Insights into obstetric anesthesia practices: a quantitative survey among physicians across Arab countries.

BACKGROUND: Obstetric anesthesia guidelines are essential for standardizing obstetric anesthesia practices globally and ensuring high-quality patient care. However, practices may vary across different settings, and there are limited data from Arab countries. This study aims to gain insights into obstetric anesthesia practices in several major hospitals across Arab countries. METHODS: A questionnaire was emailed to 85 obstetric anesthesiologists/anesthesia chairpersons in major hospitals, including academic medical institutions and central hospitals, across 11/22 Arab countries. This survey ga

Counseling After Obstetric Anal Sphincter Injury: How do Health Care Providers Perform?

IMPORTANCE: Obstetric anal sphincter injuries are associated with significant morbidity and increased recurrence risk in subsequent deliveries. In subsequent pregnancies, it is important to provide appropriate counseling and mode of delivery planning. OBJECTIVES: The primary objective of this study was to determine the rate of obstetric anal sphincter injury counseling that patients with prior injury receive in their subsequent pregnancy. Secondary objectives include assessing the counseling quality, effect of counseling on delivery decisions, and counseling rates by patient risk factors. ST

Maternal and Fetal Outcomes of Contemporary Obstetric Interventions: A Systematic Review.

Labour and delivery interventions play an important role in maternal and neonatal care, particularly when labour progress is delayed, fetal well-being is uncertain, or high-risk obstetric conditions are present. Current literature often examines these interventions separately, whereas clinical practice typically involves overlapping decisions regarding induction, augmentation, monitoring, caesarean timing, operative delivery, and newborn care. This review narratively summarised findings from 10 completed studies examining maternal, delivery, surgical, and neonatal outcomes across distinct obst

Estimating the obstetric co-morbidity burden using administrative data: The impact of the pregnancy-related assessment window.

BACKGROUND: Despite increased research using large administrative databases to identify determinants of maternal morbidity and mortality, the extent to which these databases capture obstetric co-morbidities is unknown. OBJECTIVE: To evaluate the impact that the time window used to assess obstetric co-morbidities has on the completeness of ascertainment of those co-morbidities. METHODS: We conducted a five-year analysis of inpatient hospitalisations of pregnant women from 2010-2014 using the Nationwide Readmissions Database. For each woman, using discharge diagnoses, we identified 24 conditio

Obstetric Anesthesia and Heart Disease: Practical Clinical Considerations.

Maternal morbidity and mortality as a result of cardiac disease is increasing in the United States. Safe management of pregnancy in women with heart disease requires appropriate anesthetic, cardiac, and obstetric care. The anesthesiologist should risk stratify pregnant patients based upon cardiac disease etiology and severity in order to determine the appropriate type of hospital and location within the hospital for delivery and anesthetic management. Increased intrapartum hemodynamic monitoring may be necessary and neuraxial analgesia and anesthesia is typically appropriate. The anesthesiolog

Risk factors for obstetric anal sphincter injury: an exploratory Norwegian regional cohort study including amniotomy as a potential novel determinant.

BACKGROUND: Obstetric anal sphincter injury refers to the unintended tearing of the anal sphincter and/or rectal mucosa during vaginal delivery, a complication associated with significant morbidity. Well-known risk factors for obstetric anal sphincter injury include primiparity, high maternal age, instrumental delivery, oxytocin augmentation, and large fetal size. Recently, we identified amniotomy as a potential novel independent risk factor, regardless of parity. This study aims to further explore whether amniotomy increases the risk of obstetric anal sphincter injury including known determin

Episiotomy use among vaginal deliveries and the association with anal sphincter injury: a population-based retrospective cohort study.

BACKGROUND: The rate of obstetric anal sphincter injury has increased in recent years, particularly among operative vaginal deliveries. We sought to characterize temporal trends in episiotomy use and to quantify the association between episiotomy and obstetric anal sphincter injury. METHODS: Using a population-based retrospective cohort study design of hospital data from 2004 to 2017, we studied all vaginal deliveries of singleton infants at term gestation in Canada (excluding Quebec). Rates of obstetric anal sphincter injury were contrasted between women who had an episiotomy and those who d

The symptom of stress incontinence caused by pregnancy or delivery in primiparas.

Three hundred five primiparas were interviewed repeatedly about stress incontinence before and during pregnancy and after delivery. Eleven (4%) had stress incontinence before pregnancy and 98 (32%) during pregnancy, whereas 21 (7%) developed it after delivery. According to the International Continence Society definition, the corresponding frequencies were one (0.3%), three (1%), and one (0.3%), respectively. Obstetric factors such as length of the second stage of labor, head circumference, birth weight, and episiotomy seemed to be associated with, whereas cesarean delivery seemed to protect ag

Adding Insult to Injury: Levator Ani Avulsion in Women With Obstetric Anal Sphincter Injuries.

OBJECTIVE: In women with obstetric anal sphincter injuries, we compared the rate of major levator ani avulsion after forceps-assisted delivery versus spontaneous vaginal delivery. METHODS: Prospective cohort of primiparous women with obstetric anal sphincter injuries. The primary outcome was the rate of major levator ani avulsion as measured by 3-dimensional transvaginal ultrasonography performed between 1 and 2 weeks postpartum. Secondary outcomes included ultrasonographic anteroposterior hiatal diameter, levator hiatal area, and levator-urethra gap, and differences in validated pelvic disor

Pregnancy and delivery in ehlers-danlos syndrome (hypermobility type): review of the literature.

Ehlers-Danlos syndrome (EDS) is a group of connective tissue disorders which are divided into various distinguishable phenotypes. The type of EDS determines the potential obstetric complications. Due to the spectrum of clinical manifestation and overlap between phenotypes, there are no standardised obstetric management guidelines. Existing literature illustrates different obstetric management in hypermobility type of EDS, including uneventful term vaginal deliveries as well as preterm cesarean section deliveries. This paper discusses obstetric management of a woman with EDS hypermobility type.

Obstetric Outcomes Among Underage Mothers: Age-Stratified Analysis from a Romanian Hospital-Based Cohort.

Background and Objectives: Adolescent pregnancy is traditionally associated with increased obstetric risk, particularly among very young adolescents. Romania continues to report one of the highest adolescent birth rates in the European Union, yet age-stratified clinical data on obstetric outcomes among minors remain limited. This study aimed to evaluate maternal and intrapartum outcomes among underage mothers (<18 years), stratified by age (<15, 15-16, and 17 years), and to identify predictors of adverse obstetric outcomes. Materials and Methods: We conducted a retrospective cohort study of al

The impact of comprehensive training on safe cesarean delivery on non-physician surgeons' knowledge in Tigray, Ethiopia: a pre- and post-test embedded mixed-methods study.

BACKGROUND: In Ethiopia, non-physician surgeons, such as Integrated Emergency Surgical Officers (IESOs) and MSc Clinical Midwives, provide critical comprehensive emergency obstetric and surgical care in areas, where physician shortages persist. Although task shifting has improved access to maternal healthcare services in many low-resource settings, maternal and newborn mortality remain high, particularly in the conflict-affected Tigray region. This study aimed to assess the impact of a comprehensive training program on safe cesarean delivery, essential newborn care, and obstetric anesthesia on

Analysis of obstetric complications reported to the National Patient Insurance Association in Finland from 1987 to 1995.

BACKGROUND: The launch of the National Patient Insurance Association in 1987 offered a good starting point to evaluate obstetric claims in Finland. In order to obtain full compensation after patient injury, proof of malpractice is no longer required. Thus, the register of the Association offers a solid data base to analyze these injuries. METHODS: A nationwide descriptive study of obstetric claims reported to the National Patient Insurance Association from 1.5.1987 to 31.12.1995. The recorded statistical datafiles of the Association were used in the analysis. RESULTS: A total of 801 obstetri
