Since several years percutaneous balloon dilatation has gained more and more importance as a method circumventing thoracotomy. Induced by the results in the international literature [1-14] we performed successfully percutaneous pulmonary balloon dilatations in 8 patients between June 1988 and March 1990. Three patients (aged 6.5 to 27 years) had an isolated, prevailing valvular pulmonary stenosis, while the five others had a complex congenital anomaly with valvular and subvalvular pulmonary stenosis. The paper reports the results of pulmonary balloon dilatations in the latter five patients.

The case described in this report is typical of a patient with a choledochal cyst. Although common in Japan, such cases are being increasingly reported in other countries. Because diagnosis depends most importantly on clinical suspicion, detailed history taking and appropriate early intervention are mandated. Additional radiologic studies may help confirm the diagnosis. The crucial step in management is deciding between cyst excision and internal drainage. The ultimate decision depends on the individual patient profile. Long-term follow-up is mandated by the increased risk of biliary carcinoma

The follow-up study has been performed in 4 male patients with nephrogenic diabetes insipidus lasting for 4 to 15 years. Hydronephrosis or hydroureter appeared to be caused by polyuria with or without urinary tract obstruction. The efficacy of drainage operation is emphasized when polyuria surpasses the transporting ability of urine in the urinary tract.

A case is reported of an unusual complication in which acute gastric dilatation occurred in an incarcerated hiatus hernia, resulting in left ventricular failure in the postoperative period. Failure to recognize this gave rise to concern when the patient re-presented for further surgery.

There has been widespread use of periodic dilatations in the management of subglottic stenosis. However, some authorities have questioned the value in the overall rehabilitation of patients affected by this disorder. Angiographically equivocal aneurysmal dilatation at the junction of the internal carotid and the posterior communicating arteries was noted in a patient with documented subarachnoid hemorrhage. This was initially thought to be infundibular dilatation. A craniotomy revealed an actual aneurysm, which was obliterated successfully. Whether infundibular dilatation is a preaneurysmal state or a benign dilatation is controversial. The management of angiographically noted infundibular dilatation in cases of "essential" subarachnoid hemorrhage is discussed.

Anal sphincter dilatation with forefinger and middle fingers of both hands for a two-minute period under strong pressure results in a measurable tonus-decrease of the sphincter ani over a period of 2-3 weeks. This tonus-decrease is of therapeutic advantage in numerous diseases, particularly in those of the anal area. At the same time, the stretching may well be used to protect anastomoses in the colon or to dilate the operative access in transanal operations. Complications arising from the stretching described are insignificant.
