Clinical anatomy of the elbow and shoulder.

The elbow patients herein discussed feature common soft tissue conditions such as tennis elbow, golfers' elbow and olecranon bursitis. Relevant anatomical structures for these conditions can easily be identified and demonstrated by cross examination by instructors and participants. Patients usually present rotator cuff tendinopathy, frozen shoulder, axillary neuropathy and suprascapular neuropathy. The structures involved in tendinopathy and frozen shoulder can be easily identified and demonstrated under normal conditions. The axillary and the suprascapular nerves have surface landmarks but ca…

[Research advancement of the tennis elbow].

Tennis elbow is a common disease. The current research showed that the basic pathology of tennis elbow is not inflammation, but lateral elbow tendinopathy. Ultrasound image has been frequently used for diagnosis of tennis elbow, it can evaluate the condition of disease and show prognosis. Tennis elbow needs a comprehensive treatment. Most patients can obtain satisfactory effects by non-operation, especially the application of the Sodium hyaluronate, autologous blood and botulinum toxin. The principle of operation is to clear the common extensor tendon and repair or rebuild the tendon dead cent…

Treatment for ulnar neuropathy at the elbow.

BACKGROUND: Ulnar neuropathy at the elbow is the second most common entrapment neuropathy after carpal tunnel syndrome. Treatment may be conservative or surgical but optimal management remains controversial. OBJECTIVES: The objectives of this systematic review were to determine the effectiveness and safety of conservative and surgical treatments in ulnar neuropathy at the elbow. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group Specialized Register (16 February 2010), Cochrane Central Register of Controlled Trials (The Cochrane Library 2010, Issue 1), MEDLINE (January 196…

Synovial chondromatosis of the elbow in a child.

Synovial chondromatosis is cartilaginous metaplasia of mesenchymal remnants of synovial tissue of the joints. Its main characteristic is the formation of cartilaginous nodules in the synovium and inside the articular space (loose bodies). It usually presents between the third and fifth decades and is rare in children. It presents as a mono-articular pathology affecting large joints such as the knee, hip, and elbow. The main symptoms are pain, swelling, and limitation of movements in the affected joint. Diagnosis is made by panoramic radiographs, computed tomography scan, and mainly magnetic re…

Arthroscopic synovectomy for the rheumatoid elbow.

PURPOSE: To describe an arthroscopic technique for complete removal of the synovium from the elbow joint and to investigate the clinical outcomes of arthroscopic synovectomy in patients with rheumatoid elbow. METHODS: Arthroscopic synovectomy was performed on 26 rheumatoid elbows in 25 patients with radiographic changes of Larsen grade 3 or less. We performed total synovectomy of the elbow using multiple portals and by dividing the elbow into the anterior, posterior, and radiocapitellar compartments. At a mean follow-up of 33.9 months (range, 13 to 68 months), pain was evaluated with a visual…

Pediatric throwing injuries of the elbow.

Throwing injuries of the elbow in the pediatric population are common. These can occur as lateral compression osteochondral injuries such as osteochondritis dissecans or medial tension overload such as so-called Little League elbow. Extension overload injuries may present in teens as stress injuries of the olecranon process. We review the imaging findings of each of these common injuries, with an emphasis on their magnetic resonance imaging appearance.

[Standardised ultrasound scanning of the elbow].

Musculoskeletal ultrasound (US) is noninvasive, nonionisating and cost-effective imaging diagnostic technique. It is a useful imaging modality for the diagnosis of joint and soft tissue pathology and can be considered as an extension of physical examination. Elbow is easily accessed due to its superficial position. A high resolution, multi-frequency (10-15 MHz) linear transducer should be used. US investigation of the elbow includes scanning of the anterior, medial, lateral and posterior part of the joint. Common flexor and extensor origin at the humearal epicondyles, triceps tendon, distal bi…

Neglected posterior dislocation of the elbow.

The study included two adults and four children who underwent open reduction for subacute, untreated posterior dislocation of the elbow. Elbow stiffness was the main indication for surgery, and in all cases elbow movement was inadequate for the activities of daily living. Mean time from injury was 2 months and mean follow-up was 19 months. At final follow-up there were no cases of instability; according to the Mayo Elbow Performance Index(14), four outcomes were excellent, one good and one poor; the mean score improved from five preoperatively to 83 postoperatively, mean elbow flexion from 18 …

Overuse and traumatic injuries of the elbow.

MR imaging is a useful modality for evaluating athletes presenting with elbow pain. Osteochondral injuries and ligamentous injuries are well seen on MR imaging. Ligamentous injuries may be associated with clinical instability syndromes, the secondary signs of which may be evident on MR images. Enthesopathies and distal biceps tendon injuries are common clinical problems that may be seen in both professional and recreational athletes. Nerve compression syndromes may be investigated using MR imaging; however, the usual aim of imaging is to exclude an underlying space-occupying lesion. This artic…

An unusual case of elbow dislocation.

The annual incidence of elbow dislocation is 6 to 8 cases per 100,000 in the United States; these dislocations represent 11% to 28% of all elbow injuries. The frequency of elbow dislocation is second to that of shoulder dislocation. Adult elbow dislocations are classified by the direction of displacement and associated fractures. They can be anterior, posterior, lateral, or divergent, with the most common type being posterior displacement of both the radius and ulna in relation to the distal humerus. This case report presents a rare case of a twisting dislocation of the elbow joint without any…

The rheumatic causes of elbow instability.

Rheumatoid arthritis is the most common cause of elbow instability, but other causes include other erosive arthritides and noninflammatory diseases. Surgical consultation should be obtained for refractory pain or disability, or when physical examination or imaging reveals instability, erosions, or impending pathologic fracture. Medical management for rheumatoid arthritis includes early treatment and combination therapy, including biologic response modifiers. Current recommendations for rheumatic medicines, including perioperative use, are discussed.

Unstable fracture-dislocations of the elbow.

Fracture-dislocations of the elbow are devastating injuries. The surgeon must maintain a high index of suspicion when evaluating an elbow dislocation to avoid missing critical associated injuries. Patterns of unstable fracture-dislocations include the "terrible triad" injury of the elbow (elbow dislocation, radial head fracture, and coronoid fracture), transolecranon fracture-dislocations, and the posterior Monteggia lesion. Complex fracture-dislocations of the elbow are treated surgically and are challenging injuries to manage. Elbow stability must be restored by addressing the specific compo…

The Steindler flexorplasty for the arthrogrypotic elbow.

PURPOSE: The arthrogrypotic elbow often lacks active flexion. If active elbow flexion can be provided by muscle transfer, patient independence increases and the patient can function in a less conspicuous manner by avoiding adaptive mechanisms. The purpose of this article is to review the outcome of patients with arthrogryposis treated with the Steindler flexorplasty to obtain active elbow flexion. METHODS: Seventeen elbows in 10 patients with an average age of 7 years were treated surgically with the Steindler flexorplasty procedure. Before surgery none of the patients was able to flex active…

Posterolateral rotatory instability of the elbow.

Posterolateral rotatory instability of the elbow is a three-dimensional displacement pattern of abnormal external rotatory subluxation of the ulna coupled with valgus displacement on the humeral trochlea. This pattern causes the forearm bones to displace into external rotation and valgus during flexion of the elbow. Injury to the lateral ulnar collateral ligament allows abnormal supination of the ulna on the humerus. The radial head, being locked in the sigmoid (radial) notch of the proximal ulna by the annular ligament, subluxates posterior to the capitellum. The abnormality is usually posttr…

Posterolateral rotatory instability of the elbow.

Posterolateral rotatory instability (PLRI) of the elbow is recurrent elbow instability caused by injury to the radial ulnohumeral ligament or lateral ulnar collateral ligament. Deficiencies of the radial ulnohumeral ligament and laxity of the lateral capsule allow the proximal radioulnar joint to rotate and the radial head to subluxate posteriorly when stressed. Whether isolated or in conjunction with other injuries, PLRI represents the initial stage in the pathology of the unstable elbow. The diagnosis of PLRI can be difficult because the provocative tests are challenging to perform. Diagnost…

Arthroscopy of the elbow for loose bodies.

This retrospective study confirms the benefit of arthroscopic removal of loose bodies from the elbow, and documents the accuracy of history and plain radiographs. In contrast to previous reports, patients with a diagnosis of osteoarthritis had a similar outcome to those with an alternative diagnosis.

Arthroscopy of the elbow for synovial chondromatosis.

Two cases of arthroscopic removal of multiple loose bodies are presented. Successful results were achieved, and patients had 4-year follow-up. Both cases involved classic manifestations of synovial chondromatosis. Associated degenerative changes were present in one case; histologic examination could not confirm synovial origin of the disease. Arthroscopy for removal of symptomatic loose bodies is an excellent choice as exemplified by these two cases. Care should be taken to review histologic findings, which may help in understanding the natural history of this type of process.
