Bullous skin lesions that indicate sweat gland necrosis have been known to occur in drug-induced coma resulting from barbiturates and in carbon monoxide poisoning. To our knowledge, this is the first cases in which diazepam is implicated in causing bullous lesions over pressure points, and the first case showing on biopsy specimen eccrine sweat gland and sweat duct necrosis. The pineal hormone melatonin (0.1 mg/kg) produced a more pronounced antistressor effect as compared to the anxiolytic diazepam (same dose) in rats, which was manifested by decreased anxiety, improved heart rhythm variability, and prevented stress-induced violation of circadian locomotion. Forty patients over 65 undergoing subarachnoid anesthesia with bupivacaine 0.50% (5 mg) and fentanyl (0.1 mg) were subdivided into two equal groups: one was premedicated with atropine and chlordesmethyldiazepam (0.03 mg/kg-1) and the other with atropine and diazepam (0.015 mg/Kg-1). A statistically significant difference was found in the group treated with diazepam which required an increase for anesthetic drugs during surgery. The authors suggest a probable synergic or an enhanced effect between intramuscular chlordesmethyldiazepam and opiates in spinal anesthesia.

The acute onset of chorea was seen in a ten week-old male following abrupt cessation of a seven-day course of sedation with diazepam and pentobarbital. These movements slowly resolved over a six-day period and the infant was without neurologic sequelae one week after his chorea resolved. A cerebral MRI was normal. We speculate that the mechanism by which chorea may occur as a withdrawal phenomenon to benzodiazepine/barbiturate treatment is by a functional overactivity of the dopaminergic system in the basal ganglia.
