[Limited applications for hydroxyethyl starch : background and alternative concepts].

BACKGROUND: Within the framework of a risk assessment procedure the Committee for Risk Assessment of Pharmacovigilance (PRAC) of the European Medicines Agency (EMA) came to the conclusion that the benefits of hydroxylethyl starch infusion solutions (HES) no longer outweighed the risks and on 14 June 2013 recommended that approval should be suspended. Until the procedure has finally been concluded, which could last several months, the Federal Institute for Drugs and Medical Products (BfArM) has recommended that HES should not be used. AIM: The aim of this article is to present the data situati

Terminology used to describe volume expanding resuscitation fluid.

Many published reports state that the type of resuscitation fluid to be used for volume expansion should be "isotonic crystalloid" or "isotonic fluid". Generally, this terminology refers to normal saline (NS) and lactated Ringer's (LR) solution. This terminology is suboptimal technically since not all "isotonic" crystalloids are sufficient volume expanders. A total of 48 reference books from the areas of general pediatrics, pediatric emergency medicine, emergency medicine, pediatric critical care, pediatric surgery, general surgery, internal medicine, family medicine, adult critical care, obst

Initial cardioplegic flush with crystalloid cardioplegia improves donor heart preservation and function via NR4A3 upregulation and metabolic reprogramming.

Static cold storage (SCS) remains the gold standard for preserving donor hearts for heart transplantation. The currently used University of Wisconsin (UW) solution has limitations in inducing rapid cardiac arrest and preservation of the heart, especially when a long cold ischemic time is required. Here, we propose a novel donor heart procurement and preservation method-crystalloid cardioplegia (CC)+UW involving an initial cardioplegic flush with crystalloid cardioplegia followed by preservation in UW solution. Our results demonstrated that compared with using UW solution alone, the CC+UW metho

Colloid solutions: a clinical update.

Albumin, dextran, gelatin, and hydroxyethyl starch (HES) solutions are colloids that efficiently expand the circulating blood volume. The administration of colloids restores the intravascular volume with minimal risk of tissue edema in comparison with crystalloid solutions alone. However, colloids are always given for surgical and critically ill patients. The type of the colloid, volumes applied, aggressiveness of fluid resuscitation, and the volume status at the initial phase of administration determine their clinical responses. The outcome after fluid resuscitation with various colloids in c…

Estimating the Starling equation parameters in living humans by using volume kinetic analysis.

The transcapillary filtration of fluid is governed by the Starling forces. The purpose of the present study was to simultaneously estimate all Starling parameters in living humans by using likelihood mathematics. First, the capillary filtration was estimated by volume kinetic analysis at 3,015 time points during 193 intravenous infusion experiments using crystalloid and colloid fluid in adult volunteers and surgical patients placed in the supine position. Second, the obtained capillary filtration and the measured plasma colloid osmotic pressure were entered into a new kinetic analysis to estim

Efficacy of Tranexamic Acid in Blood Versus Crystalloid-Resuscitated Trauma/Hemorrhagic Shock.

INTRODUCTION: Whole blood (WB) or blood products are not always immediately available for repletion of lost intravascular volume in trauma/hemorrhagic shock (T/HS), and thus, resuscitation with crystalloid solutions is often necessary. Recently, we have shown enteral tranexamic acid (TXA) to be effective as a mild protease inhibitor in blood-resuscitated T/HS by counteracting proteolytic activity in and leaking from the gut with resultant preservation of systemic vascular integrity. We hypothesized that enteral TXA would improve hemodynamic stability after T/HS in the absence of blood reperfus…

Technical limitations in the rapid infusion of intravenous fluids.

We compared fluid delivery, both in vitro and in vivo, using various combinations of fluid sets and intravenous catheters. Administration sets were a minidrip, a maxidrip, and a blood infusion set. The catheters included 14-, 16-, 18-, and 20-gauge short catheters, 16- and 19-gauge long catheters, and an 8 French catheter introducer for flow-directed pulmonary arterial lines. Blood infusion tubing alone delivered fluid at 3.12 +/- .07 mL/second, significantly faster than either the maxidrip (2.59 +/- .06, P less than .01) or the minidrip (0.56 +/- .02, P less than .001). The 8 French introduce

Normal saline versus balanced crystalloids in patients with prerenal acute kidney injury and pre-existing chronic kidney disease.

INTRODUCTION: Normal saline (N/S) and Ringer's-Lactate (L/R), are administered in everyday clinical practice. Despite that, N/S increases the risk of sodium overload and hyperchloremic metabolic acidosis. In contrast, L/R has lower sodium content, significantly less chloride and contains lactates. In this study we compare the efficacy of L/R versus N/S administration in patients with prerenal acute kidney injury (AKI) and pre-established chronic kidney disease (CKD). METHODS: In this prospective open-label study we included patients with prerenal AKI and previously known CKD stage III-V witho

Statistical analysis plan for Better Evidence for Selecting Transplant Fluids (BEST-Fluids): a randomised controlled trial of the effect of intravenous fluid therapy with balanced crystalloid versus saline on the incidence of delayed graft function in deceased donor kidney transplantation.

BACKGROUND: Delayed graft function, or the requirement for dialysis due to poor kidney function, is a frequent complication of deceased donor kidney transplantation that is associated with inferior outcomes. Intravenous fluids with a high chloride content, such as isotonic sodium chloride (0.9% saline), are widely used in transplantation but may increase the risk of poor kidney function. The primary objective of the BEST-Fluids trial is to compare the effect of a balanced low-chloride crystalloid, Plasma-Lyte 148 (Plasmalyte), versus 0.9% saline on the incidence of DGF in deceased donor kidney

Acute normovolemic haemodilution for management of blood loss during radical prostatectomy.

INTRODUCTION: The reduction of the risks of anemia and allogeneic transfusion is one the basic parts of the anaesthesia management in large urological procedures. We used acute normovolemic haemodilution (ANH) as a technique of autologous blood procurement in patients scheduled for radical prostatectomy. METHODS: 15 patients undergoing radical prostatectomy were enrolled in our study. After starting general anaesthesia the left radial artery line was placed for invasive blood pressure monitoring and withdrawing blood for ANH. The restoration of circulated volume was instituted by infusion of

Inefficacy of intraperitoneal fluid administration in a shock model.

Intraperitoneal (IP) fluid administration has been previously used to treat dehydrated children. The relative ease and reported safety of this route suggests its potential utility for volume resuscitation in the acutely ill child. Previous research regarding IP fluid infusion has not examined its use in shock states. This experiment sought to examine the efficacy of an IP fluid infusion in a shock model. Ten immature swine were randomized to receive either no treatment (n = 5) or 30 ml/kg of warmed lactated Ringer's solution (n = 5) IP 20 minutes after a graded 40% hemorrhage. Mean arterial pr

Efficiency of the Thermal Jacket on the delivered temperature of prewarmed crystalloid intravenous fluid.

Warming of intravenous fluids can decrease the incidence of hypothermia in surgical patients. A quasi-experimental research design was used to compare the delivered temperature of fluids using a conventional blood warmer and the Thermal Jacket, an insulation device designed for intravenous fluid bags. Fluids were divided into one control and three experimental groups. Fluids were initially either room temperature (22.05 +/- 0.45 degrees C) or prewarmed to 41.45 +/- 1.05 degrees C and infused using no temperature maintenance device, a Thermal Jacket, or a water bath blood warmer. Temperatures w

Systemic complications of fluid resuscitation.

Fluid administration in critically ill individuals is frequently a major component of their therapy. There are important effects on blood pressure and maintenance of cardiac output and oxygen delivery, as detailed elsewhere in this text. There are also potentially negative side effects of this therapy, which have been less well defined. Edema of the gastrointestinal tract has been well described, primarily with crystalloid infusions. Gastrointestinal edema may have very complicated effects on albumin kinetics, fluid flux, and ion flux. It may lead to development of ileus. Increased nasogastric

The use of blood in burn shock. Clinical and experimental study.

We have given whole blood as one of the main constituents of burn shock resuscitation for the past 28 years. To appraise the value of using whole blood, we have summarized the clinical experience of 2630 burn patients. Overall mortality was 4.18%. The lethal area of the burn were 50% of the population is expected to die was 82.8% total body surface area and 57.4% third-degree burns. The incidence of renal failure, pulmonary edema, and gastrointestinal bleeding was 0.9%, 0.4%, and 0.6% respectively. To confirm the advantage of transfusion of whole blood, we have carried out a series of experime

Factors affecting rapid fluid resuscitation with large-bore introducer catheters.

Rapid fluid administration is the cornerstone of successful trauma resuscitation. Percutaneous insertion of catheter introducers has gained wide acceptance as a quick and reliable means of rapid intravascular volume expansion. Factors that affect rapid fluid resuscitation with these devices include catheter introducer kinking, the type and temperature of infusate, and diameter of co-apted administration tubing. Rates of flow through 8.5 French catheters from 0 to 80 degrees of catheter angulation were determined in vitro for various fluids (crystalloid, whole blood, diluted packed cells) and a
