Objective: To evaluate the safety and efficacy of a new domestic distal perforated stent graft (Talos stent) in the treatment of Stanford type B aortic dissection (TBAD). Methods: Twenty-five patients with TBAD treated with Talos stent in Yan'an Hospital Affiliated to Kunming Medical University from February 2018 to December 2019 were selected as the research subjects. Intraoperative angiography was performed to determine the number of branch arteries that remained after stent release. On postoperative day 5 (POD5), the pain intensity of the patients was evaluated by visual analog scale (VAS).

OBJECTIVES: The critical step in total endovascular aortic arch repair is to ensure alignment of fenestrations with, and thus maintenance of flow to, supra-aortic trunks. This experimental study evaluates the feasibility and accuracy of a double-fenestrated physician-modified endovascular graft [single common large fenestration for the brachiocephalic trunk and left common carotid artery and a distal small fenestration for left subclavian artery (LSA) with a preloaded guidewire for the LSA] for total endovascular aortic arch repair.
METHODS: Eight fresh human cadaveric thoracic aortas were ha

An 83-year-old woman was injured in a traffic accident. Enhanced computed tomography (CT) showed aortic injury on the isthmus, Stanford type A aortic dissection and intracranial hemorrhage. Neurological deficit was not noted. We immediately started to lower the blood pressure, and her hemodynamic status remained stable. Although intracranial bleeding had not worsened on the next day, a false lumen of the aortic arch was newly enhanced on CT. Emergency total arch replacement with frozen elephant trunk was performed under deep hypothermia and selective cerebral perfusion. Intraoperative angiogra

OBJECTIVE: In Marfan syndrome (MFS) patients, endovascular repair carries a risk of aortic wall injury, which may result in retrograde aortic dissection, dilatation, or false aneurysm at the landing zones. It was hypothesised that potentially these complications may be avoided using modified practices. This study aimed to describe experience of a specific protocol for endovascular aortic repair in patients with MFS.
METHODS: All MFS patients treated by aortic endovascular repair between February 2015 and August 2018 were included prospectively. The following rules were applied: (i) excluding

OBJECTIVES: To evaluate early and mid-term clinical outcomes and to assess the potential of the frozen elephant trunk technique to induce remodelling of downstream aortic segments in acute and chronic thoracic aortic dissections.
METHODS: Over a 4-year period, 65 patients (48 men, aged 61 ± 12 years) underwent total aortic arch replacement using the frozen elephant trunk technique for acute (n = 31) and chronic (n = 34) thoracic aortic dissections at our institution. We assessed diameter changes at 3 levels: the L1 segment at the stent graft level; the L2 segment at the thoraco-abdominal tran

OBJECTIVE: To evaluate the efficacy of endovascular aortic repair plus chimney technique in the treatment of Stanford type B aortic dissection involving aortic arch.
METHODS: From June 2009 to March 2012, 32 patients of aortic arch dissection with primary entry tear next to the orifices of supra-aortic arteries were treated with chimney technique. Chimney technique was used to reconstruct left subclavian artery (n = 2) and left common carotid artery (n = 28). Double chimney technique was use to reconstruct innominate artery and left common carotid artery simultaneously in 2 patients.

A 42-year-old male with Marfan syndrome, who had undergone aortic root and total arch replacement for type-A acute aortic dissection at the age of 40, underwent descending aorta replacement with woven Dacron (Vascutek Ltd., Renfrewshire, Scotland) because of pseudoaneurysm at the site of the distal anastomosis and an enlarged pseudolumen of the dissecting descending aorta. The fourth and eighth ribs were cut at their anterior and posterior sites to allow wide exposure of the entire descending aorta. Postoperative computed tomographic scanning showed that the vascular prosthesis posteriorly con

Patients with aneurysmal dissections involving both the thoracic and the abdominal aorta are particularly challenging to treat with endovascular techniques because of the natural communications at the level of the visceral arteries. We present the case of a patient with Marfan syndrome with an aneurysmal aortic dissection involving the thoracic and abdominal aorta who was treated by a combination of endografts, surgical bypass, and a fenestrated tube graft.

Usually blood does not stagnate nor clot in its vessels. Spasm in a vein or artery, constricting the wall or sphincter, seines and assembles the ingredients of a thrombus. It further leads to rupture of the vessel's wall, so extravascular glue can complete or augment the recipe. The mechanisms of blood clotting, include three prime factors, stasis, dessication and extravascular glue. Treatment of facial nerve disorders is based in part on assumptions regarding the intrinsic blood supply of the nerve. This study was designed to comprehensively delineate the intrinsic facial nerve microcirculation and its relation to the extrinsic circulation in an animal model. Twenty-eight cat facial nerves were removed intact from brain stem to stylomastoid foramen following intravital fixation. Specimens were studied by gross dissection, silicone injection and tissue clearing, complete vessel counts on serial cross sections of individual nerves, and scanning electron microscopy or transm

In this case report, sudden cardiac death caused by intussusception of a coronary artery is discussed. A 47-year-old man was found dead in the nursing home where he lived, following an episode of polyphagia and two of vomiting. Upon cadaveric dissection, an overdistention of the large intestine was noted. Re-evaluation of the formalin-fixed whole heart revealed occlusion of the circumflex branch of the left coronary artery, which was not macroscopically attributable to vascular thrombosis or an atheromatous plaque. Histological investigations revealed ischaemic-type histological changes of the

BACKGROUND: Acute Type A Aortic Dissection (ATAAD) involving the aortic root is managed with either the Bentall or valve-sparing David procedure, but the optimal approach remains debated. Prior meta-analyses are limited by outdated cohorts, indirect comparisons, and high heterogeneity. We conducted an updated meta-analysis of recent studies (2020–2025) comparing David and Bentall procedures, incorporating reconstructed individual patient data to assess long-term survival. METHODS: A systematic search identified studies directly comparing the David and Bentall procedures in ATAAD. Statistical a

BACKGROUND: Intracranial arterial dissection (ICAD) and intracranial atherosclerotic stenosis (ICAS) are often difficult to differentiate, and studies on their background factors and prognosis are scarce. Information on prognosis, including recurrence, is necessary for stroke care, and clarification of epidemiological and clinical differences between the two diseases is important for appropriately handling their heterogeneity. This study aimed to determine the association of ICAD and ICAS with in-hospital recurrence and prognosis and compare their background and clinical findings.

False lumen thrombosis (FLT) in type B aortic dissection has been associated with the progression of dissection and treatment outcome. Existing computational models mostly assume rigid wall behavior which ignores the effect of flap motion on flow and thrombus formation within the FL. In this study, we have combined a fully coupled fluid-structure interaction (FSI) approach with a shear-driven thrombosis model described by a series of convection-diffusion reaction equations. The integrated FSI-thrombosis model has been applied to an idealized dissection geometry to investigate the interaction b

OBJECTIVE: Radical vaginal trachelectomy is the fertility-preserving surgery for patients with early stage cervical cancer. However, it has not gained widespread approval by gynecologic oncologists because of difficulties in the dissection of the bladder pillars and identification of the ureter during the vaginal portion of the surgery.
METHOD: We describe a modification of radical vaginal trachelectomy for easier dissection of the bladder pillar. Following pelvic lymphadenectomy, the vesicovaginal space is widely opened laparoscopically. After identification of the uterine arteries, one shou
