Possible Contribution of a Diverticulum to the Development and Rupture of Colonic Lymphangioma.

A 55-year-old Japanese man with a history of diverticulitis underwent colonoscopy for careful evaluation of progressive anemia. A 5-mm depressed lesion oozing spontaneously was observed at the hepatic flexure. On suspicion of depressed-type of cancer, right-sided hemicolectomy was performed. Histopathological examination indicated a collapsed lymphangioma exactly over a diverticulum, which had previously been complicated diverticulitis. The colonic mucosa and lymphangioma prolapsed beyond the subserosal layer via the muscularis propria defect, resulting in a depressed lesion and mucosal lacera

Accuracy and reliability of preoperative CT in perforated diverticulitis: Post hoc secondary analysis from the SCANDIV trial.

BACKGROUND AND AIMS: Undetected cancers and fecal contamination in perforated diverticulitis may lead to failure of laparoscopic lavage. This study aimed to evaluate the diagnostic accuracy of preoperative computed tomography (CT) examinations and the interobserver agreement for detecting colonic malignancy and fecal contamination in patients with perforated diverticulitis. METHODS: This study included 198 patients with suspected perforated diverticulitis from the SCANDIV trial, randomized to laparoscopic lavage or primary resection. CT examinations of all patients were independently reviewed

Diverticular Disease: A Review on Pathophysiology and Recent Evidence.

Diverticular disease is common condition globally, especially in Western countries. Diverticulitis, Symptomatic uncomplicated Diverticular disease and Segmental Colitis associated with diverticula constitute diverticular disease. Although most patients with diverticula are asymptomatic, around 25% of patients will experience symptoms whilst 5% of patients have an episode of acute diverticulitis. The prevalence increases with age with more than one theory being put forward to explain its pathogenesis. Faecolith entrapment in diverticula results in colonic mucosal damage and oedema, bacterial pr

Small bowel diverticulitis: an imaging review of an uncommon entity.

In this review, we discuss the clinical and radiologic findings of small bowel diverticulosis, which is infrequently encountered during practice and far less common than colonic diverticulosis. Small bowel diverticulosis can present with a range of emergent symptomatic complications including diverticulitis, perforation, or hemorrhage. Here, we focus on the clinical features, pathogenesis, radiologic findings, and treatment of small bowel diverticulitis. Although not routinely considered in the differential diagnosis of an acute abdomen, prospective radiologic diagnosis of small bowel divertic

[Sigmoid diverticulitis].

Colonic diverticulitis has become a relatively frequent disease, with the sigma being affected by up to 95 per cent of all cases. Its clinical relevance has proved to depend on the prognosis of life-threatening diverticulitis complications. Sixty patients who had received surgical treatment at the Surgical Department of Potsdam District Hospital were analysed. That analysis together with evaluation of literature has been used for therapeutic recommendations. For cases of sigmoid diverticulitis early elective intervention is advised. The method of Henri Hartmann is considered an effective appro

Diverticulitis: A Review.

IMPORTANCE: Diverticulosis is defined by the presence of multiple outpouchings (diverticula) originating from the intestinal lumen. Diverticulitis is defined as inflammation of these diverticula. The annual incidence of diverticulitis in the US is approximately 180 per 100 000 people, resulting in approximately 200 000 hospital admissions annually and an estimated health care expenditure of more than $6.3 billion/year. OBSERVATIONS: Risk factors for diverticular disease include age older than 65 years, genetic factors such as variant in the tumor necrosis factor superfamily member 15 (TNFSFI5

Predictive factors on CT imaging for progression of uncomplicated into complicated acute diverticulitis.

PURPOSE: Since outpatient treatment and omitting antibiotics for uncomplicated acute colonic diverticulitis have been proven to be safe in the majority of patients, selection of patients that may not be suited for this treatment strategy becomes an important topic. The aim of this study is to identify computed tomography (CT) imaging predictors for a complicated disease course of initially uncomplicated acute diverticulitis. METHODS: CT imaging from a randomized controlled trial (DIABOLO study) of an observational vs. antibiotic treatment strategy of first-episode uncomplicated acute divertic

Long-term follow-up after an initial episode of diverticulitis: what are the predictors of recurrence?

PURPOSE: The purpose of our study was to determine the clinical and CT predictors of recurrent disease after a first episode of diverticulitis that was successfully managed nonoperatively. METHODS: We retrospectively analyzed 954 consecutive patients who presented to our institution with diverticulitis from 2002 to 2008. Patients were identified with International Classification of Diseases, 9th Revision/Current Procedural Terminology codes. Patients were excluded if they had subsequent colectomy based on the first attack (n = 81), or if the attack they had between 2002 and 2008 was not their

Effectiveness of clinical guidelines in the management of acute sigmoid diverticulitis. Results of a prospective diagnostic and therapeutic clinical trial.

BACKGROUND: Evidence-based criteria in the therapeutic choice for sigmoid acute diverticulitis (AD) are lacking. It is necessary to differentiate an acute episode of diverticular disease, not complicated (NCAD) and complicated (CAD) because these stages of diverticular disease needs different approach. METHODS: In a prospective study on 377 consecutive patients admitted for AD, 265 had NCAD and 112 CAD diagnosed with CT scan. Thirty-six of 265 with NCAD were operated on due to two or more previous episodes of AD. On 188 patients with NCAD followed-up, 35 had further episodes of NCAD and 2 had

Right-side diverticulitis.

Colonic diverticula develop at specific weak spots, where the vasa recta enter the colonic circular smooth muscle layer.1 They are usually seen in the left colon. Their most common complication is diverticulitis, with mild cases resolving even without antibiotic therapy.2 Right-side diverticulitis develops in only 1.5% of cases, primarily on the anterior aspect of the cecum, proximal to the ileocecal valve (80%).4 Given its low incidence, location, and the fact that it involves younger patients, a differential diagnosis is needed to rule out abdominal inflammatory conditions such as appendicit

Role of endoscopy in patients with diverticular disease.

Acute diverticulitis occurs in 10-25% of patients with diverticulosis. Colonoscopy is advised 6 weeks after an attack of acute diverticulitis in order to completely evaluate the colonic lumen and exclude a potential malignancy. We conducted several studies aimed to evaluate the feasibility and safety of early colonoscopy in patients with acute diverticulitis. Consecutive patients hospitalized for acute diverticulitis were included. In the first phase of the study, patients with adjacent peri-diverticular air/fluid on CT were excluded. In the second phase of the study, we included patients with

Colocutaneous fistula complicating sigmoid diverticulitis.

Colocutaneous fistula is a very rare complication of colonic diverticular disease. Herein we describe a case with a fistula connecting the sigmoid with the left flank area complicating diverticulitis of the sigmoid colon. An 85-year-old female patient with a history of acute diverticulitis 3 months earlier, was admitted with a subcutaneous abscess in the left flank. The abscess was drained and subsequently a colocutaneous fistula was established. At operation the sigmoid colon with the fistulous tract were excised. The patient had no postoperative complications and at present, 2 years later, s

[Surgical treatment of diverticulitis: the current state of affairs].

Diverticulitis is a common benign gastrointestinal condition with a variable clinical course, ranging from mild, self-limiting episodes to severe presentations requiring surgery. This article presents two contrasting cases of diverticulitis requiring surgery: one of recurrent, uncomplicated diverticulitis, and another of acute diverticulitis with a covered perforation progressing to purulent peritonitis. These cases illustrate the broad spectrum of disease severity and the evolving, individualized approach to management. Elective sigmoid resection may improve quality of life in patients with c

Perforated Jejunal Diverticulitis As An Uncommon Culprit Of Acute Abdomen: A Case Report And Review Of Literature.

Small intestinal diverticula are rare compared to colonic diverticula. Jejunal diverticulosis can occur in older men. These diverticula are usually asymptomatic but can present with acute abdomen when symptomatic. Complicated cases can present with perforation, peritonitis, and abscess formation. CT scan is the ideal imaging modality to diagnose perforated jejunal diverticulitis. Complicated cases warrant surgical intervention. A high clinical suspicion is necessary for the timely diagnosis of perforated diverticulitis. We are presenting a case of a 45-year-old who presented with an acute abdo

Pylephlebitis With Splenic and Mesenteric Vein Thrombosis in a Patient With Diverticulitis.

Diverticulitis is a common gastrointestinal complaint that refers to inflammation of colonic diverticula. Its incidence has increased partly due to the increase in prevalence of diverticulosis, which results from poor dietary habits and chronic constipation. An acute diverticulitis episode can vary in severity, ranging from outpatient management of mild abdominal discomfort to inpatient admission requiring emergent surgery. Some common complications associated with diverticulitis include bowel wall perforation, microperforation, abscess formation, bowel obstruction, and colonic fistulas. A les

Colonic obstruction due to sigmoid muscular hyperplasia.

We report a case of acute colonic obstruction initially presumed to be secondary to acute diverticulitis, necessitating emergent surgical intervention. Pathologic examination failed to reveal evidence of inflammation, fibrosis or neoplasia. Marked hypertrophy of the sigmoid circular muscle layer was documented and thought to be the etiology of the colonic obstruction.

[Clinical analysis of 554 patients with colorectal diverticulosis].

Objective: Most patients with asymptomatic colorectal diverticulosis are easily overlooked. However, some of diverticulosis become diverticulitis, bleeding and even perforation, which cause extensive harm to patients. The purpose of this study is to analyze the incidence, clinical features, diagnosis and treatment of colorectal diverticulosis in order to improve the clinical understanding of diverticulosis and its related complications. Methods: A descriptive cohort study was carried out. Clinical data of 554 patients with colorectal diverticulosis confirmed by CT, colonoscopy, digestive tract

[Emergency surgical treatment of complicated acute diverticulitis].

Twenty-five percent of patients undergoing surgery for acute complicated diverticulitis represent emergencies. This condition is currently treated by colonic resection with primary anastomosis with or without colostomy, or by a Hartmann operation. We report on our experience with 52 consecutive patients with generalized peritonitis (8 cases), peri- and paracolonic abscesses (19 cases), severe pelvic abscesses (12 cases) and multiple abscesses with visceral fistulas (13 cases). All patients had emergency surgery. In 50/52 patients (96.2%) we performed a colonic resection with primary anastomosi

Limitations in the CT diagnosis of acute diverticulitis: comparison of CT, contrast enema, and pathologic findings in 16 patients.

Pitfalls in CT diagnosis of acute diverticulitis were investigated in 16 patients with the disease who had misleading or equivocal CT features. The CT appearance was correlated with contrast enemas (13 cases) and with surgical assessment and pathologic evaluation of resected specimens (12 cases). Limitations in the CT diagnosis were related to (1) marked thickening of the colonic wall, between 1 and 3 cm in cross section simulating colonic neoplasm (all patients); (2) inability to visualize small amounts of fibropurulent exudate in the absence of pericolic inflammatory changes (eight patients)
