[Microbiological control of drug resistance of causative agents in suppurative-septic infections in newborn infants].

Resistance of the main causative agents of purulent septic infections such as pneumonia, meningitis, sepsis, etc. to 11 chemotherapeutic drugs was studied. The pathogens were isolated from autopsies of 203 newborns who died within 1981 to 1987. Among 2978 isolates belonging to Enterobacteriaceae (2466 strains) and Pseudomonadaceae (512 strains) which constituted 88% of all the isolates, strains with multiple resistance predominated: 90% of the strains resistant to 4 or more antibiotics, 77% of the strains resistant to 6 or more antibiotics and 48% of the strains resistant to 8 or more antibiot

Drug resistance in leprosy.

Leprosy is caused by Mycobacterium leprae. Currently, leprosy control is mainly based on WHO-recommended multi-drug treatment; thus, emergence of drug resistance is a major concern. M. leprae isolates resistant to single and multiple drugs have been encountered. In this review, the history of chemotherapy and drug resistance in leprosy and molecular biological insights for drug resistance are described. New methodologies to test susceptibility to anti-leprosy drugs instead of the traditional mouse footpad method are introduced. Awareness of the need to monitor drug resistance to prevent the sp

[Research progress on mitochondrial dysfunction in multiple myeloma].

Multiple myeloma ranks as the second most prevalent hematological malignancy. It remains incurable and is characterized by frequent drug resistance and disease relapse. Mitochondria, as a key hub for cellular energy metabolism and homeostasis regulation, have been found to have functional abnormalities in various cancers. The occurrence, progression and drug resistance of multiple myeloma are also closely linked to mitochondrial dysfunction. Existing research has shown that multiple myeloma cells acquire survival advantages and escape therapeutic elimination by enhancing oxidative phosphorylat

Multidrug resistance in cancer: current understandings and future perspective.

Multidrug resistance (MDR) remains the core clinical barrier limiting the achievement of durable and effective treatment for various tumors. With the deepening of understanding, MDR has been redefined from the traditional, static "drug efflux" model to a systemically adaptive survival strategy driven by highly heterogeneous tumor populations under therapeutic pressure, characterized by dynamic evolution. This complex phenotype arises from the deep intertwining and synergistic action of multi-layered survival networks. Herein, this review systematically delineates the evolution and multidimensi

Counteracting antibiotic resistance enzymes and efflux pumps.

Bacterial pathogens are constantly evolving new resistance mechanisms against antibiotics; hence, strategies to potentiate existing antibiotics or combat mechanisms of resistance using adjuvants are always in demand. Recently, inhibitors have been identified that counteract enzymatic modification of the drugs isoniazid and rifampin, which have implications in the study of multi-drug-resistant mycobacteria. A wealth of structural studies on efflux pumps from diverse bacteria has also fueled the design of new small-molecule and peptide-based agents to prevent the active transport of antibiotics.

Antibacterial activities with the structure-activity relationship of coumarin derivatives.

The development of drug-resistant bacteria, as well as multidrug-resistant pathogens related to substantial mortality, is an important global health threat. The wide range of biological activities and the wealthy use of coumarin-containing drugs in the clinic have encouraged more and more interest in this class of heterocycles. Various coumarin-based antibiotic hybrids have been developed in the last decade and most of them exhibited potential antibacterial potency. This present review summarizes the current innovations of coumarin-based derivatives with potential antibacterial activities agai

Nanocarriers based on vitamin E-TPGS: Design principle and molecular insights into improving the efficacy of anticancer drugs.

α-Tocopheryl polyethylene glycol succinate (vitamin E-TPGS or TPGS) is a nonionic amphiphile synthesized by the esterification of vitamin E succinate. It has been categorized as a safe excipient by US-FDA and EMA. Like other polyethylene glycol (PEG) condensates, TPGS spontaneously forms kinetically stable core-shell micelles (diameter 12-15 nm) and exhibits interesting properties as solubilizing agent, emulsifier, dispersant and gelling agent. Its aggregation behavior can be tuned in association with other amphiphiles and organic additives. These properties have been exploited for developing

Extensively drug-resistant tuberculosis: "there must be some kind of way out of here".

Over the past 7 decades, Mycobacterium tuberculosis has developed resistance to virtually every new drug used to treat tuberculosis, resulting recently in the global emergence of extensively drug-resistant tuberculosis. In an individual, treatment with a single new drug results in acquired drug resistance within weeks to months. On a population basis, the pattern is just as consistent. After a new drug is introduced, drug-resistant cases or case series are reported within months to years, typically leading to focused surveys, and within several years, dramatic outbreaks with extraordinary mort

Changing trends in childhood tuberculosis.

Several changes have been observed in the epidemiology, clinical manifestations, diagnostic modalities and treatment of tuberculosis. Emergence of HIV epidemic and drug resistance have posed significant challenges. With increase in the number of diseased adults and spread of HIV infection, the infection rates in children are likely to increase. It is estimated that in developing countries, the annual risk of tuberculosis infection in children is 2.5%. Nearly 8-20% of the deaths caused by tuberculosis occur in children. Extra pulmonary tuberculosis has increased over last two decades. HIV infec

Best in class: a good principle for antibiotic usage to limit resistance development?

The causes of antibiotic resistance are often complex and it is difficult to identify strategies to prevent or delay its emergence. One strategy has been to use less active members of a drug class, so that when resistance develops the more active members will still prevail. This strategy may often fail because this resistance may form the basis of resistance to the whole class. Often, less active drugs are the first to be discovered and more active versions follow, so we have had no choice; however, increasingly less active drugs are available to deal with specific infections and this may have

Antimicrobial resistance and serotype distribution of Streptococcus pneumoniae strains isolated in southern Taiwan.

The antimicrobial susceptibilities and serotypes of 115 Streptococcus pneumonia strains isolated in southern Taiwan from January 1990 to December 1993 were determined. All isolates were susceptible to cephalothin, cefotaxime, trimethoprim/sulfamethoxazole and vancomycin, and 14 of the isolates were resistant to penicillin G. The oxacillin disk method for presumptive detection of resistance to penicillin had a sensitivity of 85.7% and specificity of 97.0%. Resistance rates were as follows: erythromycin 62.2%, tetracycline 71.3%, clindamycin 46.1% and chloramphenicol 19.1%. Eighty-four percent o

Resistant TB: Newer Drugs and Community Approach.

Drug resistance in tuberculosis (TB) is a serious problem compromising both the treatment and control programs. Poor usage of the available anti TB drugs has led to progressive drug resistance-multi drug resistance (MDR), extensively drug-resistance (XDR) and even total drug resistance (TDR). While drug sensitive TB is completely curable, MDR-TB is difficult to treat, XDR and TDR are often fatal. Non availability of new drugs to treat drug resistant cases further complicates the problem. Two-Pronged Anti-Tumor Therapy by a New Polymer-Paclitaxel Conjugate Micelle with an Anti-Multidrug Resistance Effect.

INTRODUCTION: Cancerous tumors are still a major disease that threatens human life, with tumor multidrug resistance (MDR) being one of the main reasons for the failure of chemotherapy. Thus, reversing tumor MDR has become a research focus of medical scientists. METHODS: Here, a reduction-sensitive polymer prodrug micelle, mPEG-DCA-SS-PTX (PDSP), was manufactured with a new polymer inhibitor of drug resistance as a carrier to overcome MDR and improve the anti-tumor effect of PTX. RESULTS: The PDSP micelles display good stability, double-responsive drug release, and excellent biocompatibility.

Post-translational modifications in drug resistance.

Resistance to antitumor drugs, antimicrobial drugs, and antiviral drugs severely limits treatment effectiveness and cure rate of diseases. Protein post-translational modifications (PTMs) represented by glycosylation, ubiquitination, SUMOylation, acetylation, phosphorylation, palmitoylation, and lactylation are closely related to drug resistance. PTMs are typically achieved by adding sugar chains (glycosylation), small proteins (ubiquitination), lipids (palmitoylation), or functional groups (lactylation) to amino acid residues. These covalent additions are usually the results of signaling casca

Identification of Compounds With Potential Dual Inhibitory Activity Against Drug Efflux Pumps in Resistant Cancer Cells and Bacteria: Protocol for a Systematic Review.

BACKGROUND: Drug efflux mediated by transporter proteins is one of the major mechanisms conferring multidrug resistance (MDR) to antimicrobial agents in bacteria and to chemotherapeutics in cancer cells. Therefore, the development or identification of efflux modulators represents a promising strategy to overcome the resistant phenotype. Various chemical compounds have been tested in experimental studies as reversal agents either in combination with antimicrobial or anticancer drugs and have shown sensitizing activity in resistant bacteria or cancer cell lines. Owing to the common resistance me

Highly diversified multiply drug-resistant HIV-1 quasispecies in PBMCs: a case report.

BACKGROUND: Although drug resistance is a major challenge in HIV therapy, the effect of drug resistance mutations on HIV evolution in vivo is not well understood. We have now investigated genetic heterogeneity in HIV-1 by performing drug resistance genotyping of the PR-RT regions of viruses derived from plasma and peripheral blood mononuclear cells (PBMCs) of a single patient who had failed multiple regimens of anti-retroviral therapy. RESULTS: Patterns of drug resistance mutations showed that the viral populations in PBMCs were more heterogeneous than in plasma. Extensive analysis of HIV fro

The role of cytokines in ovarian cancer drug resistance.

Ovarian cancer is the leading cause of death among women diagnosed with female reproductive system cancers. While significant advances have been made in treating various types of cancer, progress in ovarian cancer treatment over the past 20 years has been minimal, and the treatment course of ovarian cancer is not linear. Although many patients initially respond to treatment and may experience tumor regression, 80% of patients relapse after one or multiple chemotherapy sessions. Cytokines play a crucial role in driving tumor progression, relapse, and drug resistance in ovarian cancer. The knowl
