Free Riding in Healthcare Through a Game-Theoretic Lens: A Cross-Domain Narrative Review and Conceptual Synthesis.

Background/Objectives: Free riding in healthcare occurs when actors benefit from health-related public goods, risk-pooling arrangements, common resources, or cooperative institutions while contributing less than is socially optimal. This review clarifies how free-rider dynamics differ across vaccination, health insurance and universal health coverage, antimicrobial resistance, organ donation and transplant allocation, and global health cooperation. Methods: A narrative review with conceptual synthesis was conducted. Searches of PubMed and Scopus were complemented by citation tracking and targe

Cost-effectiveness of physical activity-oriented interventions for improving mental health: a systematic review.

QUESTION: Are physical activity-oriented interventions for improving mental health conditions cost-effective? This study systematically identified and summarized published evidence on the cost-effectiveness of physical activity-oriented interventions for improving mental health conditions. STUDY SELECTION AND ANALYSIS: We searched four databases (PubMed/Medline, Science Direct, PsychArticles, PsychINFO) for published studies (from any time and region) that (1) assessed physical activity-oriented interventions in mental health disorders, (2) undertook a full economic evaluation (and specifical

Dissociation of intake and incentive sensitization during intermittent- and continuous-access heroin self-administration in rats.

RATIONALE: Opioid misuse is a prominent public health concern, although patterns of use may confer different vulnerability to relapse. Continuous-access (ContA) self-administration has traditionally been used in preclinical models to study drug-motivated behaviors and produces robust escalation of intake and tolerance development. Alternatively, studies using intermittent access (IntA), where self-administration occurs in discrete drug-available periods, suggest that overall intake may be dissociable from subsequent increases in motivation (i.e., incentive sensitization). However, IntA paradig

Pediatric donor heart acceptance practices in the United States: What is really being considered?

BACKGROUND: Recent studies demonstrate high offer decline and organ non-utilization rates are associated with increased pediatric heart transplant waitlist mortality. We sought to determine which donor, candidate, and offer specific variables most importantly influenced these decisions using only data available at the time of each offer. METHODS: Retrospective review of pediatric (<18 years) heart donor offers made to pediatric candidates in the United States between 2010 and 2020. In addition to standard donor, candidate, and offer data available in UNOS, we extracted objective and qualitati

Event-related potentials during the ultimatum game in people with symptoms of depression and/or social anxiety.

Depression and social anxiety are common disorders that have a profound impact on social functioning. The need for studying the neural substrates of social interactions in mental disorders using interactive tasks has been emphasized. The field of neuroeconomics, which combines neuroscience techniques and behavioral economics multiplayer tasks such as the Ultimatum Game (UG), can contribute in this direction. We assessed emotions, behavior, and Event-Related Potentials in participants with depression and/or social anxiety symptoms (MD/SA, n = 63, 57 females) and healthy controls (n = 72, 67 fem

Orexin-1 receptor signaling in ventral tegmental area mediates cue-driven demand for cocaine.

Drug-associated sensory cues increase motivation for drug and the orexin system is importantly involved in this stimulus-enhanced motivation. Ventral tegmental area (VTA) is a major target by which orexin signaling modulates reward behaviors, but it is unknown whether this circuit is necessary for cue-driven motivation for cocaine. Here, we investigated the role of VTA orexin signaling in cue-driven motivation for cocaine using a behavioral economics (BE) paradigm. We found that infusion of the orexin-1 receptor (Ox1R) antagonist SB-334867 (SB) into VTA prior to BE testing reduced motivation w

The nexus of opioids, pain, and addiction: Challenges and solutions.

Pain and addiction are complex disorders with many commonalities. Beneficial outcomes for both disorders can be achieved through similar principles such as individualized medication selection and dosing, comprehensive multi-modal therapies, and judicious modification of treatment as indicated by the patient's status. This is implicit in the term "medication assisted treatment" (MAT) for opioid use disorders (OUD), and is equally important in pain management; however, for many OUD and pain patients, medication is central to the treatment plan and should neither be denied nor withdrawn if critic

What do urologists think patients need to know when starting on androgen deprivation therapy? The perspective from Canada versus countries with lower gross domestic product.

BACKGROUND: Androgen deprivation therapy (ADT) side effects are numerous and negatively impact prostate cancer patients' quality of life. There is considerable discrepancy though among Canadian urologists regarding what ADT side effects and side effect management strategies. Little is known about global differences in ADT patient education. METHODS: International respondents were recruited via online posting and at an international urology conference. Hypotheses suggest that economic and cultural differences influence patient education practices; therefore, international respondents were divi

Quality of life and diagnostic imaging outcomes.

The US Preventive Services Task Force recently promulgated revised guidelines for screening mammography. Criticisms were related to the undervaluation of future lives saved and the overvaluation of negative impacts of mammography. Radiologists downplayed quality-of-life factors, potentially understating the value of all imaging procedures. The task force's recommendations for core needle biopsy, based on similar conceptual frameworks, were not met with equivalent responses. Full appreciation of the costs and benefits of screening provides the basis for making the best decisions for individuals

Meal replacement therapy for obesity and diabetes remission: efficacy assessment and considerations of barriers and facilitators to adherence among U.S. individuals with low economic resources.

Individuals in the United States with lower economic resources face a disproportionate burden of obesity and co-morbid conditions. This review summarizes the efficacy of MR programs for the treatment of obesity and diabetes and alerts clinicians to potential barriers and facilitators to the uptake of such programs so they may tailor their prescriptive approach. Implementation of effective behavioral and lifestyle interventions for obesity and diabetes in low-income settings is fraught with barriers and under-studied. The dearth of data on the use of MR programs in populations with low economic

Does early non-familial child care impact the development of mental health problems, risk behaviors, and educational outcomes until young adulthood?-Findings from an 18-year longitudinal study.

BACKGROUND: International studies exploring the enduring effects of early non-familial childhood care on mental health outcomes show varying impacts depending on the quality of care, age of entry, and socio-economic factors. METHODS: This German longitudinal study, involving 225 families, investigated the impact of early non-familial child care on the development of mental health problems, risk behaviors, and academic as well as vocational achievements in young adulthood (Mage = 22.6 years, SDage = 1.1 years). RESULTS: In comparison to parental care, early non-familial childcare was associat

A Comparison of the Status, Legal, Economic, and Psychological Characteristics of Types of Adult Male Gamblers.

Gambling behavior is not a unique behavior. There are certain differences in behavior, gambling habits, gambling beliefs, and their reflection in psychosocial life. We have compared three groups of adult male gamblers—sports gamblers (n = 41), machine gamblers (n = 36), and poker gamblers (n = 35)—in regard to measures of personal status and legal-social characteristics. We found no difference between groups in terms of the length of gambling behavior, personal status, or age. We found no legal difference between groups in terms of the number of court cases for debt, stealing, or family court

Cumulative abuse experiences, physical health and health behaviors.

PURPOSE: Our purpose was to investigate the complex relationship between a range of lifetime abuse experiences with current physical health and health behaviors. METHODS: Between October 1998 and May 1999, interviews were conducted with 557 ethnically diverse women seen at two urban primary care centers. Seven forms of abuse were measured: childhood physical and sexual abuse, past physical and sexual intimate partner violence (IPV), and recent emotional, physical, and sexual IPV. Severity was measured for six of these forms. Multiple non-specific physical symptoms were measured with a modifie

Combined effects of gender affirmation and economic hardship on vulnerability to HIV: a qualitative analysis among U.S. adult transgender women.

BACKGROUND: Transgender women ("trans women"), particularly African-American and Latina trans women, have disproportionately high prevalence of HIV in the United States (U.S.). In order to decrease gender dysphoria and overcome discrimination, trans women affirm their gender through social and medical transition, often in contexts of economic hardship and sexual risk. This study qualitatively examined how gender-affirming behaviors enhance or diminish vulnerability to HIV in light of structural and economic barriers to gender transition. METHODS: We conducted individual interviews with 19 adu

The dual use dilemma: Is it better to have tried and failed to quit smoking with e-cigarettes or to have never tried e-cigarettes at all?

E-cigarettes are an effective way to stop smoking but there is disagreement regarding their use as treatment. One concern is that recommending e-cigarettes for adults who smoke (AWS) could result in sustained dual use of both products among those who fail to quit smoking. Given most quit attempts end in failure, it is important to understand the likelihood of developing dual use and its consequences when considering e-cigarettes for AWS. We describe variability in dual use within a behavioral economics framework, including literature which addresses the likelihood that trying to quit smoking w

How culturally wise psychological interventions can help reduce poverty.

Poverty is characterized by multidimensional economic, social, and psychological constraints that undermine people's agency to pursue new opportunities and shape their life outcomes. How can interventions best support the agency of low-income individuals and, in so doing, boost poverty-reduction efforts? We theorize and find that agency interventions are effective when designed to be "culturally wise," i.e., attuned to the model of agency predominant in a cultural context. Focusing on low-income women in rural Niger, Study 1 finds that local mental models of economic success primarily reflect

Depleting retinoic acid synthesis in the nucleus accumbens shell produces a protective phenotype for emotional reactivity and drug-taking in rats.

RATIONALE: In previous work, a convergent transcriptomic approach strongly suggested a role for retinoic acid (RA) in controlling the emotion- and reward-related functions of the nucleus accumbens shell (NAcSh). OBJECTIVE: Here, we causally assess the role of NAcSh RA in controlling anxiety-, emotion-, and reward-related behavior in rats and explore cellular mechanisms that may underlie this phenotype. METHODS: Rats underwent bilateral knockdown of the retinoic acid synthesis enzyme Aldh1a1 in the NAcSh. Anxiety-related behavior was assessed using open-field exploration, elevated plus maze,

N-of-1 health optimization: Digital monitoring of biomarker dynamics to gamify adherence to metabolic switching.

The digital health field is experiencing substantial growth due to its potential for sustained and longitudinal deployment. In turn, this may drive improved monitoring and intervention as catalysts for behavioral change compared to traditional point-of-care practices. In particular, the increase in incidence of population health challenges such as diabetes, heart disease, fatty liver disease, and other disorders coupled with rising healthcare costs have emphasized the importance of exploring technical, economics, and implementation considerations, among others in the decentralization of health

Cell-Type-Specific Regulation of Cocaine Reward by the E2F3a Transcription Factor in Nucleus Accumbens.

The development of drug addiction is characterized by molecular changes in brain reward regions that lead to the transition from recreational to compulsive drug use. These neurobiological processes in brain reward regions, such as the nucleus accumbens (NAc), are orchestrated in large part by transcriptional regulation. Our group recently identified the transcription factor E2F3a as a novel regulator of cocaine's rewarding effects and gene expression regulation in the NAc of male mice. Despite this progress, no information is available about the role of E2F3a in regulating cocaine reward at th
