Case report on a 57-year-old man who died in progressive respiratory distress with bilateral infiltrates. Sarcoidosis was suspected because of the presence of non-caseating epitheloid granulomas in transbronchial lung biopsies and the elevation of circulating antibodies directed against Kveim antigen. Temperature rose progressively one month after starting steroid treatment. Broad spectrum antibiotics were given without success. Autopsy showed invasive isolated pulmonary cryptococcosis.

A roentgenogram of a 31-year-old office worker who had developed pleuritic pain showed multiple abscess cavities in the left lower lobe, and a clinical diagnosis of cryptococcosis was considered because of cryptococci recovered from a sputum smear and a positive indirect fluorescent antibody test. Lobectomy (after unsuccessful amphotericin therapy) proved the presence of a destructive cryptococcosis with Aspergillus proliferating in the cavities.

A 52-year-old white diabetic male with 4-weeks history of persistent cough followed by headache, drenching night sweats, low-grade fever, worsening photophobia, nausea and vomiting was presented. Examination was significant for photophobia and diminution of vision. His spinal fluid and blood cultures were positive for Cryptococcus neoformans Intravenous fluconazole were given for 2 weeks followed by oral fluconazole. There was significant improvement in systemic and ocular symptoms. HIV serology was negative, but his CD4 counts were low with inverted CD4:CD8 ratio.

A case of an 82-year-old female with primary laryngeal cryptococcosis who had undergone long-term corticosteroid therapy for chronic obstructive pulmonary disease and rheumatoid arthritis is reported.
