Update on craniofacial surgery: the differential diagnosis of lambdoid synostosis/posterior plagiocephaly.

There has been an evolution of thought on the diagnosis and treatment of posterior plagiocephaly. Synostotic posterior plagiocephaly (lambdoid synostosis) can be diagnosed and differentiated from non-synostotic posterior plagiocephaly (positional molding) based on specific cosmetic and radiologic criteria. The advent of high-resolution three-dimensional CT analysis of cranial morphology combined with meticulous clinical studies of the two major causes of posterior plagiocephaly has added much to our understanding of this skull deformity. Children with lambdoid synostosis have a trapezoid-shape

Is craniofacial morphology in Apert and Crouzon syndromes the same?

This article reviews previous research on the craniofacial development in Apert and Crouzon syndromes and adds new roentgencephalometric information. It is concluded that craniofacial development in the two syndromes is not the same. Marked differences were found in the calvaria, cranial base, orbit, maxilla, zygoma, incisal occlusion, and soft tissue profile. In general, abnormal craniofacial morphology was more severe in Apert syndrome than in Crouzon syndrome.

Craniometaphyseal dysplasia: a rare cause of persistent macrocephaly.

Craniometaphyseal dysplasia is a rare sclerosing bone disorder that may be difficult to recognise in early childhood. We report a patient initially evaluated in the toddler period for macrocephaly and craniofacial dysmorphism, in whom early neuroimaging was unremarkable, supporting the diagnosis of macrocephaly of infancy. During early childhood, following head trauma, incidental radiological findings revealed progressive cranial sclerosis. Persistent macrocephaly and craniofacial features prompted further investigation, leading to the diagnosis of craniometaphyseal dysplasia, confirmed by ide

The vascular-osteogenic interface in craniofacial development: a structured review of emerging associations in congenital malformations.

Craniofacial skeletal morphogenesis depends on tightly coordinated interactions between vascular development and osteogenesis. Although congenital craniofacial malformations are traditionally conceptualized as bone-intrinsic disorders, emerging molecular evidence suggests that dysregulated vascular pathways may contribute to skeletal maldevelopment. Here, we synthesize emerging experimental and genetic evidence linking perturbations in vascular development to congenital craniofacial osseous phenotypes. A structured literature search was conducted in PubMed and Embase to identify experimental a

The role of FAT family cadherins in craniofacial development: A scoping review.

OBJECTIVES: To map the existing evidence on the role of FAT family cadherins in craniofacial development, identify knowledge gaps, and infor … y suture, and alveolar bone. Six case reports described four congenital syndromes that present with craniofacial abnormalities. CONCLUSIONS: The available evidence indicates that FAT family cadherins are substantially involved in craniofacial morphogenesis. However, important research gaps remain, particularly regarding their cooperative functions and precise spatiotempora …

Pharyngeal Arches, Chapter 3: Craniofacial Syndromes.

Pharyngeal arch development is complex and can result in a wide range of unique head and neck anomalies. Understanding the developmental processes and the related pathoanatomy is essential for clinicians who manage affected patients. This manuscript is the third in a 4-part educational series for plastic surgeons focusing on normal and abnormal pharyngeal arch development. A summary of the etiology and clinical features of each syndrome covered in this manuscript is provided in the Supplemental Table, Supplemental Digital Content 1, http://links.lww.com/SCS/I489 for quick reference.

Origin stories of neural crest roles in craniofacial development: A tale of the meninges.

As developmental biologists, we tell origin stories to explain differences in vertebrate craniofacial form and complexity. Some of these differences are linked to diseases, notably of patients born with congenital malformations, many of them syndromic. These defective processes impede proper neural crest cell migration, differentiation, and patterning of bones, cartilage, joints, muscle and other connective tissues, including of the head. For example, a rare form of holoprosencephaly called cyclopia is a particular quandary in its diverse presentation. This syndromic congenital disease include

Morphological relationship between the cranial base and facial anomalies in humans: Challenges and future perspectives.

The cranial base (CB) and facial skeleton, despite differing in function and origin, are developmentally related. However, the mechanisms underlying the structural connection remain poorly understood. This article reviews CB malformations in congenital facial anomalies to identify the morphological basis for the relationship between these regions. Various types of CB deformities were identified in each facial anomaly; however, even anomalies with similar facial features did not necessarily share the same CB deformity, indicating that the underlying mechanisms exhibit complex interactions. Critical appraisal of the expert knowledge elicitation (EKE) methodology to identify uncertainties in building cumulative assessment groups for craniofacial alterations.

The European Food Safety Authority (EFSA) conducted a retrospective cumulative dietary risk assessment (CRA) on active substances (AS) and metabolites included in the plant protection products registered in Europe which could provoke craniofacial alterations. Two Cumulative Assessment groups (CAGs) were established: one for alterations due to abnormal skeletal development (CAG-DAC) and one for head soft tissue alterations and brain neural tube defects (CAG-DAH). The probability that each substance is correctly assigned to the specific CAGs (CAG-membership probability) was assessed using weight

Orbital Hypertelorism.

Orbital hypertelorism may be seen in isolation or more commonly within the context of other congenital craniofacial dyscrasias such as craniosynostosis, midface hypoplasia, exorbitism, atypical orofacial clefting, and malocclusion. Careful consideration of the preoperative cranial, orbital, and midface phenotypes, as well as the time of intervention, is imperative to choosing the optimal surgical approach to orbital repositioning. Virtual surgical planning serves as a powerful tool to increase surgical accuracy, decrease skeletal relapse, and mitigate historical complication profiles.

Ocular manifestations and treatment progress of Crouzon syndrome.

PURPOSE: Crouzon syndrome is a congenital genetic disease caused by mutations of the FGFR2 gene on chromosome 10. It is usually inherited in an autosomal dominant pattern and is one of the most common types of craniosynostosis syndromes. This article focuses on the ophthalmology-related aspects of Crouzon syndrome in order to help diagnose and develop personalized treatment plans. METHODS: A combined systematic search of PubMed electronic database by using Boolean operators AND and OR was conducted, choosing the following keywords: "Crouzon", "craniosynostosis", " eye ", " oculus ", " ocular

Adenoid facies: a long-term vicious cycle of mouth breathing, adenoid hypertrophy, and atypical craniofacial development.

Adenoid hypertrophy (AH) is characterized by pathological hyperplasia of the nasopharyngeal tonsils, a component of Waldryer's ring, which represents the first immune defense of the upper respiratory tract. The pathogenic factors contributing to AH remain to be comprehensively investigated to date. Although some studies suggest that environmental exposure to smoke and allergens, respiratory tract infections, and hormonal influences likely contribute to the development of AH, further research is necessary for fully elucidating the effects of these factors on the onset and progression of AH. AH

Long-term treatment outcomes from a patient's perspective with Treacher Collins syndrome.

The management of patients with Treacher Collins Syndrome (TCS) is complex and involves many different specialists within multidisciplinary teams (MDT). The treatment pathway extends from birth well into adulthood and is associated with a heavy burden of care. Due to the extensive nature of the interaction with these patients, MDT members have opportunities to provide enhanced patient-centred care and support. This case report provides an overview of the current knowledge of the aetiology of TCS, the management of these patients and provides a unique perspective from one of the coauthors who ha

Tessier 3 and 4 Clefts and Choanal Atresia: An Unusual Association?

INTRODUCTION: Craniofacial clefts are rare congenital anomalies that might involve both soft tissue and skeletal components. The association of Tessier cleft number 3 and 4 with choanal atresia appears to be unusual and only few clinical cases have been reported in published literature. OBJECTIVES: Report a series of 13 cases of choanal atresia in patients with Tessier numbers 3 or 4 clefts and the literature review on this topic. METHODS: A literature review was undertaken via PUBMED database before April 2020 addressing the association between Tessier numbers 3 or 4 clefts and choanal atre
