Background and Objectives: The adult superficial middle cerebral vein (SMCV) commonly drains into the middle cranial fossa. However, different embryonic types persist, in which the SMCV drains into the lateral sinus. The basal type of SMCV coursing on the middle fossa floor is a scarce variant. Materials and Methods: During a retrospective study of archived computed tomography angiography (CTA) and magnetic resonance angiography (MRA) files, three rare adult cases of the basal or sphenopetrosal type of SMCV were found and further documented. Results: In the first case, which was evaluated via

Twelve anencephalic and four normal fetuses 26 to 40 weeks gestational age were compared by anatomic, radiographic and histologic methods in order to gain information concerning morphogenesis. In the anencephalics, alterations located within the body of the sphenoid bone led to a reduced cranial floor angle and a more vertical clivus. The reduced lateral extension of the lesser and greater wings of the sphenoid constricted the anterior and middle cranial fossae respectively. The posterior cranial fossa tended to have an increased transverse dimension related to the supraoccipital and exoccipit

Pathophysiology and tomography of the blood vessels of the posterior cranial fossa gain clinical interest in treatment of diseases of the inner ear, complications of middle ear inflammations, tumors of the pyramid and cerebello-pontine angle. Numerous variations in the arterial venous system restrict neuroradiological procedures. Techniques to treat a thrombosis of the sinuses were developed 50 years ago. Surgical procedures to remove glomus tumors of the pyramid could be improved by new anatomical and surgical experiences. Unilateral neck dissection occasionally alters the blood flow in the s

Degenerative changes of the temporomandibular joint (TMJ) present with a broad spectrum of morphological alterations. However, erosions leading to a glenoid fossa defect and condylar impingement of the temporal lobe are a rare finding. A 77-year-old female patient presented with limited mouth opening and pre-auricular pain during mastication on the left side. She denied any neurological dysfunction. Her medical history included poliomyelitis, multiple cancers, and osteonecrosis of the left tibial plateau. Computed tomography revealed advanced degeneration of both TMJs. On the left side, a glen

BACKGROUND AND IMPORTANCE: The anterolateral thigh (ALT) free flap is a versatile reconstructive option for complex 3-dimensional defects of the head and neck. It can be tailored for cutaneous, fasciocutaneous, musculocutaneous, or flow-through configurations based on patient needs. Chimeric flaps comprise distinct tissue components on a shared vascular pedicle and allow simultaneous functional and aesthetic reconstruction in a single stage. We present a novel extension of this concept incorporating a third vascularized component to address multilayer skull base defects.

A case of trigeminal neurinoma with marked extracranial extension is reported with a review of the literature. A 60-year-old female noticed numbness over the left side of the face during the proceeding 15 years. Two years prior to admission, she began to complain of itching and lancinating pain at the left lateral aspect of the nose. Neurologic examination on admission revealed diminished corneal reflex on the left side and hypesthesia in the distribution of the left trigeminal nerve. There was no weakness or atrophy of the ipsilateral masticatory muscles. The remaining cranial nerves and cere

OBJECTIVE: The relationship of the maxilla, with its thin walls, to the nasal and oral cavities, the orbit, and the infratemporal and pterygopalatine fossae makes it a suitable route for accessing lesions involving both the central and lateral cranial base. In this study, we compared the surgical anatomy and exposure obtained by two unilateral transmaxillary approaches, one directed through an upper subtotal maxillectomy, and the other through a lower subtotal maxillectomy.
METHODS: Cadaveric specimens examined, with 3 to 40x magnification, provided the material for this study.
RESULTS: Both

Neurilemmomas of the 12th cranial nerve are rare tumors. We present the first reported case of one occurring in the middle ear. This report details the diagnosis and treatment of a large 12th nerve neurilemmoma in a patient whose initial symptoms included vertigo, otalgia, and left-sided tongue fasciculations with atrophy. Selective carotid angiography and computed tomography scanning defined the extent of the lesion. It involved the skull base and parapharyngeal space with extension into the middle ear and mastoid to the level of the middle fossa and posterior fossa dura. Treatment consisted

OBJECTIVE: The infratemporal fossa (ITF) represents a complex anatomical region of critical relevance in skull base surgery, particularly due to its involvement in the extension of neoplastic lesions. Surgical access to this region remains technically demanding. The orbitozygomatic (OZ) and transmandibular (TM) approaches offer distinct anatomical perspectives and operative corridors. This study aimed to anatomically compare these two techniques to delineate their microsurgical fields, advantages, and limitations in accessing the ITF.

We report a case of intracranial adenoid cystic carcinoma in the middle cranial fossa. A 40-year-old woman had noticed a tumor in the left external auditory canal 8 years previously. It was totally resected at that time. Histologically, the tumor was diagnosed as an adenoid cystic carcinoma. Recently the patient presented with loss of consciousness, suffering from mild recent-memory disturbance. Neurological examination revealed no other abnormalities. Magnetic resonance imaging (MRI) demonstrated an enhanced solid mass with a multicystic component in the left temporal lobe. She underwent subt

The authors report a case of middle cranial fossa neurinoma arising from the left greater superficial petrosal nerve in a 21-year-old woman who presented with a left-sided otitis media that chronically recurred over a period of 5 years. On examination, the patient had a left-sided mild conductive hearing impairment and a slight disturbance in tear secretion on the left side, with sensory disturbance in the left palate. Three-dimensional computerized tomography scans clearly demonstrated the enlargement of the foramen lacerum and foramen ovale, and heavily T2-weighted magnetic resonance images

BACKGROUND: Trigeminal schwannomas (TSs) are intracranial tumors that can cause significant brainstem compression. TS resection can be challenging because of the risk of new neurologic and cranial nerve deficits, especially with large (≥ 3 cm) or giant (≥ 4 cm) TSs. As prior surgical series include TSs of all sizes, we herein present our clinical experience treating large and giant TSs via microsurgical resection.
METHODS: This was a retrospective, single-surgeon case series of adult patients with large or giant TSs treated with microsurgery in 2012-2023.
RESULTS: Seven patients underwent mi

BACKGROUND: Access to tumors of the middle cranial base has traditionally required wide surgical exposures via open craniotomies. These open techniques often require the use of potentially disfiguring skin incisions and are often associated with a significant degree of brain retraction and potential morbidity. We report our experience with the use of a minimally invasive supraorbital endoscopic approach through the eyebrow for excision of middle cranial base tumors in 2 cases.
METHODS: We describe 2 patients with large-sized middle cranial fossa tumors (a medial sphenoid wing meningioma measu

PURPOSE: The parasellar region is a small but complex structure, and a large variety of tumors arise from this region. Postoperative morbidity for these tumors depends significantly on the cranial nerves located in the lateral part of this parasellar region. The aim of this study is to understand the anatomical structures of the lateral parasellar region and describe the inter-dural approach for parasellar tumors.
METHODS: To understand the anatomical structure, microsurgical dissection of both sides of six cadaveric heads and histological study of three cadaveric heads were performed. Based
