Saving pulps--a biological basis. An overview.

AIM: Cavity preparation and restoration variables have an uncertain relationship to pulp injury and repair responses. The purpose of this paper is to determine the importance of cavity preparation and restoration variables, by ranking them according to their effect on pulp injury (odontoblast survival) and pulp repair (reactionary dentine secretion). METHOD: The seven studies reviewed are all based on the protocols described by Murray, Smith and colleagues in 2000 and 2001. RESULTS: The studies reviewed provide new perspectives on the importance of cavity preparation and restoration variable

Response of the pulp of dogs to capping with mineral trioxide aggregate or a calcium hydroxide cement.

This study was conducted to observe the response of dogs' dental pulp to mineral trioxide aggregate (MTA) and a calcium hydroxide cement when used as pulp capping materials. After the pulps of 30 teeth were exposed, they were capped with either MTA or a calcium hydroxide cement. Histological analysis was performed 2 months after treatment. Results showed a healing process with complete tubular dentin bridge formation and no inflammation in any of the pulps capped with MTA. On the other hand, only five specimens from the calcium hydroxide cement group formed a complete dentin bridge. In this ex

Biocompatibility of Clearfil Liner Bond 2 and Clearfil AP-X system on nonexposed and exposed primate teeth.

OBJECTIVE: Recent studies have demonstrated that acid etching of vital dentin and pulpal tissue does not retard pulpal healing, odontoblastoid cell differentiation, or dentinal bridge formation when the pulp is capped with adhesive resins. The purpose of this study was to evaluate the pulpal response in nonexposed and exposed monkey pulps to treatment with the Clearfil Liner Bond 2 and Clearfil AP-X system. METHOD AND MATERIALS: Class V and Class I cavities in nonexposed and exposed pulps were observed at 7 or 8, 27, and 97 days. RESULTS: There were no differences in pulpal inflammation betw

Healing of pulp exposures: an ultrastructural study.

The fine structure of tissue changes during the first 14 days following pulp exposure and capping with a hard setting Ca(OH)2 cement has been studied. The early changes included hemorrhage and moderate inflammation which were largely resolved during the first week. During the second week differentiation of cells occurred at the wound surface. These cells had the characteristic features of odontoblasts and formed a predentin-like collagen matrix. The capping material was closely adapted to cellular structures at the wound surface or to the predentin-like matrix at all observation periods. Denti

Citrate Transporter NaCT and Enamel Mineralization: The Slc13a5R337* Mouse Model.

Solute Carrier Family 13 Member 5 (SLC13A5) encodes the sodium-dependent citrate cotransporter NaCT, which mediates citrate transport across cell membranes. Pathogenic variants in SLC13A5 cause developmental and epileptic encephalopathy 25 with amelogenesis imperfecta, DEE25; OMIM #615905, a debilitating autosomal recessive disorder. To better define the role of NaCT in ameloblast function and enamel mineralization, we used CRISPR/Cas9 genome editing to generate Slc13a5R337* knock-in mice that terminate NaCT translation at the Arg337 codon, which is homologous to the human SLC13A5R333* variant

Novel Midkine-Derived Peptide Promotes Reparative Dentine Formation via DDIT4/mTOR Pathway-Mediated Activation of Autophagy.

AIM: Midkine (MK) protein plays a critical regulatory role in tooth development and odontogenic differentiation, and has been shown to significantly promote reparative dentine formation. However, the direct application of recombinant MK protein as a pulp capping material presents clinical challenges and may increase medical costs. In contrast, peptides offer a cost-effective and feasible alternative by overcoming many of the limitations associated with full-length proteins. This study aimed to develop a novel MK-derived peptide for use in vital pulp therapy (VPT). METHODOLOGY: The human MK pr

Sympathetic nerves inhibit dental pulp stem cell differentiation and tertiary dentine formation via α1B-adrenergic receptor.

AIM: The dental pulp is extensively innervated. Our previous study showed that the α1B-adrenergic receptor ADRA1B is the primary sympathetic neurotransmitter receptor in human dental pulp stem cells (hDPSCs) and its expression was enhanced in inflamed pulp tissues. The sympathetic nerve system (SNS) negatively impacts the proliferation and migration capacities of hDPSCs via ADRA1B. In this study, we aimed to explore the role of the SNS in odontoblast differentiation of hDPSCs and tertiary dentine formation during dentine-pulp injury repair. METHODOLOGY: We investigated in vitro function of AD

Microleakage in endodontics.

A three-dimensional obturation and complete coronal and apical seal is one the important aims of root canal treatment. Since microorganisms may remain in the root canal system after instrumentation, a tight apical seal is desired to prevent bacteria and their by-products from invading the apex. A perfect apical seal is also desired to prevent apical percolation. One of the major objectives of tooth restoration is the protection of exposed dentine against bacteria and their toxins. The interface between the restoration and dental hard tissue is an area of clinical concern as insufficient sealin

Pathological investigation of caries and occlusal pulpar exposure in donkey cheek teeth using computerised axial tomography with histological and ultrastructural examinations.

Post-mortem examination of 16 donkey cheek teeth (CT) with caries (both peripheral and infundibular) and pulpar exposure were performed using computerised axial tomography (CAT), histology and scanning electron microscopy. CAT imaging was found to be useful to assess the presence and extent of caries and pulp exposure in individual donkey CT. Histology identified the loss of occlusal secondary dentine, and showed pulp necrosis in teeth with pulpar exposure. Viable pulp was present more apically in one exposed pulp horn, with its occlusal aspect sealed off from the exposed aspect of the pulp ho

Immunohistochemical localization of LIM mineralization protein 1 in pulp-dentin complex of human teeth with normal and pathologic conditions.

LIM mineralization protein 1 (LMP-1) is an essential positive regulator of osteoblast differentiation and maturation and bone formation. However, the expression and distribution of LMP-1 in human teeth are largely unknown. The aim of this study was to detect the expression of LMP-1 in normal healthy human teeth and human teeth with various pathologic conditions. LMP-1 expression was determined by immunohistochemistry in all of the samples including 12 healthy teeth, 10 teeth with caries, 12 teeth with pulpitis, and 4 teeth with pulp calcification. We found that LMP-1 was expressed primarily in

Mineral trioxide aggregate pulpotomies: a case series outcomes assessment.

BACKGROUND: The greatest threats to developing teeth are dental caries and traumatic injury. A primary goal of all restorative treatment is to maintain pulp vitality so that normal root development or apexogenesis can occur. If pulpal exposure occurs, then a pulpotomy procedure aims to preserve pulp vitality to allow for normal root development. Historically, calcium hydroxide has been the material of choice for pulpotomy procedures. Recently, an alternative material called mineral trioxide aggregate (MTA) has demonstrated the ability to induce hard-tissue formation in pulpal tissue. The autho

The effect of TGF-beta 2 on dentin apposition and hardness in transgenic mice.

Transforming growth factor beta, TGF-beta, is expressed during tooth formation and can induce pre-odontoblast differentiation and formation of functional odontoblast-like cells in vitro. In addition, exogenous TGF-beta can increase reparative dentin formation, presumably by acting on odontoblasts. In this study, we examined the tooth phenotype of transgenic mice, in which TGF-beta 2 expression is directed by the osteocalcin promoter. Previous studies have shown that these mice have a bone phenotype that resembles that of human osteoporosis, including the existence of spontaneous fractures. Mic

Malignant infantile osteopetrosis: dental effects in paediatric patients. Case reports.

AIM: Malignant Infantile Osteopetrosis is a hereditary pathology caused due to osteoclastic cells which are incapable of carrying out their functions and hence do not resorb osseous tissue where required. Thus the consequence is that during growth phase, the medullary cavities and nervous tissue cavities do not undergo sufficient growth and the corresponding organs do not develop adequately. The aim of this study is to outline the role of the pediatric dentist who has to carry out protocols of primary, secondary, tertiary prevention intervening at many levels. Clinical features and dental effe

Age Estimation Using Monoradicular Teeth by comparing Pulp/Tooth Area Ratios between Bucco-lingual and Mesio-distal Periapical Radiographs.

BACKGROUND: Age estimation is an integral tool in identification of the victims when visual identification is not possible. Secondary dentin deposition is a regular ageing process and can be utilized in dental age estimation when development and eruption parameters cannot be utilized. METHODS: In total, 38 monoradicular teeth of Nepalese subjects were assessed for tooth pulp ratio. Intraoral periapical radiographs of these teeth were taken, and pulp tooth ratio was calculated and dental age was estimated. RESULTS: The mean chronological age was 46.79 years and the mean estimated age was 44.3

Comparison of Microleakage of Different Silver Diamine Fluoride Formulations (Solutions and Gel) with Resin-modified Glass Ionomer Cement Restoration on Human Teeth by Dye Penetration Method Using Stereomicroscopy: An In Vitro Study.

INTRODUCTION: Silver-modified atraumatic restorative therapy (SMART) is the silver diamine fluoride (SDF) treatment followed by glass ionomer cement (GIC) restoration to help arrest the progression of the caries without requiring local anesthesia and cavity preparation. There is no comparison in the literature regarding the microleakage and potential of secondary caries with the aforementioned approach. Also, recently, a new formulation of SDF, that is, SDF gel, is available in the market, which could be a better material due to its ease of application and control over area of application. MA
