Cost effectiveness of melanoma in situ resection and repair by dermatology compared to non-dermatology specialties at a single institution.

Melanoma-in-situ (MIS) is treated with surgical resection by many specialties. Dermatologists perform these procedures in outpatient settings while others often employ operating rooms and general anesthesia. We hypothesized that MIS managed by dermatology was less costly than that managed by other specialties. All cases of MIS treated at our institution over a 3-year period were evaluated retrospectively for demographic and clinical characteristics and categorized by treating specialty. Estimated cost information was determined using records of charges billed. The mean total cost for MIS treat

Retrospective Analysis of Complication Rates Associated With Auricular Reconstruction After Skin Cancer Surgery.

BACKGROUND: There is limited data regarding postoperative complications after microtia surgery or tympanoplasty; however, complication rates after pinna reconstruction following tumor resection and comorbidities associated with suboptimal outcomes have not been investigated so far. OBJECTIVE: To examine the incidence of postoperative complications after auricular reconstruction surgery following tumor resection and determine the association between postoperative complications and defect size, patients' comorbidities, and reconstruction type. METHODS AND MATERIALS: In a retrospective case ser

Part 2: Management of intraoperative and perioperative bleeding.

With an increase in number of patients on antithrombotic therapies, management of bleeding during dermatologic surgery is increasingly important. As described in Part 1, perioperative discontinuation of antithrombotic therapies may increase the risk of embolic events thus the risks and benefits must be weighed carefully when deciding whether to continue or suspend therapy. However, continuing oral anticoagulants may result in increased intraoperative and postoperative bleeding. Here we describe various methods to effectively achieve hemostasis which include (1) mechanical methods to compress t

[Amputation and exarticulation of the lesser toes].

OBJECTIVE: Amputations and exarticulations of the toes may be necessary due to several reasons. The goal is to remove necrosis or infection prior to its spread to the midfoot region. From a functional or cosmetic point of view, amputation/exarticulation of a single toe plays no major role. However, this can be different with exarticulation of several toes. INDICATIONS: Necrosis, trauma, infection, tumor, deformity. CONTRAINDICATIONS: Conditions where amputation/exarticulation of a toe is insufficient, e. g., in progressing peripheral arterial disease. SURGICAL TECHNIQUE: The toe can either

Bridging the gaps: management of lichen planus subtypes in a joint dermatology-oral surgery clinic.

Lichen planus (LP) presents with a range of clinical subtypes. It can affect the outer skin, involve the nails and present with alopecia and mucosal symptoms to varying degrees. LP of the outer skin mostly shows a self-limiting course; however, this is not the case for lichen planopilaris and the mucosa-affecting subtypes. The pathogenesis of LP is still incompletely understood. As a result, an effective, targeted therapy is currently lacking and different immunomodulatory approaches are being used in clinical practice. The management of patients with severe oral LP mucosae can be particularly

Picosecond laser versus historical responses to Q-switched lasers for tattoo treatment.

BackgroundTattooing is increasingly common among adolescents and young adults. This study explores the clinical efficacy of picosecond laser for the correction of tattoos. Literature suggests that 25-47% of the population have had a tattoo at some point in their life (1). The incidence is slightly higher in adults 18-35 years (22-47%) compared to college students (18-25 years of age(1, 2). Recent surveys report 25% of tattooed adults expressing regret, while 4% of tattooed students had already undergone some form of tattoo-removal procedure (3). A common request for tattoo removal includes poo

High-frequency (20 MHz) high-intensity focused ultrasound: New ablative method for color-independent tattoo removal in 1-3 sessions. An open-label exploratory study.

BACKGROUND: High-intensity focused ultrasound (HIFU) operating at 20 MHz is new and potentially applicable to ablative tattoo removal. The method was documented safe and rational in preclinical testing. MATERIALS AND METHODS: High-intensity focused ultrasound was introduced to subjects when lasers and dermatome shaving had failed or caused side effects. Transducers with focal depths between 1.1 mm and 1.7 mm in the skin were used, and settings of 0.4-1.2 J/shot at pulse durations of 150 ms were applied. Tattoos were covered with synergistic "shoulder-by-shoulder" focused ultrasound shots. Eff

Surgical Management of a Chronic Neck Abscess in a U.S. Navy Bottlenose Dolphin.

Surgical intervention on cetaceans is rarely performed due to challenges including general anesthesia and post-operative wound healing. This report describes the evaluation and treatment of an adult female bottlenose dolphin (Tursiops truncatus) with the US Navy Marine Mammal Program, with a chronic ventral cervical abscess caused by Candida glabrata. Despite aspiration and lavage along with multiple antifungal drugs, the patient developed inspiratory stridor with decreased performance level and surgical treatment was pursued. Under general anesthesia with the dolphin in dorsal recumbency posi

Trauma Management of the Auricle.

Smaller injuries of the auricle, such as lacerations without tissue loss, have more or less standardized treatment protocols that require thorough wound closure of each affected layer. Even extended lacerations of larger parts of the ear quite often heal with only minor irregularities. New in vivo diagnostic tools have aided the understanding of this outstanding "skin flap behavior." At the other end of the trauma severity spectrum are partial or complete amputations of the ear. Here, the debate has become more intense over the last decade. There were numerous reports of successful microvascul

[Arsenical keratosis treated by dermatome shaving].

Cutaneous malignancy in association with arsenic exposure is a rare but well-documented phenomenon. Signs of chronic arsenic exposure are very rare in Denmark today. However, arsenic was used in the medical treatment of psoriasis vulgaris up till the 1980's and several patients suffer from this arsenic treatment today. This case report shows that arsenical keratosis can be treated by dermatome shaving, a superficial destructive therapy.

[Transcutaneous identification of cricotracheal and tracheal stenosis].

BACKGROUND: In the reconstruction of laryngotracheal stenosis, the exact localization of the level of the stenosis in relation to the cricoid arch is of paramount importance. This report describes an easy, fast and reliable technique for projecting stenotic segments of the subglottic trachea onto the tracheal front wall. MATERIAL AND METHOD: Directly before the reconstructive procedure, the stenosis is visualized using microlaryngoscopy. An endo-extralaryngeal needle-carrier is used to drive a suture from the inside through the skin. This takes the surgeon directly to the anterior tracheal wa

Q-switched ruby laser treatment of tattoos and benign pigmented skin lesions: a critical review.

The Q-switched ruby laser (694 nm, 25-40 nsec) is an effective and safe therapeutic device for the treatment of tattoos and well-defined, benign, pigmented epidermal and dermal lesions. Because of its selective mode of action, dermal pigments of natural and artificial origin are destroyed photothermically and removed without scar. This method is exceptionally suited for the elimination of lay and professional tattoos, traumatic tattoos, and permanent makeup. Other frequent indications include benign pigmented lesions such as lentigines, freckles, café-au-lait spots, seborrheic keratosis, and B

[Management of soft-tissue defects related to war injuries].

The paper analyzed our experiences with repairing the skin defects in 120 wounded patients in the first war year. After primarily surgical observations patients were hospitalized in the ORL clinic for a further care. Most of wounds were by artillery projectiles. Extensivity, intensivity and primary contamination are three characteristics of war wounds that makes problems in definitely repairing of defects. This is because of the secondary repair 67.80% defects. Spontaneous reepithelization is noticed in 14.80% defects. By direct sutures we closed 16.00%, free skin tissues grafts 23.26%, used v

Hyperinflammatory proliferative (blastomycosis-like) pyodermas: review, mechanisms, and therapy.

Some sites of chronic localized infection have a vigorous granulocytic inflammatory response and develop marked tissue proliferation. This is most common in patients with poor nutrition, halogen ingestion or foreign bodies. The granulocytes release connective tissue-activating peptides which cause fibroblast proliferation and ground substance formation. The dermal papillae enlarge and support a larger germinative cell volume, producing pseudoepitheliomatous hyperplasia. Hypertrophic granulation tissue or "proud flesh" appears to be a similar phenomenon without the epithelial component. In a mi
