Requirement for written informed consent and selection bias in a chart-review prostate cancer study.

Although retrospective chart-review studies are typically performed using a waiver of written informed consent, many institutional review boards (IRB) do not approve such waivers for chart-review studies using prospective enrollment, which could introduce selection bias in participant characteristics and outcomes, thereby impairing representativeness and validity. We aim to determine this bias in a chart-review prostate cancer (PCa) study using prospective enrollment. Using an IRB-approved chart-review protocol with a waiver of written informed consent for prospective enrollment, we identified

Parents Refusing Dialysis for a 3-Month-Old With Renal Failure.

A 3-month-old boy with failure to thrive was referred to a nephrology clinic after a diagnostic workup for failure to thrive revealed a serum urea nitrogen level of 95 mg/dL and creatinine level of 3.6 mg/dL. A renal ultrasound revealed marked bilateral hydronephrosis with little remaining renal cortex in either kidney. A voiding cystourethrogram revealed evidence of posterior urethral valves. The child had no evident comorbidities. Fulguration of the valves was successfully performed but did not lead to improvement in kidney function. The nephrologists recommended the initiation of dialysis w

International feasibility trial on the use of an interactive mobile health platform for cardiac rehabilitation: protocol of the Diversity 1 study.

INTRODUCTION: The implementation of home-based cardiac rehabilitation has demonstrated potential to increase patient participation, but the content and the delivering of the programmes varies across countries. The objective of this study is to investigate whether an Australian-validated mobile health (mHealth) platform for cardiac rehabilitation will be accepted and adopted irrespectively from the existing organisational and contextual factors in five different European countries. METHODS AND ANALYSIS: This international multicentre feasibility study will use surveys, preliminary observations

A case of deceptive mastectomy.

This paper poses the question, "what are providers' obligations to patients who lie?" This question is explored through the lens of a specific case: a 26-year-old woman who requests prophylactic bilateral mastectomy with reconstruction reports a significant and dramatic family history, but does not want to undergo genetic testing. Using a conversational-style discussion, the case is explored by a breast surgeon, genetic counselor/medical geneticist, clinical psychologist, chair of a hospital ethics committee and director of a clinical ethics consultation service.

Molecular epidemiology of dengue in Southeast Asia (SEA): Protocol of systematic review and meta-analysis.

INTRODUCTION: Dengue fever is a major global public health challenge caused by the Arbovirus and transmitted by Aedes mosquitoes. The increasing incidence of dengue, particularly in the Southeast Asia (SEA) region, including Malaysia, highlights the urgent need for a comprehensive understanding of dengue molecular epidemiology. This study aims to systematically review various aspects of dengue molecular epidemiology to gain insights into the disease's dynamics, transmission and circulation. Providing evidence-based insights is crucial for the prevention and control of dengue. METHODS AND ANAL

Ventilation management and outcomes in out-of-hospital cardiac arrest: a protocol for a preplanned secondary analysis of the TTM2 trial.

INTRODUCTION: Mechanical ventilation is a fundamental component in the management of patients post cardiac arrest. However, the ventilator settings and the gas-exchange targets used after cardiac arrest may not be optimal to minimise post-anoxic secondary brain injury. Therefore, questions remain regarding the best ventilator management in such patients. METHODS AND ANALYSIS: This is a preplanned analysis of the international randomised controlled trial, targeted hypothermia versus targeted normothermia after out-of-hospital cardiac arrest (OHCA)-target temperature management 2 (TTM2). Oral paracetamol versus oral ibuprofen for closure of haemodynamically significant patent ductus arteriosus in preterm neonates (<32 weeks): a blinded, randomised, active-controlled, non-inferiority trial.

INTRODUCTION: Haemodynamically significant patent ductus arteriosus (hsPDA) is a common cause of mortality and morbidity in preterm infants. Existing medical therapies with ibuprofen or indomethacin have multiple adverse effects. Hence, an alternative drug like paracetamol given through oral route with less side effects need to be tested in an appropriate study design with least risk of bias to arrive at a conclusion. METHODS AND ANALYSIS: Multisite, randomised, active-controlled, non-inferiority design. The primary objective is to study the efficacy of oral paracetamol for closure of hsPDA i

Prospective accuracy study on an artificial intelligence-based ultrasound system for gestational age estimation among pregnant women in Ghana, Kenya and South Africa: protocol.

BACKGROUND: Risk screening for pre-eclampsia relies on accurate gestational age assessment, but routine access to ultrasound-based gestational dating remains challenging in many low- and middle-income countries. As part of the formative work for the 'Preventing pre-eclampsia: Evaluating AspiRin Low-dose regimens following risk Screening' (PEARLS) platform, we aim to validate and implement an artificial intelligence (AI)-based algorithm for estimation of gestational age, using blind sweeps done with a handheld ultrasound device. This study protocol outlines the accuracy cohort for AI-based gest

Fragility fractures of the pelvis in the elderly: protocol for an international prospective multicentre observational cohort study.

INTRODUCTION: Fragility fractures of the pelvis (FFPs) are low-energy osteoporotic fractures that primarily affect elderly patients. They are associated with substantial morbidity, mortality and healthcare burden, and their incidence continues to rise as the global population ages. Despite the availability of dedicated classification systems and advances in surgical techniques, high-quality evidence to guide optimal management remains limited, and treatment algorithms are not standardised. To address these gaps, we established an international, observational cohort study designed to collect co

Study protocol: an open-label individually randomised controlled trial to assess the efficacy of artemether-lumefantrine prophylaxis for malaria among forest goers in Cambodia.

INTRODUCTION: In the Greater Mekong Subregion, adults are at highest risk for malaria. The most relevant disease vectors bite during daytime and outdoors which makes forest work a high-risk activity for malaria. The absence of effective vector control strategies and limited periods of exposure during forest visits suggest that chemoprophylaxis could be an appropriate strategy to protect forest goers against malaria. METHODS AND ANALYSIS: The protocol describes an open-label randomised controlled trial of artemether-lumefantrine (AL) versus multivitamin as prophylaxis against malaria among for

Protocol for a prospective observational cohort study collecting data on demographics, symptoms and biomarkers in people with mesothelioma (ASSESS-meso).

INTRODUCTION: Mesothelioma is a heterogeneous disease that can be challenging to monitor and prognosticate. ASSESS-meso is a multicentre, prospective, longitudinal observational cohort study of patients with mesothelioma. The primary aim is to describe different clinical phenotypes and investigate predictive and prognostic factors, including biomarkers from blood and pleural fluid. The secondary aim is to provide a resource for future trials and substudies. METHODS AND ANALYSIS: We aim to recruit 700 patients with a histological, cytological or clinicopathological diagnosis of mesothelioma, a
