A histological comparison of direct pulp capping agents in primary canines.

The purpose of this study was to evaluate the pulpal response to four calcium hydroxide preparations in primary teeth. Data indicated the calcium hydroxide-saline paste induced more inflammation and produced a greater zone of mummification than the three commercial preparations studied. Unintentional deep impaction of the medicament or dentinal chips increased inflammation for all categories. All medicaments produced a superficial necrosis (mummification) of the pulp. The greatest degree of mummification was seen with the calcium hydroxide-saline paste. The calcium hydroxide-saline paste produ

Pulpectomy: immediate root canal filling with calcium hydroxide. Concept and procedures.

From the clinical aspect the endodontic treatment is divided into two groups: (1) teeth with pulp vitality (biopulpectomy) and (2) pulpless teeth (necropulpectomy). The authors point out the necessity of knowing the clinical pathologic conditions and the macroscopic aspect of the dental pulp before endodontic treatment. They describe the clinical procedures that must be performed in cases when the tooth presents pulp vitality (biopulpectomy), emphasizing (1) the necessity of a field of operation in an aseptic condition; (2) the necessity of a noninjuring instrumentation; (3) the use of cytophy

Corticosteroids in dentistry.

The application of corticosteroids to cut dentine following cavity preparation, to carious dentine in indirect pulp capping, to the exposed pulp or to pulp remnants and periapical tissues during root canal therapy has been claimed to eliminate post operative pain and inflammation. Other investigators have thrown doubt on the validity of these claims and have reported continued inflammation progressing to necrosis following such applications. This paper reports detailed clinical and histological findings in a series of over 500 teeth some of which received conventional treatment while others we

CRISPR-edited DPSCs constitutively expressing BDNF enhance dentin regeneration in injured teeth.

Dental caries, a prevalent global health issue, results from complex bacterial interactions. In response to harmful stimuli, a desirable outcome for the tooth is the formation of tertiary dentin, a protective reparative process that generates new hard tissue. This reparative dentinogenesis is associated with significant inflammation, which triggers the recruitment and differentiation of dental pulp stem cells (DPSCs). Previously, we have demonstrated that brain-derived neurotrophic factor (BDNF) and its receptor tropomyosin receptor kinase B (TrkB), key mediators of neural functions, are activ

Reversible and irreversible painful pulpitides: diagnosis and treatment.

The foregoing clinical evidence indicates that when pain is severe, or when mild to moderate pain is present with a previous history of pain in the aching tooth, with or without periapical radiolucency, the tooth is in the IRPP category. Treatment dictates endodontic therapy or extraction. On the other hand, when clinical evidence indicates that the pain is mild or moderate with no previous history of pain, normal pulp vitality, and there is no positive percussion sign, the pulp is in the RPP category. Treatment dictates indirect or direct pulp capping in teeth with or without periapical radio

Minimally invasive clinical approach in indirect pulp therapy and healing of deep carious lesions.

Indirect pulp treatment is a conservative vital pulp procedure performed in deep carious lesion approximating the pulp, but without signs or symptoms of pulp degeneration. Removing the carious biomass along with sealing the residual caries from extrinsic substrate and oral bacteria makes residual caries after the first excavation less active. This allows time for pulpo dentinal complex to form tertiary dentine so that at the second excavation, there is less likelihood of pulpal exposure. It has also been suggested that by changing the cavity environment from an active lesion into a more slowly

Partial Pulpotomy in Mature Permanent Molars with Symptoms Indicated Irreversible Pulpitis Using MTA: A Study of Three Case Reports over Four-Year Follow-Up.

VPT is vital pulp therapy, a biologically based procedure that combines several therapeutic techniques to maintain the entire or a portion of the dental pulp. Interest in VPT has grown due to recent developments in bioactive materials and an understanding of biological pulp reparative responses. This case report is aimed at evaluating the success rate of partial pulpotomy in permanent molars with symptoms indicating irreversible pulpitis using MTA and presenting with extremely deep carious lesions over four years of follow-up. All patients came with spontaneous and severe pain. Each tooth was

Indirect pulp therapy and stepwise excavation.

Various treatment concepts have been suggested to solve the deep carious lesion dilemma. Recent systematic reviews are presented. Their conclusions are based on very few studies, and the main message is that optimal randomized clinical studies are lacking. Observational studies on indirect pulp treatment and stepwise excavation demonstrate that these treatments avoid pulp exposures, but it cannot be said which approach is best. A less invasive modified stepwise excavation approach is described, focusing on changing on active lesion into on arrested lesion even without performing an excavation

Biocompatibility of an adhesive system applied to exposed human dental pulp.

Human pulp tissue was directly capped with All Bond 2, or calcium hydroxide and evaluated 7, 30, or 60 days after the procedures. Histological analysis was performed to assess the inflammatory cell response, tissue disorganization, dentin bridging, and the presence of bacteria. At 7 days, with All Bond 2 capping, there was a large area of neutrophilic infiltrate underlying the pulp capping material, and the death of adjacent odontoblasts, was observed. However, with time, the neutrophilic reaction was replaced by fibroblastic proliferation with macrophages and giant cells surrounding globules

Direct capping with four different materials in humans: histological analysis of odontoblast activity.

Pulp inflammation in restored teeth is mainly due to the presence of bacteria or bacterial products introduced by microleakage around the restoration or to the material toxicity. Recent knowledge has permitted a precise identification of the risks for pulpal irritation associated with adhesive materials and procedures. The purpose of this work was to evaluate the cellular events that occur in direct pulp exposure capped using different materials. Twenty-four vital teeth without caries, scheduled for extraction for orthodontic reasons, were selected. After a control of the hemostasis, each pulp
