Jaccoud's arthropathy in adult dermatomyositis.

A patient with dermatomyositis developed a non-erosive deforming arthropathy of the hands and feet, consistent with Jaccoud's arthropathy. This type of arthropathy has not been described in dermatomyositis or polymyositis.

Spontaneous pneumomediastinum in adult dermatomyositis.

Spontaneous pneumomediastinum has not been reported in adult polymyositis or dermatomyositis, either in conjunction with spontaneous pneumothorax or in isolation. Spontaneous pneumothorax has been rarely reported as a complication of rheumatoid arthritis, systemic lupus erythematosus, scleroderma, and childhood dermatomyositis. It is associated with active, progressive pulmonary involvement and a poor prognosis. We describe an adult with dermatomyositis and spontaneous pneumomediastinum with a favourable outcome.

Drug treatment of juvenile dermatomyositis.

A series of 29 children with dermatomyositis has been reviewed and the outcome compared between cases treated by us initially with a low dosage short duration course of corticosteroids, and those referred late and having had various different therapeutic regimens and usually more active and higher dosage drug schedules. There were fewer relapses and less morbidity in the low dosage short term group. It was noted also that there was no relation between the initial mode of onset or severity and the eventual outcome or course of the disease. However, pronounced skin vasculitis appeared to reflect…

Recalcitrant Dermatomyositis Treated With Anifrolumab.

Dermatomyositis is an idiopathic inflammatory myopathy which can present with distinctive skin features. Despite the many treatment modalities for the treatment of dermatomyositis some patients remain refractory to treatment. We present a case of a 38-year-old man with recalcitrant dermatomyositis who was successfully treated with the interferon α receptor 1-inhibiting monoclonal antibody anifrolumab. Many studies have shown a correlation between dermatomyositis disease activity and the type 1 interferon pathway activity. We believe anifrolumab may be a promising option for severe cutaneous de…

Promising Treatments in Juvenile Dermatomyositis.

Juvenile dermatomyositis (JDM) is an autoimmune systemic connective tissue disorder treated by various anti-inflammatory and immunomodulating agents. JDM typically requires immunomodulatory therapies including corticosteroids, disease-modifying antirheumatic drugs, intravenous immunoglobulins, and other steroid-sparing therapies. It is important to also employ nonpharmacologic interventions, which are integral in the treatment of JDM to optimize outcomes. These include photoprotection, physical therapy, occupational therapy, and mental health support. There are increasing number of exciting, e…

[Anti-MDA5 dermatomyositis. Literature review].

Dermatomyositis positive anti-melanoma differentiation-associated gene 5 (anti-MDA5 DM) is a rare disease that represents less than 2%. The prevalence of anti-MDA5 DM ranges from 7 to 60%, with higher prevalence in Asian (11-60%) and women. The clinical picture may be variable and is accompanied by the typical features of dermatomyositis, such as periorbital heliotrope (blue-purple) rash with edema, erythematous rash on the face, or the anterior chest (in a V-sign), and back and shoulders (in a shawl sign), violaceous papules or plaques located on the dorsal part of the metacarpophalangeal or …
