IgE responses are highly modulated in human filariasis and allergic symptoms usually associated with IgE antibodies are seldom observed in individuals with Elephantiasis and Microfilaremia. Only a minor group with tropical pulmonary eosinophilia (TPE) suffer from nocturnal asthma. One possible mechanism may be the presence of IgG blocking antibodies which can block IgE mediated histamine release in vitro. Using immunoblot analysis we have further shown that remarkably similar pattern of antigen recognition were obtained with IgE and IgG antibodies which would be a major prerequisite for the bl…

Wuchereria bancrofti is an endemic filarial nematode spread by a mosquito vector. The clinical manifestations vary from asymptomatic microfilaremeia to lymhoedema. We report two cases of microfilaria of Wuchereria bancrofti in bone marrow presenting as peripheral pancytopenia. The patient did not have any classical features of lymphoedema or elephantiasis. The diagnostic demonstration of microfilaria with unusual presentations has been done in cytology specimens from various sites. Microfilaria infection in association with peripheral blood pancytopenia has been rarely reported. An aetiopathol…

This article explores the medical conceptualization of the causes of diseases in nineteenth-century Colombia. It traces the history of some of the pathologies that were of major concern among nineteenth-century doctors: periodic fevers (yellow fever and malaria), continuous fevers (typhoid fever), and leprosy (Greek elephantiasis). By comparing the transforming conceptualizations of these diseases, this article shows that their changing pattern, the idea of climatic determinism of diseases (neo-Hippocratism and medical geography), the weak standing of the medical community in Colombian society…

A clinical and investigational study of 25 cases of Donovanosis was undertaken. The incidence was found to be 1.53% of all STD cases and 2.9% of GUD. M:F ratio was 2.12:1. Incidence was more in unmarried people. Fleshy exuberant type was seen in 88% of cases. Two patients (8%) had extragenital ulcers. Donovan bodies were found in 88%. Pseudo elephantiasis was seen in 8 patients. Biopsy was done in 8 cases and showed acanthosis, plasma cell infiltration and pseudo epitheliomatous hyperplasia. One patient developed squamous cell carcinoma of vulva.

A new focus of onchocerciasis is reported from Upper Atbara River area in the Sudan. This river is a known seasonal breeding place of Simulium damnosum. The area was sparsely populated in the past but now, following agricultural development, is inhibited mainly by immigrants from Northern Nigeria and Chad. Clinically the common features are skin changes of the legs, thighs buttocks, arms and trunk. Other features are corneal opacities and small often deeply placed subcutaneous nodules. Choroido-retinal degeneration, genital elephantiasis and hydrocoele, commonly associated with onchocerciasis …

Interviews and physical examinations for filarial disease were conducted in Howrah near Calcutta, West Bengal, India, in a study area where long term quantitative entomological studies have disclosed high intensity, year round exposure to Wuchereria bancrofti. Little elephantiasis or other serious filariasis-related disease was noted, but half the males over 20 years of age had genital lesions of various types. Ninety-two percent of households had at least one resident with microfilaremia or presumptive clinical evidence of filarial disease.

Podoconiosis, or non-filarial elephantiasis, affects nearly 4 million people globally, primarily in regions with volcanic red clay soil. Characterized by lower extremity lymphedema from prolonged barefoot exposure to irritant soil particles, it causes significant economic losses, including $200 million annually in Ethiopia. While its pathogenesis is unclear, genetic predisposition (HLA expression), inflammatory responses, and altered skin microbiota are implicated. Prevention focuses on footwear and hygiene, though these are costly. The stigma surrounding visible symptoms highlights the need f…

BACKGROUND: Podoconiosis is a non-infectious geochemical lymphoedema of the lower legs associated with a significant burden of morbidity. There are historical reports of podoconiosis in India, but its current endemicity status is uncertain. In this investigation we aimed to prioritise the selection of districts for pilot mapping of podoconiosis in India.
METHODS: Through a consultative workshop bringing together expert opinion on podoconiosis with public health and NTDs in India, we developed a framework for the prioritisation of pilot areas. The four criteria for prioritisation were predicte

Filariasis is an infectious disease of the lymphatics and subcutaneous tissues caused by nematodes or filariae. Carried by mosquito vectors, this disease causes millions of people to suffer from lymphedema and elephantiasis, characteristics of filariasis infection. This disease can be diagnosed through the identification of microfilariae in blood or skin samples, antigen detection, radiographic imaging, or polymerase chain reaction. Mass drug administration by the World Health Organization has helped to diminish the incidence of filariasis. However, continued research on new drugs and vaccinat…

Generalized gingival enlargement can be caused by a variety of etiological factors. It can be inherited (hereditary gingival fibromatosis [HGF]); associated with other diseases characterizing a syndrome; or induced as a side effect of systemic drugs, such as phenytoin, cyclosporin, or nifedipine. HGF, previously known as elephantiasis gingivae, hereditary gingival hyperplasia, and hypertrophic gingiva, is a genetic disorder characterized by a progressive enlargement of the gingiva. This review will focus on diagnosis, treatment, and control of HGF. The pattern of inheritance, the histopatholog…

Filariasis is a world health problem that is frequently seen in tropical and subtropical countries. In endemic areas, the clinical spectrum of extremity swelling, lymphangitis, or elephantiasis is usually recognized as filariasis. In the United States, diagnosis of the disease may be more difficult because of lack of familiarity with this infection. We present a case of filaremic arthropathy of the ankle joint and the magnetic resonance imaging (MRI) findings of this disease. It is the first reported case of MRI findings in a human patient. MRI has been done on animal models with filariasis, a…

In a Bancroftian filariasis survey on the coast of Tanzania microfilaria rates rose with age reaching 53% in the 60-69 year group followed by a slight fall above this age. The most important clinical manifestations were hydrocoele, funiculitis and elephantiasis, with hydrocoele presenting the most serious public health problem. Hydrocoele rates increased with age reaching 90% above the age of 70. The highest proportion of large hydrocoeles were also in the older age groups. A satisfactory methodology was established for the planning of future surveys leading to control.

An indirect immunofluorescence assay using papaïn treated microfilariae (IFI-WBp) has been performed to attempt diagnosis of bancroftian filariasis. The authors have analysed 237 bed-rests of patients consulting for specific and non specific lymphangitis, chyluria, elephantiasis, hydrocele or just asymptomatic hypereosinophilia. Taking into account the clinical aspects, the parasitological and the serological results, they have shown the efficiency of IFI-WBp (titre greater than 80) to diagnose filariasis in French Polynesia where only one species of filaria is present (Wuchereria bancrofti va…

The four subclasses of IgG are distinct in structure, function, and degree of participation in the antibody response to complex antigens. Looking for differential responsiveness of potential pathogenetic significance, we have analyzed both quantitatively and qualitatively the filaria-specific IgG subclass responses of 20 patients with lymphatic filariasis presenting either with chronic lymphatic obstructive pathology and elephantiasis (CP) or with asymptomatic microfilaremia (MF). Subclass-specific monoclonal antibodies were used in an enzyme-linked immunosorbent assay to study IgG filarial an…

The relationship between infection and disease has been a subject of intense debate in human filariasis. Most patients with chronic disease such as lymphedema or elephantiasis are free of demonstrable current infection. Based on cross-sectional data and a few assumptions and presumptions, two models of natural history of filariasis have been in vogue during the past two decades. The only (but arduous) way to understand the sequence of events is to follow up subjects with and without patent infections over a period of several years. This article offers critical comments and highlights the insig…

A survey was conducted in Kresek, West Java, Indonesia and 813 persons from six villages examined for filariasis. Only eight had microfilaremia and one male person had elephantiasis. Comparing results obtained approximately ten years earlier and the present survey, there was a marked decrease in the microfilarial rate in Kresek, especially in Penameng, during the interim. There was no drug treatment, insecticide application or any dramatic population movement. The development of a well-organized irrigation system and the conversion of the swamps into rice-fields resulted in a decrease of breed…

Filariases are neglected tropical diseases caused by Clade III parasitic nematodes transmitted by biting insects as microfilaria which mature into female and male adults in the host. One example of filariasis is lymphatic filariasis or elephantiasis, caused by three species of filaria: Brugia malayi, Wuchereria bancrofti, and/or Brugia timori. Elephantiasis results from the occlusion of lymphatic vessels by adult parasites and subsequent swelling of the limbs. It is a disfiguring disease that occurs in 120 million people worldwide. Another filarial disease is onchocerciasis caused by Onchocerc…

BACKGROUND: Lymphatic filariasis (LF) is a parasitic disease that causes permanent disability (elephantiasis). Currently used antifilarial drugs are failing to control LF and there is resurgence in some areas. Looking for new antifilarial leads, we found that Calotropis procera plant parts have been used in traditional medicine for alleviating elephantiasis but the antifilarial activity is not known.
OBJECTIVE: In the present study, the antifilarial activity of ethanolic extract (A001) and its hexane fraction (F001) of C. procera flowers was investigated using the human filarial parasite Brug…
