Median pancreatectomy for tumors of the neck and body of the pancreas.

BACKGROUND: When enucleation is too risky because of possible damage of the main pancreatic duct, benign tumors located in the neck or body of the pancreas are usually removed by a left (spleno)-pancreatectomy or by a pancreatoduodenectomy. But standard pancreatic resection results in an important loss of normal pancreatic parenchyma and may cause impairment of exocrine and endocrine function. The aim of this study was to evaluate early and longterm results of median pancreatectomy, a limited resection of the midportion of the pancreas, in selected patients with benign or borderline tumors of

Effect of the lateral wall valgisation in the treatment of patients with endocrine orbitopathy.

Orbital decompression is an established procedure used to correct exophthalmos that results from excess orbital soft tissue. This study aimed to explore a new minimally-invasive technique that features three-dimensional planning and patient-specific implants for lateral valgisation (LAVA) of the orbital wall. We analysed the outcomes of this procedure in nine endocrine orbitopathy (EO) patients (32-65 years of age with a mean clinical activity score of 4.3) who underwent this procedure between 2021 and 2022, including seven patients diagnosed with dysthyroid optic neuropathy. The impact of LAV

Functional and medical outcomes after tailored surgery for pain due to chronic pancreatitis.

OBJECTIVE: We measured a comprehensive set of outcome measures after different surgical procedures for painful chronic pancreatitis (CP) at long-term follow-up. BACKGROUND: Pain caused by CP can be alleviated through operative intervention with type of procedure depending on anatomical abnormalities. Outcome measures include functional (pain relief, quality of life [QoL]), medical (endo- and exocrine function), and clinical (reoperation) results reported by patient. METHODS: A cross-sectional cohort of 223 consecutive patients who underwent surgical drainage, head resection, or left-sided pa

Surgical outcomes from robotic-assisted adrenalectomy: a case series on experience in a large tertiary referral centre.

BACKGROUND: Robotic-assisted surgery is gaining wider acceptance in most surgical specialties. However, in the United Kingdom, endocrine surgery has not fully adopted robotics. This paper aims to present the early experience and outcomes of patients who underwent robotic-assisted adrenalectomy at a tertiary referral endocrine surgery department. Outcomes were compared to national and international performance measures. METHODS: Seventy-two elective robotic adrenalectomies were performed over a 24-month period. Data collected included patient demographics, co-morbidities, indication for surger

Endoscopic Endonasal Approach for Residual and Recurrent Craniopharyngioma after Transcranial Approach: A Multi-institutional Experience.

Objectives  While the endoscopic endonasal approach (EEA) has become a well-established surgery for resection of craniopharyngiomas (CP), the utility of this procedure following subtotal resection from open transcranial approach (TCA) surgery has yet to be explored. Here we present a multi-institutional case series of patients who underwent EEA for treatment of recurrent CP originally treated by TCA, demonstrating the viability of this approach as salvage surgery. Design  Retrospective cohort. Setting  Loyola University Medical Center and University of Pittsburgh Medical Center. Participants

Metabolic and surgical factors affecting postoperative quality of life in patients with total pancreatectomy with or without splenectomy: Single center results.

OBJECTIVES: Pancreatic resection may be required in the treatment of patients with pathologies of the pancreas. Total pancreatectomy is a major surgical procedure with serious risk of mortality and morbidity, and patient selection is important for prognosis. The endocrine and exocrine pancreatic insufficiency that develops in patients after total pancreatectomy can lead to a serious decrease in the quality of life of the patients due to pain, diarrhea, vomiting etc. Our aim was to evaluate the effect of total pancreatectomy with spleen preservation as well as splenectomy on the quality of life

Immunoglobulin G4-Related Thyroid Diseases.

Immunoglobulin G4-related disease (IgG4-RD) is a new disease category involving many organ systems, including the endocrine system in general and the thyroid in particular. Since an initial association was made between hypothyroidism and autoimmune (IgG4-related) pancreatitis, more forms of IgG4-related thyroid disease (IgG4-RTD) have been recognized. Four subcategories of IgG4-RTD have so far been identified: Riedel thyroiditis (RT), fibrosing variant of Hashimoto thyroiditis (FVHT), IgG4-related Hashimoto thyroiditis, and Graves disease with elevated IgG4 levels. Although a male predominance

Endoscopic disconnection of hypothalamic hamartoma: an already outdated story?

OBJECTIVE: Surgical management of hypothalamic hamartomas (HHs) presents substantial challenges due to their deep-seated location and proximity to critical neurovascular structures. Less invasive techniques, such as endoscopic disconnection and laser interstitial thermal therapy (LITT), have become preferred over traditional microsurgery to enhance seizure control while minimizing complications. Notably, LITT has steadily gained popularity in recent years. The aim of this study was to assess seizure, neurocognitive, and endocrine outcomes following endoscopic disconnection in patients with HH-

Management strategies for bilateral intra-abdominal testes: Insights from a tertiary paediatric centre.

INTRODUCTION: Cryptorchidism, occurring in 2-4 % of male newborns, necessitates orchidopexy to mitigate associated risks, preferably between ages 3-12 months. While unilateral cases are common, bilateral intra-abdominal testes (BIAT) present unique challenges. This study reviews outcomes following bilateral Fowler-Stephens orchidopexy (FSO) at a paediatric surgical tertiary centre, aiming to enhance preoperative counselling and contribute to management consensus. METHODS: A retrospective analysis was conducted at a tertiary paediatric centre, identifying BIAT cases between 2005 and 2021. Clin

Chronic Pancreatitis: Diagnosis and Treatment.

Chronic pancreatitis is an irreversible and progressive disorder of the pancreas characterized by inflammation, fibrosis, and scarring. Exocrine and endocrine functions are lost, often leading to chronic pain. The etiology is multifactorial, although alcoholism is the most significant risk factor in adults. The average age at diagnosis is 35 to 55 years. If chronic pancreatitis is suspected, contrast-enhanced computed tomography is the best imaging modality for diagnosis. Computed tomography may be inconclusive in early stages of the disease, so other modalities such as magnetic resonance imag

How should we manage pain in ventilated neonates?

Newborn babies, even if extremely preterm, show responses to pain. The major stress responses seen with surgical pain are associated with serious adverse medical outcomes. There is an ethical imperative to consider pain relief in babies, despite the fact that they cannot verbalise their experience. Ventilator support, and accompanying treatments are described as distressing in adults, and are associated with an endocrine stress response in babies. Opiates have been shown to reduce physiological instability in sick newborn babies. Despite this, they have not been shown to reduce morbidity when

Chronic pancreatitis.

Chronic pancreatitis is the progressive and permanent destruction of the pancreas resulting in exocrine and endocrine insufficiency and, often, chronic disabling pain. The etiology is multifactorial. Alcoholism plays a significant role in adults, whereas genetic and structural defects predominate in children. The average age at diagnosis is 35 to 55 years. Morbidity and mortality are secondary to chronic pain and complications (e.g., diabetes, pancreatic cancer). Contrast-enhanced computed tomography is the radiographic test of choice for diagnosis, with ductal calcifications being pathognomon

Effect of hypothalamic deafferentation on hypophysial and other endocrine gland blood flows.

The effects of partial or complete deafferentation of the medical basal hypothalamus (MBH) on blood flow through endocrine glands was evaluated using a modification of Sapirstein's indicator fractionation method. Adult female Wistar rats were distributed into three groups consisting of normal control animals (n = 12), animals in which rostral deafferentation (Rd) of the MBH was performed (n = 11), and animals in which complete deafferentation (Cd) of the MBH was done (n = 6). Three weeks after the surgical procedures, Cd reduced adenohypophysial weight by 30% and raised its blood flow levels b

The gram-positive tonsillar and nasal flora of piglets before and after weaning.

OBJECTIVE: In girls with congenital adrenal hyperplasia (CAH), genital ambiguity usually leads to a rapid neonatal diagnosis. Rarely, CAH causes complete virilization and male sex assignment with a delayed diagnosis. After being confronted with very specific problems in two of such patients, we collected data of patients with CAH and complete virilization in a nationwide study to delineate specific problems of these rare patients in order to improve their management. [Development and early experience of an integrated multidisciplinary ward model for adrenal disorders].

Adrenal diseases encompass functional disorders (including primary aldosteronism, pheochromocytoma and paraganglioma, and Cushing's syndrome) and non-functional adrenal incidentalomas, requiring endocrine functional evaluation for diagnosis and often surgical intervention for treatment. Under the traditional departmental model, patients frequently experience repeated hospitalizations, fragmented clinical information, and unclear responsibility boundaries. This study describes the institutional design and preliminary operational experience of an integrated ward dedicated to adrenal disorders at

Metabolic improvement effects of jejunoileal side-to-side anastomosis in patients with type 2 diabetes and the glucagon-like peptide-1 mechanism.

Wang et al explored the metabolic improvement effects of jejunoileal side-to-side anastomosis in patients with type 2 diabetes mellitus (T2DM), focusing on its multitarget metabolic regulatory potential through enhanced secretion of glucagon-like peptide-1. This surgical procedure alters the direction of nutrient flow, activates distal ileal L cells, and increases endogenous glucagon-like peptide-1 levels, supporting glucose homeostasis, enhancing insulin sensitivity, regulating body weight, and improving cardiovascular health. This structural adjustment transforms the gastrointestinal tract i

Evaluation and management of osteochondral lesions of the knee.

Osteochondral lesions of the knee is a common disorder in adolescents, although it may present in children and adults. Despite the fact that the disorder was discovered more than a century ago, no specific causes have been identified, although relationships with ischemia, irregular ossification of epiphyseal cartilage, genetic influences, trauma, and endocrine disorders have been postulated. Taking a thorough history and performing a thorough physical examination will facilitate diagnosis of this condition. Radiographic and magnetic resonance imaging are useful diagnostic tools that aid in the

Effects of cryptorchidy, parabiosis, and estrogen administration upon Leydig cell tumorigenesis in Fischer rats.

The effects on Leydig cell tumorigenesis of surgical cryptorchidy, parabiotic union with a castrate male partner, and a combination on the two procedures as well as of the continuous exposure to a high level of diethylstilbestrol stimulation were studied in the high-tumor Fischer rat. Localized foci of adenomatous hyperplasia were found to have developed in the scrotal testes of most animals by 13.5 months of age. Parabiosis with a castrate male partner did not appear to alter the process significantly, but surgical cryptorchidy, even when combined with parabiosis, completely prevented the dev

Impaired dexamethasone resorption in two patients with pseudo-Cushing after bariatric surgery: Implications for immunosuppressive treatment.

Two cases of middle-aged female patients treated by gastric bypass surgery for weight loss presented to our clinic for a follow-up examination 3-6 months after the surgical procedure (a mini gastric bypass and a modified single anastomosis sleeve-ileostomy). In both patients increased ACTH levels and either high serum cortisol or an increased urinary cortisol excretion was apparent and triggered further endocrine testing. Serum cortisol could not be suppressed adequately by 2 and 4 mg dexamethasone in the standardized oral overnight suppression test while midnight salivary cortisol dropped wel
