Nox4 Mediates Diastolic Function in a Genetic Model of Pitx2 Haploinsufficiency.

Heart failure with preserved ejection fraction (HFpEF) commonly coexists with atrial fibrillation (AF), but shared mechanisms remain unclear. In this study, we hypothesized that Pitx2, a transcription factor located near the strongest genetic locus associated with AF in humans, increases susceptibility to HFpEF-like remodeling. We also sought to understand pathways that might be central to this increased risk. Male and female Pitx2+/- mice and wild-type littermates received 3-week subcutaneous osmotic pump infusion of saline or angiotensin II (Ang II; 500 ng/kg/min). Cardiac structure and func

A simplified, tier-based method for clinical evaluation of diastolic function.

Assessment of left ventricular diastolic function is inherently complex, yet it must be sufficiently simplified for consistent application in clinical practice. Interpretation is challenged by overlapping terminology, load dependence of haemodynamic variables, and the need to integrate multiple echocardiographic parameters that are often discordant. This review presents a simplified, clinically oriented approach to applying contemporary imaging guidelines for diastolic function assessment, with a focus on the 2025 American Society of Echocardiography recommendations. A probabilistic diagnostic

Agentic Autodiscovery of Diastolic Dysfunction Phenotypes from Surface Electrocardiogram.

BACKGROUND: Left ventricular diastolic dysfunction (LVDD) is a major determinant of heart failure (HF), yet its assessment relies on multiparametric echocardiography, limiting scalability. We previously demonstrated that generative artificial intelligence (AI) can synthesize tissue Doppler imaging (TDI) waveforms from the 12-lead ECG. The growing complexity of candidate architecture creates a need for automated model-discovery frameworks. OBJECTIVES: To evaluate agentic AI-based auto-discovery for ECG-based LVDD assessment using either raw ECG or synthetic TDI waveforms. METHODS: Two attenti

Diastolic Dysfunction Reclassification Following Implementation of the 2025 Echocardiographic Guidelines.

BACKGROUND: Echocardiographic grading of diastolic dysfunction influences diagnosis, risk assessment, and longitudinal management in cardiovascular disease. PROJECT RATIONALE: Updated 2025 diastolic dysfunction criteria emphasize integrated assessment of transmitral inflow, tissue Doppler indices, left atrial volume, tricuspid regurgitation velocity, and filling pressure estimates. Their implementation may alter classifications assigned using older criteria. PROJECT SUMMARY: We performed a retrospective quality improvement analysis of 99 echocardiograms obtained from Syngo, a cardiovascular

Reversible Fibroblast Trajectories Regulated by MR Underlie Diastolic Dysfunction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a major health issue of our time. The pathophysiology of HFpEF is diverse, linked to comorbidities, such as kidney disease or obesity, and different from heart failure with reduced ejection fraction. As a consequence, treatment options are still limited. Recent clinical trials indicate beneficial effects of MR (mineralocorticoid receptor) antagonists in HFpEF, but the underlying mechanisms are incompletely understood. METHODS AND RESULTS: We treated male and female mice with aldosterone and a high salt-diet to induce a prec

Focusing on Diastolic Dysfunction to Achieve Consensus With the Term Diabetic Cardiomyopathy.

Diabetic cardiomyopathy (DbCM) is commonly defined as ventricular dysfunction in a person living with diabetes in the absence of hypertension and/or coronary artery disease (CAD). There is increasing recognition in the field that this is a poor definition to reflect the pathology of DbCM, as many individuals with diabetes (both type 1 and type 2) are often at risk of or have coexisting hypertension or CAD. On the contrary, there is increasing evidence that people living with diabetes, particularly in the early stages of disease progression, often have asymptomatic and subclinical diastolic dys

Isolated Diastolic Hypertension and Cardiovascular Disease Risk in Young Adults.

BACKGROUND: Whether isolated diastolic hypertension (IDH) in young adults represents a benign, age-specific phenotype or confers increased cardiovascular disease risk remains uncertain. We assessed the associations between hypertension subtypes, IDH, isolated systolic hypertension, and systolic-diastolic hypertension, with atherosclerotic cardiovascular disease and heart failure. We also characterized longitudinal transitions in hypertension subtypes from young adulthood into later life. METHODS: Adults aged ≥18 years without cardiovascular disease were included from 4 US cohorts. Hypertensio
