Absence of cardiac impairment in patients after severe acute respiratory syndrome coronavirus type 2 infection: A long-term follow-up study.

BACKGROUND: Concerns exist that long-term cardiac alterations occur after severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) infection, particularly in patients who were hospitalized in the acute phase or who remain symptomatic. This study investigates potential long-term functional and morphological alterations after SARS-CoV-2 infection. METHODS: The authors of this study investigated patients after SARS-CoV-2 infection by using a mobile 1.5T clinical magnetic resonance scanner for cardiac alterations. Cardiac function and dimensions were assessed using a highly efficient car

Ventricular function and tissue characterization by cardiac magnetic resonance imaging following hospitalization for multisystem inflammatory syndrome in children: a prospective study.

BACKGROUND: Multisystem inflammatory syndrome in children (MIS-C) is a severe life-threatening manifestation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection that often presents with acute cardiac dysfunction and cardiogenic shock. While recovery from acute illness is excellent, the long-term myocardial impact is unknown. OBJECTIVE: To compare cardiac MRI findings in children 6-9 months after their hospitalization with MIS-C against MRI findings in healthy controls to assess for residual myocardial disease. MATERIALS AND METHODS: We prospectively performed cardiac MR

Cardiac-gated intracranial elastance in a swine model of raised intracranial pressure: a novel method to assess intracranial pressure-volume dynamics.

OBJECTIVE: Previous studies have demonstrated the importance of intracranial elastance; however, methodological difficulties have limited widespread clinical use. Measuring elastance may offer potential benefit in helping to identify patients at risk for untoward intracranial pressure (ICP) elevation from small rises in intracranial volume. The authors sought to develop an easily used method that accounts for the changing ICP that occurs over a cardiac cycle and to assess this method in a large-animal model over a broad range of ICPs. METHODS: The authors used their previously described cardi

Reversible Myocardial Calcification Following Acute Heart Failure and Kidney Injury Caused by Valsalva Sinus Rupture.

A 47-year-old previously healthy man was referred to a local hospital with chest tightness, oliguria, and lower extremity edema for seven days. An initial investigation revealed acute heart failure and kidney injury. The patient was intensively treated with cardiac and renal replacement therapy, and cardiorenal function improved one week later. Two months later, echocardiography was performed because chest tightness and edema had not resolved. Echocardiography showed Valsalva sinus rupture, and the patient was transferred to our center. Myocardial calcification was observed in the left ventric

Immunoglobulin G4-Related Myocarditis with Eosinophilic Infiltration: A Case Report.

Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is a fibroinflammatory disorder that can involve any organ system; however, myocarditis is extremely rare. A 52-year-old male with dyspnea and chest discomfort underwent cardiac MRI that revealed edema and nodular, patchy, mesocardial and subendoardial delayed enhancement of left ventricle, suggesting myocarditis. Laboratory findings revealed elevated serum IgG4 and eosinophilia. Cardiac biopsy confirmed eosinophilic myocarditis with IgG4-positive cells. Here, we present an unusual case of IgG4-RD manifesting as eosinophilic myocarditis.

Pickering Syndrome Manifesting as Recurrent Pulmonary Edema in an Older Woman With Renal Artery Stenosis.

Pickering syndrome, characterized by recurrent episodes of flash pulmonary edema (FPE) and renal impairment, is associated with renal artery stenosis (RAS). This case highlights its manifestation and management in an older adult patient. An 86-year-old woman with hypertension, chronic kidney disease, and a single functioning kidney presented with recurrent FPE episodes. Imaging revealed severe left RAS and an atrophic right kidney. Despite initial medical management, recurrent episodes of pulmonary edema led to renal angioplasty with stent placement. Follow-up imaging revealed restenosis in th

Recurrent insulin edema in an adolescent with type 2 diabetes mellitus.

Insulin edema is a rare complication of insulin therapy in both type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM), typically following the initiation or intensification of treatment. It has been associated with both long-acting and short-acting insulin formulations and is most observed in newly diagnosed patients. We report on the case of a 13-year-old patient with recurrent insulin edema, first occurring after the initiation of insulin therapy and again following the transition to a closed-loop insulin pump system. The diagnosis of insulin edema was made after excluding card

Fluid Overload.

Fluid overload (FO) is characterized by hypervolemia, edema, or both. In clinical practice it is usually suspected when a patient shows evidence of pulmonary edema, peripheral edema, or body cavity effusion. FO may be a consequence of spontaneous disease, or may be a complication of intravenous fluid therapy. Most clinical studies of the association of FO with fluid therapy and risk of harm define it in terms of an increase in body weight of at least 5-10%, or a positive fluid balance of the same magnitude when fluid intake and urine output are measured. Numerous observational clinical studies

[Ballantyne syndrome associated with fetal cardiac rhabdomyoma: a case report].

Ballantyne syndrome or mirror syndrome was first described in 1892. It is a disorder affecting pregnant women describing the association of fetal anasarca complicated by more or less generalized maternal edema and albuminuria (and sometimes anemia). It is a rare clinical entity. Diagnosis is based on a triad consisting of fetal hydrops, generalized maternal edema and placentomegaly. It can be associated with fetal hydrops from any cause. Diagnostic should be suspected in patients with maternal edema syndrome associated with fetal anasarca. Guarded fetal prognosis can be associated with high ma

Polymorphism of the cardiac manifestations in dermatomyositis.

Cardiac involvement in polymyositis and dermatomyositis usually is asymptomatic and rarely the principal clinical feature at the time of initial presentation. We describe a patient with dermatomyositis who presented with overt acute pericarditis which has not, to our knowledge, been previously reported. Acute pulmonary edema resulting from documented acute myocarditis developed during a second exacerbation. Long-term follow-up failed to demonstrate new cardiac complications during subsequent exacerbations, adding to the known polymorphism of the heart involvement in these systemic disorders.

Long-term follow-up of a child with idiopathic restrictive cardiomyopathy.

A ten-year-old male with palpebral edema was proved to have a restrictive hemodynamic pattern by cardiac catheterization. Sequential echocardiographic examination revealed uniformly depressed left ventricular diastolic properties, well-maintained left ventricular systolic function, and a progression of obliterative change of the left ventricular cavity. Myocardial biopsy at 17 years of age disclosed a normal endocardium and marked interstitial fibrosis of the myocardium. Consequently, a decrease of cardiac output and a marked biatrial enlargement have developed in association with persistent a

Intraoperative Takotsubo Syndrome.

Takotsubo cardiomyopathy is a rare condition that presents a diagnostic challenge due to its close resemblance to acute myocardial ischemia and other cardiac disorders. The excessive production of catecholamines triggers abnormal and severe changes in the myocardium, typically resulting in significant dyskinesia of the left ventricle's apex, reduced ejection fraction, hypotension, and pulmonary edema. Recent reports suggest that potential risk factors may include postmenopausal syndrome and intense, unexpected stress, whether related to life events or medical conditions. Complications such as

The role of inflammation in stress cardiomyopathy.

Stress cardiomyopathy (SC) is an increasingly recognized form of acute heart failure, which has been linked to a wide variety of emotional and physical triggers. The pathophysiological mechanisms of the disease remain incompletely understood, however, inflammation has been recently shown to play a pivotal role. This review summarizes the most notable findings of myocardial inflammation, demonstrated from biopsies and cardiac magnetic resonance imaging in humans. In the acute stage macrophage infiltration appears to represent the substrate for myocardial edema, together defining the local myoca

Complex venous malformation in an elderly female: A case report.

A 76-year-old female taking oral medications for chronic atrial fibrillation and hypertension was admitted to our hospital for examination of bilateral leg edema. Transthoracic echocardiography showed an enlarged coronary sinus of about 40 mm. Contrast-enhanced computed tomography revealed absence of the left innominate vein, a persistent left superior vena cava, and absence of the hepatic segment of the inferior vena cava. The interrupted inferior vena cava connected with the hemiazygos vein. The left superior vena cava also continued to the hemiazygos vein with reflux to the enlarged coronar

High Respiratory and Cardiac Drive Exacerbate Secondary Lung Injury in Patients With Critical Illness.

The high respiratory and cardiac drive is essential to the host-organ unregulated response. When a primary disease and an unregulated secondary response are uncontrolled, the patient may present in a high respiratory and cardiac drive state. High respiratory drive can cause damage to the lungs, pulmonary circulation, and diaphragm, while high cardiac drive can lead to fluid leakage and infiltration as well as pulmonary interstitial edema. A "respiratory and cardiac dual high drive" state may be a sign of an unregulated response and can lead to secondary lung injury through the increase of tran

The role of comprehensive stress cardiac MRI in autoimmune rheumatic disease: A review.

Cardiovascular disease is the leading cause of morbidity and mortality in patients with autoimmune rheumatic diseases. Much of this may be attributed to systemic inflammation resulting in coronary atherosclerosis and myocarditis. Cardiac magnetic resonance imaging is the gold standard for the evaluation of cardiac structure and function, including tissue characterization, which allows for detection of myocardial edema, inflammation, and fibrosis. Advances in parametric mapping and coronary flow reserve measurement techniques have the potential to change the diagnosis, risk stratification, and

Spinal anesthesia after rapid treatment of pulmonary edema in patient with severe preeclampsia: a case report.

INTRODUCTION AND IMPORTANCE: Severe preeclampsia is a significant cause of maternal and neonatal morbidity and mortality. The choice of anesthesia technique in such cases is particularly challenging, as both general anesthesia (GA) and regional anesthesia (RA) have their own advantages and limitations. CASE PRESENTATION: A 28-year-old primigravida Iranian woman with a twin pregnancy at 30 weeks and 3 days of gestation, with a history of diet-controlled gestational diabetes, a body mass index of 35 kg/m2, and a history of upper respiratory tract infection approximately 4 weeks' prior, presente

Cardiac dysrhythmia following pneumonectomy. Clinical correlates and prognostic significance.

Cardiac tachydysrhythmias occurred in 53 (22 percent) of 236 consecutive patients undergoing pneumonectomy. All patients had preoperative electrocardiograms which showed normal sinus rhythm. Patients did not receive digitalis before surgery. Atrial fibrillation was the most common dysrhythmia (64 percent; 34/53), followed by supraventricular tachycardia (23 percent; 12/53) and atrial flutter (13 percent; 7/53). No episodes of ventricular tachycardia were documented. Elevated concentrations of cardiac enzymes were associated with 12 (28 percent) of 43 tachydysrhythmias. Recurrent or persistent

Rare Cause of Restrictive Cardiomyopathy: A Case Report of Löffler Endocarditis.

Löffler endocarditis, an idiopathic hypereosinophilic syndrome, can present symptoms of heart failure and peripheral eosinophilia. We present the case of a 46-year-old woman with fatigue, shortness of breath, and edema, who was found to have thrombocytopenia, elevated cardiac biomarkers, and hypereosinophilia (2.96 k/uL). Cardiac MRI revealed hypertrophic cardiomyopathy, right ventricular hypertrophy, and a restrictive filling pattern, raising suspicion for Löffler endocarditis. Despite initial corticosteroid and diuretic treatment, she was readmitted with persistent symptoms. This case highli

Primary Hypothyroidism Presenting as Cardiac Tamponade.

UNLABELLED: Hypothyroidism is commonly associated with pericardial effusion, but it can be rarely complicated by cardiac tamponade. We report a case series of two patients who presented with shortness of breath and distension of the abdomen, progressing to generalized edema. Each of them was found to have cardiac tamponade at presentation and eventually diagnosed with hypothyroidism. They were managed by urgent pericardiocentesis followed by intermittent drainage of the collected pericardial effusion along with thyroxine replacement to which they responded. The presence of cardiac tamponade wi
