Outcome of the unaffected contralateral hip in unilateral slipped capital femoral epiphysis: a report comparing prophylactic fixation with observation.

This was a retrospective review of 80 patients with unilateral slipped capital femoral epiphysis treated with either prophylactic fixation or observation. The unaffected hip of 44 patients (mean age 12.6 years) were subjected to simultaneous prophylactic fixation and 36 patients (mean age 13.4 years) were managed by observation. Fisher's exact test showed a significantly high incidence of sequential slip of unaffected hips in the observation group in comparison with prophylactic fixation (P=0.002). No cases had avascular necrosis or chondrolysis. Prophylactic fixation significantly reduces the

Impact of Childhood Obesity on Capital Femoral Epiphysis Morphology: A Large-Scale, Automated 3D-CT Study and Potential Implications for SCFE Pathogenesis.

BACKGROUND: The precise pathological mechanisms through which obesity increases the risk of slipped capital femoral epiphysis (SCFE) remain unclear. We aimed to investigate the impact of childhood obesity on the morphology of the capital femoral epiphysis in children and adolescents without hip disorders. METHODS: We performed a retrospective cross-sectional study that included 4,888 children and adolescents 7 to 19 years of age who underwent a hip or pelvic computed tomography (CT) scan, most for abdominal pain, between 2004 and 2022. Automated 3D-CT analysis assessed epiphyseal tubercle hei

Physeal Growth Following In Situ Pinning for Slipped Capital Femoral Epiphysis and Hypothyroid Treatment: A Case Report.

Slipped capital femoral epiphysis (SCFE) is a common adolescent hip condition, most often seen during periods of rapid bone growth. Deficiency in thyroid hormone levels can lead to reduced bone turnover and altered epiphyseal plate activity, which may influence the outcome of SCFE pinning and other orthopedic interventions crossing the epiphysis of the femur. Our patient was a 12-year-old female child with a two-month history of atraumatic right hip pain who presented with bilateral slipped capital femoral epiphysis (SCFE) and underwent successful bilateral in situ pinning. An endocrinology wo

[Determination of skeletal age using the Oxford Score in children with epiphyseolysis of the femur head].

Pelvis radiographs of 47 children with slipped capital femoral epiphyses (SCFE) and 24 without a hip disorder were reviewed in a retrospective study by four readers to determine the skeletal age by the Oxford method. 72% of the children suffering from SCFE had a body weight above the standard level but a normal body height at the time of diagnosis. The mean chronologic age was 3 years higher than the skeletal age determined by the Oxford-score. The linear regression analyses showed that children with SCFE have had a skeletal maturation delay in comparison with the normal group at the beginning

Rachitic syndrome after disodium etidronate therapy in an adolescent.

A 12-year-old patient with a severe traumatic brain injury developed heterotopic ossification (HO) with rapidly decreasing range of motion in multiple joints despite intensive passive range of motion exercises and the use of nonsteroidal antiinflammatory drugs (NSAIDs). His alkaline phosphatase was markedly elevated. Etidronate, 20mg/kg/d was used to control the ossification. After 7 months of continual etidronate use, the patient developed periarticular pain with widened growth plates, suggesting a rachitic syndrome. Serum laboratory panel including calcium, phosphorous, alkaline phosphatase,

Clinical characteristics of children with atypical and idiopathic slipped capital femoral epiphysis: description of the age-weight test and implications for further diagnostic investigation.

Slipped capital femoral epiphyses (SCFEs) can be idiopathic or atypical (associated with renal failure, radiation therapy, and endocrine disorders). The demographics of 433 children (285 idiopathic, 148 atypical) with 612 SCFEs were studied to define predictors of atypical SCFEs. Multiple logistic regression analysis showed that age and weight were predictors. For two patients of equal weight, those younger than 10 or older than 16 years of age were 4.2 times more likely to have an atypical SCFE; for two patients of equal age, those <50th percentile weight were 8.4 times more likely. The age-w

Traction reduction of acute and acute-on-chronic slipped capital femoral epiphysis.

Gentle reduction of acute or the acute portion of acute-on-chronic severe slipped capital femoral epiphyses (SCFE) is generally recommended. Reports of the use of longitudinal traction with medial rotation traction, although few, are uniform in describing a high rate of success with rare complications. Of 30 acute or acute-on-chronic SCFEs examined at the author's institution between 1970 and 1990, 13 underwent attempted traction reduction with longitudinal traction and medial rotation. Only five of these 13 hips had discernible reduction. One of these five hips developed aseptic necrosis; how

Slipped capital femoral epiphysis: long-term results of an abandoned technique.

Sixty-two (11 women and 51 men) of 76 consecutive patients treated between January 1960 and December 1979 with percutaneous pinning, traction, and ambulation in Thomas splints for slipped capital femoral epiphyses (physiolysis colli femoris; PCF) were re-examined radiologically and clinically to evaluate long-term results of an abandoned technique. Median follow-up time was 21 years (range: 8 to 28). The average age at symptom start was 14 years (range: 9 to 17). Fourteen patients were operated bilaterally, while 15 were operated on the right side only and 33 on the left side only. No addition

'Hip' pain.

'Hip' pain is usually located in the groin, upper thigh or buttock and is a common complaint. Slipped capital femoral epiphysis, avascular femoral head necrosis and apophyseal avulsion are the most common diagnoses in childhood and adolescents. Strains and fractures are common in sport-active adults. Osteoarthritis occurs in middle-aged and older adults. Trauma may result in femoral head fracture or typical muscle and tendon sprains and bursitis. Septic or inflammatory arthritis can occur at every age. Septic arthritis, fractures and acute epiphyseal slipping are real emergency cases. Congenit

Limp affecting the hip and knee in children.

Limp and pain are common findings about the hip and knee in growing children. Epiphyseal growth peculiarities in children account for many of the specific disease entities. Simple but specific physical examination and mandatory simple X-rays will virtually always give or exclude a diagnosis. problems about the hip are usually serious, for example Legg-Perthes, slipped epiphysis and septic arthritis. Most childhood knee problems are annoying but not serious. Chondromalacia patellae and Osgood-Schlatter's disease seldom require treatment-but the knee is the most common site for osteogenic sarcom

Slipped Capital Femoral Epiphysis in a 26-Month-Old Boy with Cerebral Palsy: A Case Report.

CASE: A 26-month-old child with spastic quadriplegic cerebral palsy, TPN dependence, laryngomalacia, and G6PD deficiency presented with right hip pain with no known recent trauma. Imaging revealed a slipped capital femoral epiphysis (SCFE). The patient underwent hip irrigation and debridement for potential septic arthritis, with SCFE stabilized using buried 2.0 K-wires. A single-leg spica cast was applied. Follow-up showed symptom improvement and no SCFE progression. CONCLUSION: This is the first reported case of SCFE in a child aged younger than 3 years without seizure history. SCFE should b

Which factor is more reliable considering prophylactic pinning of contralateral hip of unilateral SCFE patients?

BACKGROUND: This study evaluates the radiological parameters of developing subsequent contralateral slips in unilateral slipped capital femoral epiphysis (SCFE) patients at the time of initial presentation. METHODS: The study group included the review of unilateral SCFE patients treated between June 2007 and August 2018. Age, gen-der, side, stability, posterior slope angle, grade of slip, modified Oxford bone age score (mOBAS), the Risser classification, and the ap-pearance of the triradiate cartilage were evaluated retrospectively. Data were analyzed between two groups: subsequent contralate

Temporal trends in surgical implants for in situ fixation of stable slipped capital femoral epiphysis.

Stable slipped capital femoral epiphysis (SCFE) is a pediatric hip disorder managed with in situ fixation of the proximal femoral epiphysis, otherwise known as 'pinning the hip'. The objective of this study was to characterize how the choice of implant for in situ fixation of stable SCFE has changed over time. A systematic review of publications concerning in situ fixation of stable SCFE from January 1993 to November 2021 was conducted. The change in the type, diameter, and number of implants used in publications over time and the age of their respective patient cohorts was evaluated. A total

Controlled repositioning and primary osteoplasty as a treatment option for the management of unstable slipped capital femoral epiphysis.

The ideal management of an unstable slipped capital femoral epiphysis (SCFE) is still controversial. The currently available options are in-situ screw fixation with delayed corrective osteotomy, closed reduction and screw fixation and anatomic reduction by modified Dunn's procedure (MDP). We present the results of our technique of controlled re-positioning (CRP) with primary osteoplasty in which the epiphysis is repositioned to the preacute slip stage and a mini-open osteoplasty is done in the same sitting to avoid later femoro-acetabular impingement. We had 27 unstable slips which presented t

Slipped Capital Femoral Epiphysis in an Adult with Panhypopituitarism: A Case Report.

CASE: Our patient was a 31-year-old woman with previously undiagnosed panhypopituitarism who presented with bilateral slipped capital femoral epiphysis (SCFE). Together with endocrinology, her treatment plan involved medical management of her panhypopituitarism and surgical correction with femoral neck osteotomy. Hormone deficiencies were corrected 6 weeks after replacement therapy began, and the patient was cleared for surgery. By 5 months after osteotomy, examination revealed delayed union and prominent screws bilaterally. Hardware exchange and repeat internal fixation were then performed. A

[Specific cases of slipped capital femoral epiphysis : Endocrinopathies, hormone therapy and other rare causes].

In slipped capital femoral epiphysis (SCFE) atypical causes can be found in about 5% of cases. The patients' age differs from the typical age of SCFE patients of between age 10 and 16. There are numerous causes of atypical SCFE: different endocrine disorders, sequelae of chronic renal failure and effects of radiation and chemotherapy on the proximal femoral physis. The diagnostic and therapeutic procedures must be expanded to the search for and treatment of endocrinological disorders. Orthopaedic treatment follows the principles applied to patients with idiopathic SCFE. However, the specific t

Asynchronous slipped capital femoral epiphysis in a patient with a seizure disorder: case report and review of the literature.

Debate remains about the appropriate treatment of the asymptomatic side following treatment of a unilateral slipped capital femoral epiphysis (SCFE). A 12-year-old boy with a seizure disorder presented with an unstable SCFE on the left hip following a seizure. He underwent percutaneous pinning of the left only. At 8 months postoperatively, he returned with an unstable slip of the right hip, again following a seizure. No literature discussing the treatment of patients with risk of SCFE and seizure disorders was identified. Further study may identify an increased incidence of contralateral slip
