Prevention of hypernatraemic dehydration in breastfed newborn infants by daily weighing.

UNLABELLED: Hypernatraemic dehydration, which predominantly appears in breastfed neonates, can cause serious complications, such as convulsions, permanent brain damage and death, if recognised late. Weight loss > or = 10% of birth weight could be an early indicator for this condition. In this prospective cohort study from October 2003 to June 2005 in the postnatal ward of the University Hospital Zurich, Switzerland, all term newborns with birth weight > or = 2,500 g were weighed daily until discharge. When the weight loss was > or = 10% of birth weight, serum sodium was measured from a heel pr…

Hypernatremic dehydration in the newborn period and long-term follow up.

BACKGROUND: Hypernatremic dehydration due to inadequate fluid intake can be a significant problem during the neonatal period and thereafter. The objective of the present study was to evaluate the term newborn infants admitted to Gazi University Hospital neonatal intensive care unit (NICU) for hypernatremic dehydration between June 2001 and June 2003 and compare the results with those of the literature search performed via MEDLINE for infants with the same diagnosis. METHODS: Infants with weight loss > or =10% were evaluated for hypernatremia and the ones with serum Na level > or =150 mEq/L we…

Brain apparent diffusion coefficient decrease during correction of severe hypernatremic dehydration.

We report on the brain diffusion MR imaging findings in a neonate with severe hypernatremic dehydration, which resulted in cerebral edema (osmotic edema) and in apparent diffusion coefficient decrease, despite a careful and slow rehydration. This report provides in vivo insight into nervous cell response to osmotic challenge.

Neonatal hypernatremic dehydration associated with breast-feeding malnutrition: a retrospective survey.

BACKGROUND: Hypernatremic dehydration in neonates is a potentially devastating condition. Recent reports have identified breast-feeding malnutrition as a key factor in its pathophysiology. METHODS: Using a theoretical framework for breast-feeding kinetics, a retrospective chart review of all neonates less than 28 days of age who were seen at either British Columbia's Children's Hospital or the Vancouver Breastfeeding Centre between 1991-1994 was conducted to identify and classify possible causes of breast-feeding malnutrition among neonates who developed hypernatremic dehydration. RESULTS: T…

Renal compensation for body water loss during dehydration in neonatal rats.

The present study was designed to investigate whether the limited capacity for concentrating urine in neonatal rats is associated with an im … o 7.9 +/- 0.7 microliter/min/100 g (14 d). The raised values were maintained at the later period of dehydration. Thus serum osmolality was unchanged throughout dehydration: 287 +/- 1.0 (7 d) and 292 +/- 0.9 mosmol/kg H2O (14 d). On the other hand, the level of serum sodium concentration slightly but significantly increased (r = 0.61) when the body weight redu …

Endolymph volume changes during osmotic dehydration measured by two marker techniques.

The processes underlying endolymph volume regulation during osmotic disturbances were investigated in vivo using ionic volume markers. The m … the perilymphatic space with hypertonic media. The endolymph potassium increase produced by osmotic dehydration was of similar magnitude and time course to that of a volume marker loaded by the PVM method. Using the IVM method, it was shown that these concentration increases arose by two distinct processes. One component was the area decrease of scala media. …

Dehydration-induced release of vasopressin involves activation of hypothalamic histaminergic neurons.

The hypothalamic neurotransmitter histamine (HA) induces arginine vasopressin (AVP) release when administered centrally. We studied and characterized this effect of HA with respect to receptor involvement. In addition, we studied the possible role of hypothalamic histaminergic neurons in the mediation of a physiological stimulus (dehydration) for AVP secretion. Intracerebroventricular administration of HA, the H1-receptor agonists 2(3-bromophenyl)HA and 2-thiazolylethylamine, or the H2-receptor agonists amthamine or 4-methyl-HA stimulated AVP secretion. The stimulatory action of HA on AVP was …

Impaired high-intensity cycling performance time at low levels of dehydration.

On two separate occasions six trained subjects (peak oxygen consumption [VO2peak] 4.41/min) rode for 60 min at 70% of VO2peak and then to exhaustion at 90% of VO2peak to determine the effects of mild dehydration on high-intensity cycling performance time in the heat (32 degrees C, 60% relative humidity, wind speed 3 km/h). In one trial (F) subjects ingested a 400 ml bolus of 20 mmol/l NaCl immediately before, and then as repetitive 120 ml feedings every 10 min during the first 50 min of exercise. In the other trial they did not ingest fluid (NF) either before or during exercise. The order of t…

[Secondary rhabdomyolysis and acute renal failure in gastroenteritis with hypernatremic dehydration].

A child with severe gastroenteritis developed acute rhabdomyolysis combined with acute renal failure. The pathophysiological mechanisms of this clinical syndrome are not completely understood. Rhabdomyolysis is mainly due to osmotic changes on both sides of the cell membrane. The preventive and therapeutic measures consist of adequate fluid substitution, correction of electrolyte and acid-base imbalances, administration of diuretics and alkalization of the urine. Dialysis treatment may be necessary. In this particular case renal failure resolved but the child died of other organ failure.

Osmolar relation between cerebrospinal fluid and serum in hyperosmolar hypernatraemic dehydration.

The relation between cerebrospinal fluid (CSF) and serum osmolality was studied in 16 patients with hyperosmolar hypernatraemic dehydration before treatment. After correcting shock and acidosis, 0-45% saline in 2-5 or 5% dextrose was infused in each patient over a 48- to 72-hour period. During rehydration, serum osmolality, electrolyte concentrations, urea nitrogen, and blood pH were measured sequentially. Five patients developed severe neurological abnormalities within 48 hours of addmission (convulsions 2, convulsions with hemiplegia 2, hemiplegia 1). Of these, 3 had residual defects on foll…

Deep Dehydrated Layered Vermiculite Membrane for Selective Lithium Separation.

Two-dimensional (2D) layered membranes hold great promise in water treatment due to their simple film formation, tunable interlayer spacing, and designable surface chemistry. However, their practical application of aqueous ion sieving is largely constrained by swelling problems. Here, we report a deep dehydration strategy to suppress reswelling after ion-intercalation by removing free and bound water within the interlayer channels. The deep dehydration drives the interlayer free spacing below a critical threshold, enhancing van der Waals and electrostatic interactions, thereby energetically in…

Use of POCUS for the assessment of dehydration in pediatric patients-a narrative review.

In pediatric practice, POCUS (point-of-care ultrasound) has been mostly implemented to recognize lung conditions and pleural and pericardial effusions, but less to evaluate fluid depletion. The main aim of this review is to analyze the current literature on the assessment of dehydration in pediatric patients by using POCUS. The size of the inferior vena cava (IVC) and its change in diameter in response to respiration have been investigated as a tool to screen for hypovolemia. A dilated IVC with decreased collapsibility (< 50%) is a sign of increased right atrial pressure. On the contrary, a co…

The Effect of Endolymphatic Hydrops and Mannitol Dehydration Treatment on Guinea Pigs.

BACKGROUND: Endolymphatic hydrops (EH) is considered as the pathological correlate of Menière's disease (MD) and cause of hearing loss. The mechanism of EH, remaining unrevealed, poses challenges for formalized clinical trials. OBJECTIVE: This study aims to investigate the development of hearing loss, as well as the effect of dehydration treatment on EH animal models. METHODS: In this study, different severity EH animal models were created. The laser Doppler vibrometer (LDV) and auditory brainstem responses (ABR) were used to study the effects of EH and the dehydration effects of mannitol. T…
