuracy of cortical screw placement and reduce surgical injury and fluoroscopy radiation. The most commonly used angles and distances between the cortical bone screw insertion point and the lateral margins of the isthmus were selected as the contents of the questionnaire. METHODS: A total of 40 physicians were selected to determine the specific values for each angle and distance. Screw placements were performed on four dry and six wet lumbar spine specimens according to the pr

Two-year cortical and trabecular bone loss in CKD-5D: biochemical and clinical predictors.

UNLABELLED: This prospective two-year study of patients on chronic dialysis measured changes in bone mineral density (BMD). Patients with higher baseline BMD and shorter dialysis vintage lost more bone. Treatment with anti-hypertensives acting on the central nervous system was protective against bone loss. Baseline serum levels of sclerostin and bone-specific alkaline phosphatase predicted bone loss. INTRODUCTION: This prospective 2-year study of chronic kidney disease on dialysis (CKD-5D) patients assessed trabecular and cortical bone loss at the hip and spine and examined potential demograp

Three-dimensional mapping of cortical bone thickness in subjects with different vertical facial dimensions.

BACKGROUND: The purpose of this study was to determine differences in cortical bone thickness among subjects with different vertical facial dimensions using cone beam computed tomography (CBCT). METHODS: From 114 pre-treatment CBCT scans, 48 scans were selected to be included in the study. CBCT-synthesized lateral cephalograms were used to categorize subjects into three groups based on their vertical skeletal pattern. Cortical bone thickness (CBT) at two vertical levels (4 and 7 mm) from the alveolar crest were measured in the entire tooth-bearing region in the maxilla and mandible. RESULTS:

Soluble VEGFR1 reverses BMP2 inhibition of intramembranous ossification during healing of cortical bone defects.

BMP2 is widely used for promotion of bone repair and regeneration. However, bone formation induced by BMP2 is quite variable. Bone forming progenitor cells in different locations appear to respond to BMP2 in different ways, and repair outcomes can vary as a consequence of modulating effects by other factors. In this study, we have examined the effects of VEGF on BMP2-induced repair of a cortical bone defect, a 1 mm diameter drill hole, in the proximal tibia of mice. Treatment of the defect with either a bolus of PBS or soluble VEGFR1 (sVEGFR1), a decoy receptor for VEGF, had the same effects o

Relationship between the mandibular cortical index and calcaneal bone mineral density in postmenopausal women.

BACKGROUND AND OBJECTIVE: In clinical practice, a comparative analysis of bone mineral density (BMD) is carried out by examining different skeletal bones. This is useful for screening of postmenopausal osteoporosis (OP). The objective of this study was to determine the relation between the mandibular cortical index (MCI) and calcaneal BMD among postmenopausal women. MATERIALS AND METHODS: The study sample included 129 randomly selected postmenopausal women aged 50-77 years. The participants were examined using panoramic radiography for the analysis of the cortical layer in the mandibular base

Morphometric measurement of cortical bone trajectory for lumbar pedicle screw insertion using computed tomography.

STUDY DESIGN: A morphometric measurement of cortical bone trajectory (CBT) for the lumbar pedicle screw insertion using computed tomography (CT). OBJECTIVE: The aim of this study was to conduct a detailed morphometric measurement of the CBT. SUMMARY OF BACKGROUND DATA: The CBT is a novel lumbar pedicle screw trajectory, which follows a caudocephalad path sagittally and a laterally directed path in the transverse plane. The advantage associated with this modified technique is increased cortical bone contact, providing an enhanced screw purchase. However, little is known about the possible scr

The sealed osteons of cortical diaphyseal bone. Early observations revisited with scanning electron microscopy.

The frequency, structure, mode of formation and significance of sealed osteons remain unsettled. Sealed osteons have been reported as an unusual finding in the cortical bone of experimental animals: we extended the observation to human cortical bone studied with SEM. Tibial bone specimens from three patients who sustained a traumatic below-the-knee amputation were used in the study. The observed total mean density of osteons was 19.25/mm(2) and the percentage of sealed and partially sealed osteons was 4.2% and 1.7% respectively. The material sealing the central canal showed an X-ray microanaly

Improved peripheral cortical bone geometry after surgical treatment of primary hyperparathyroidism in postmenopausal women.

CONTEXT: Cortical bone geometry is one of the most important components of bone strength. Excess endogenous PTH or intermittent PTH administration affects cortical bone geometry; however, the changes in cortical bone geometry in patients with primary hyperparathyroidism (pHPT) after parathyroidectomy (PTX) remain unknown. OBJECTIVE: The present study was performed to examine the longitudinal effects of treating endogenous PTH excess on cortical bone geometry in postmenopausal patients with pHPT by using peripheral quantitative computed tomography. PATIENTS: Twenty postmenopausal pHPT patient

Spontaneous healing of large femoral cortical bone defects: does genetic predisposition play a role?

Traumatic segmental femoral defects are commonly high velocity injuries and their reconstruction necessitates challenging surgical procedures. These injuries present commonly as open fractures and thorough wound debridement followed by adequate soft tissue cover is mandatory before skeletal reconstruction can be commenced. The definitive reconstructive procedure is therefore often delayed and the optimal treatment option is frequently determined by the extent of the cortical defect. The complication rate is high and femoral non-union is not uncommon in these injuries but associated head injuri

Cortical bone mineral density in asymmetrical mandibles: a three-dimensional quantitative computed tomography study.

The three-dimensional distribution (3D) of the highest mineralized cortical bone was evaluated in 32 subjects between the ages of 8 and 30 years with asymmetrical mandibles using quantitative computed tomography (QCT). The geometrical distribution of the highest mineralized areas (> 1250 mg/cm3) representative of mandibular cortical bone was determined by 3D reconstruction of the images. The length of the mandible on each side was determined by defining a new linear measurement from the centre of the 3D reconstructed condyle to the midline of the symphysis as identified from a submental view.

Cortical bone maintenance in an historic Afro-American cemetery sample from Cedar Grove, Arkansas.

The relocation and analysis of 80 skeletons from the Cedar Grove Cemetery, located in southwest Arkansas, provides an opportunity to examine the level of health and nutrition experienced by Afro-Americans in the post-Reconstruction South (1878-1930). The demographic profile lends support to the interpretation that Cedar Grove participated in the nationwide decline in Afro-American health. The high frequencies of skeletal lesions indicative of dietary deficiencies and infectious disease demonstrate that this was a highly stressed population. For this analysis, adult femoral thin sections (15 fe

Skeletal pathologies caused by PLS3 inactivation are predominantly associated with impaired cortical bone integrity.

The identification of pathogenic PLS3 variants in individuals with X-linked osteoporosis (XLO) has demonstrated the importance of the actin-bundling protein plastin-3 for skeletal integrity. We and others have previously reported that Pls3-deficient mice display a skeletal phenotype that unexpectedly affects only the cortical bone compartment. To better characterize the impact of PLS3 inactivation on bone (micro-)architectural parameters in humans, we retrospectively analyzed skeletal parameters obtained from male XLO patients and compared them to corresponding data from male individuals with

Inter-implant distance correlated to different preparation protocol on cortical bone: an animal study.

BACKGROUND: Inter-implant distance (IID) plays a crucial role in maintaining peri-implant bone stability and osseointegration. Narrower IIDs (< 3 mm) have been associated with increased bone loss, but the threshold varies with implant connection type. The undersized preparation protocol, which induces static strain in the bone, enhances primary stability but may lead to microstructural damage, remodelling, or necrosis. The effect of this technique on osseointegration in adjacent implants with varying IIDs is not well documented. This study investigates the impact of undersized and non-undersiz

Cortical bone mechanics technology signal quality maintains robustness across a range of biometric profiles.

Current methods of diagnosing osteoporosis, such as DXA, have limitations in predicting fracture risk. Cortical bone mechanics technology (CBMT) offers a novel approach by using a three-point bend test with multifrequency vibration analysis to directly measure ulnar bending stiffness and calculate flexural rigidity, a mechanical property highly predictive of whole-bone strength under bending conditions. Cortical bone mechanics technology targets the diaphyseal ulna, a site composed primarily of cortical bone, enhancing its specificity for cortical bone quality. In this study of 388 postmenopau

Effect of organic matrix alteration on strain rate dependent mechanical behaviour of cortical bone.

The organic matrix phase of bone plays important role in its mechanical performance, especially in the post-yield regime. Also, the organic phase influences loading rate-dependent behaviour of bone which is relevant during the high-speed loading events. Many diseases, as well as aging, affect the matrix phase of bone which causes compromised mechanical properties. Improved understanding of alterations in the organic matrix phase on mechanical response of bone will be helpful in the mitigation of fractures associated with inferior matrix quality. In the present work, effect of alteration in org
