Uterine didelphys is a rare Müllerian anomaly occurring in approximately 0.1-0.5% of women. It is characterised by duplicated uterine and cervical structures and presents significant diagnostic and surgical challenges. Uterine didelphys is a rare congenital anomaly of the female reproductive organs, designated by the presence of the uterus as a pair of organs. This occurs as a result of the failure of the embryonic fusion of Müllerian ducts. Women with this abnormality have a paired uterus with two cervices and usually a double vagina. PURPOSE: The purpose of this work was to determine the association of renal agenesis with the different types of mullerian duct anomalies (MDAs).
METHOD: A 5 year retrospective review of MR records identified 57 patients with MDAs. Associated renal anomalies were correlated with the various types of MDAs.

CASE: We present a patient with a uterine didelphys with two cervices in a single vagina after resection of a longitudinal vaginal septum at age 12. She presented with abnormal Pap test results with the right and left cervices discordant from each other. Despite both cervices sharing stroma in the midline, her right cervix was negative for human papillomavirus (HPV) and dysplasia, while her left cervix was HPV positive with high-grade cervical dysplasia on an excisional specimen.

Uterine didelphys are rare malformations involving the Mullerian ducts. The incidence ranges from 0.1% to 3.8%. This wide range could be because of inaccurate diagnosis or to the fact that many of these diagnoses are not detected during the women's lifetime. Here, we report the management of a 16-year-old female patient who had uterine didelphys with obstructed hemivagina, resulting in obstructive hydronephrosis in her transplanted kidney.

Asymmetric obstructed uterus didelphys (Herlyn-Werner-Wunderlich syndrome) is a rare congenital müllerian anomaly consisting of uterus didelphys, hemivaginal septum, and ipsilateral renal agenesis. Herein is reported a case of incomplete Herlyn-Werner-Wunderlich syndrome diagnosed using 3-dimensional transvaginal ultrasound in a 14-year-old patient with absence of the hemivaginal septum.
