A case of non-specific granulomatous prostatitis is reported. The patient, who had a history of sudden onset of high fever and acute urinary retention, had a hard prostate on digital rectal examination that gave us an impression of prostatic cancer. Since repeated biopsy specimens from the prostate showed granuloma formation with fibrinoid necrosis, the case was diagnosed as non-specific granulomatous prostatitis. Steroid therapy promptly resolved clinical symptoms along with marked histopathologic improvement.

We report non-specific granulomatous prostatitis in a 58-year-old man who visited a Urology clinic with fever and dysuria. A 65-year-old woman was admitted to our hospital for evaluation of multiple liver tumors following endoscopic polypectomy for rectal polyp a year earlier. The polypectomy specimen was reexamined and showed inclusion of a carcinoid tumor 3 mm in diameter. The perirectal lymph node was palpated by digital examination on readmission. The patient died one mouth after readmission due to liver failure. Autopsy revealed metastatic carcinoid tumor in the liver and perirectal nodes.

Adenocarcinomas that arise from primary or secondary prostatic ducts have distinctive histopathologic features. The age of patients, symptoms, findings on digital rectal examination and determinations of serum acid and alkaline phosphatase are similar to those of patients with acinic carcinomas. Carcinomas of secondary ducts may be less responsive to endocrine manipulation and of greater malignancy than carcinomas of primary ducts. The course and survival of patients with ductal carcinomas treated conservatively are poor.
