BACKGROUND: Precocious puberty and polycystic ovarian syndrome are two different entities which appear at different stages of ovarian development. Their association is uncommon.

A 30-year-old transgender woman who was HIV positive presented to the ED with progressive severe dyspnea and hemoptysis that started 1 day earlier. The patient was undergoing antiretroviral therapy with emtricitabine-rilpivirine-tenofovir with good compliance and feminizing hormone therapy with cyproterone acetate. Data from 31 children, 23 females (74.2%) and eight males (25.8%) with precocious puberty were analysed. They included 19 cases with central precocious puberty, 5 with premature thelarche, 5 with premature adrenarche and 1 each with adrenal adenoma and virilizing hepatoblastoma. Their clinical and hormonal profile and the treatment outcome are briefly discussed.

Sitting height (SH) and sub-ischial leg length (SLL) were measured in 10 boys and 16 girls with precocious puberty; the patients were aged from 1.5 to 13.4 years at the time. Standard deviation scores (SDS) calculated for chronological age and bone age showed higher scores for SH than for SLL in all but two patients, both girls: the differences between the SDS for SH and SLL were more marked in the boys. The findings indicate that growth of the trunk is usually greater than growth of the legs in precocious puberty, particularly in boys.

Craniopharyngiomas in children are usually associated with growth retardation and hypogonadism. A patient with a craniopharyngioma treated with radiotherapy, who later developed isosexual precocious puberty, is described. She was treated with cyproterone acetate with a good clinical response. We suggest that craniopharyngiomas may become a more frequent cause of precocious puberty now that conservative therapy has been advocated for the condition.

Several antiandrogens compounds are available in the treatment of acne and hirsutism in women. Cyproterone acetate is the most effective antiandrogen. Its use must prevent the occurrence of amenorrhea with the prescription of an appropriate estrogen. A simple photographic technique was used to assess the objective response of hirsute women to treatment with cyproterone acetate (CA).
