Evidence-based practice habits: putting more sacred cows out to pasture.

For excellence in practice to be the standard for care, critical care nurses must embrace evidence-based practice as the norm. Nurses cannot knowingly continue a clinical practice despite research showing that the practice is not helpful and may even be harmful to patients. This article is based on 2 presentations on evidence-based practice from the American Association for Critical-Care Nurses' 2009 and 2010 National Teaching Institute and addresses 7 practice issues that were selected for 2 reasons. First, they are within the realm of nursing, and a change in practice could improve patient c

Nursing interventions for adult intensive care patients with risk for corneal injury: a systematic review.

PURPOSE: Sedated patients in induced comas may experience ineffective eyelid closure, presenting higher risk for corneal ulcer. The objective was to identify evidence-based practice for corneal ulcer prevention within the intensive care unit. METHODS: A systematic literature review of nine articles was performed. CONCLUSIONS: Questions remain about which care practice is most effective, although most studies consider a humidity chamber with polyethylene film to be the best practice. IMPLICATIONS FOR NURSING PRACTICE: A proposed NANDA-I nursing diagnosis, risk for corneal injury, reflects th

Development and pilot-testing of an evidence-based quality indicator set for home mechanical ventilation care: the OVER-BEAS project.

BACKGROUND: The number of patients depending on home mechanical ventilation (HMV) has increased substantially in Germany in recent years. These patients receive long-term care in different nursing facilities (nursing home, shared living community, private home). However, there are limited data available on the quality of care of HMV patients. The aim of the OVER-BEAS project was to identify quality indicators (QIs) of HMV care using an evidence-based approach. METHODS: A multidisciplinary board consisting of professionals and experts of HMV provision compiled a set of QIs between March and Se

Prevention of catheter-associated urinary tract infection in neurological post-operation patients: a best practice implementation project.

OBJECTIVE: The objective of this evidence based implementation project was to promote evidence based practice in the prevention of catheter-associated urinary tract infection (CAUTI) in the neurology and neurosurgery units of a Taipei hospital. INTRODUCTION: Catheter-associated urinary tract infection has been associated with increased morbidity, mortality, hospital costs and length of stay. Urinary drainage systems are often reservoirs of infection in patients, thus effective prevention of CAUTI requires an evidence-based approach. The CAUTI incidence in the neurosurgery units of the hospita

Fallacious reasoning and complexity as root causes of clinical inertia.

BACKGROUND: The quality gap and clinical inertia are stubborn problems that are prevalent, but difficult to explain. We puzzle why conscientious practitioners, who are keenly aware of appropriate evidence based treatments, avoid initiating or intensifying treatments. This has caused some to question that there may be hidden causes for these errors in the planning of chronic care. METHODOLOGY: A timely clue enabled the author to recognize his flawed reasoning when planning the care of two elderly women sustaining hip fracture. The sudden awareness of planning error motivated a study of the und

Putting evidence into practice: evidence-based interventions for the management of oral mucositis.

Mucositis, an inflammation of the mucous membranes, is a commonly occurring side effect of chemotherapy and radiation. Oral mucositis can cause significant clinical consequences, such as pain, malnutrition, and local and systemic infections. Nurses have a critical role in all aspects of managing mucositis, including assessing it, teaching oral care, administering pharmacologic interventions, and helping patients cope with symptom distress. Mucositis can have a negative impact on the overall treatment experience, especially when severe pain or infections occur. Many interventions for managing m

Depression treatment research in people with cancer does not reflect cancer prevalence: findings from a systematic review.

BACKGROUND: One in six people with cancer will develop depression at some point in their care. Untreated depression affects quality of life, cancer care satisfaction and healthcare expenditure. Treatments for this vulnerable heterogenous population should be evidence based and specific. A common sentiment is that psychiatric research does not reflect the prevalence of patients with cancer and comorbid depression and is biased towards certain cancers, but this has not been empirically shown. STUDY SELECTION AND ANALYSIS: A systematic review of studies on psychological and pharmacological treat

Reducing catheter-associated urinary tract infections through best practice: Sherwood Forest Hospitals' experience.

A programme to standardise catheterisation practice was introduced in Sherwood Forest Hospitals NHS Trust in 2016, with the aim of reducing the incidence of catheter-associated urinary tract infections (CAUTIs). The initiative involved the use of a catheterisation pack (Bard®Tray). Within the first year following its introduction, the CAUTI rate had been reduced from 13.3% to 2.1% (between July 2016 and June 2017), which is a reduction of more than 80%. Standardisation has also brought cost savings for the Trust of about £33 000 a year. The Trust has maintained its standardisation approach and

1. Vaginal breech birth--the phoenix arising from the ashes.

Vaginal breech birth all but disappeared from UK maternity units after the publication of the Term Breech Trial (Hannah et al 2000). However, mounting evidence does not support caesarean section as the safest mode of birth for the baby or the mother when the baby is presenting breech. But the intervening years have depleted the attending professional's skills. We describe our personal journey to regaining them, learning better ones and introducing safe choice for women. This article is the first in a series which examines the practice and evidence base for the care of women and babies aspiring

Acute cardiogenic pulmonary oedema: reflecting on the management of an intensive care unit patient.

AIM: The aim of this paper is to reflect upon the management interventions of non-invasive ventilation (NIV) and diuretic therapy that were implemented for a patient admitted to an intensive care unit (ICU) with acute cardiogenic pulmonary oedema. BACKGROUND: Acute pulmonary oedema is a serious and life-threatening complication of acute heart failure, particularly if this results from an ischaemic event. Statistics highlight that of those patients treated for acute cardiogenic pulmonary oedema, approximately only one third were alive after 1 year. Many of these patients will require intensive

Bullies and power: a look at the research.

Research indicates that bullying continues to plague schools, and school-based anti-bullying programs do little to take away the opportunities and power of the child who bullies. Research indicates methods to identify the child who bullies, and provides guidelines to change the bullying behaviors. Unfortunately, this information fails to result in changes in practice. Nomination of children who bully by their peers and the utilization of methods that remove the bully's power are promising interventions. Administrators, teachers, parents, nurses, and mental health providers need to be proactive

Effects of sexual function of essential hypertensions in women.

BACKGROUND: Hypertension, one of the increasingly common chronic diseases, is reported to be an important factor in the formation of sexual dysfunction (SD) due to the physiopathological changes it creates in the vascular endothelial structure. The evaluation of the sexual functions of hypertensive women is regarded as a significant consideration in the improvement of women's health and quality of life. AIM: This study was carried out in order to identify the sexual functions of essential hypertensive women. METHODS: The study was an experimental design. 71 essential hypertensive women with

Randomised controlled trials as part of clinical care: A seven-step routinisation framework proposal.

Evidence translation in wound care relies on the need for evidence generation. Clinical practice may generate evidence only if evidence-generating research projects, such as randomised controlled trials (RCTs), became routinised in clinical settings. The aim of this study was to identify optimal trial-related practices to routinise trial-related activities in Melbourne-located wound clinics as reported by clinicians and researchers. We conducted a secondary analysis of the available data on how to routinise RCTs in clinical care, with a focus on enablers and suggestions provided by the partici

Issues emerging from the London Learning Disability Plan.

This article gives an overview of the draft of the London Learning Disability Plan developed by the Department of Health, Social Services Inspectorate and the National Health Service Executive. The London Plan is founded on a social model of disability in which there is not only a strong focus on health and social care, but also an integration with housing, education, employment, leisure and other relevant organizations. It advocates integrated commissioning and delivery of specialist learning disability services that will require both health services and nurses to change existing structures a

The efficiency of fluid balance charting: an evidence-based management project.

This study began following an observation that fluid balance (FB) charts seemed to be overloading medical records in Hong Kong hospitals. The major issues for this study were the efficiency and appropriateness of FB charting. The study design included an analysis of 250 medical records and an opinion survey. Seventy-five doctors and 98 nurses participated in the survey. The results indicated that 50% of all medical records had FB charts and in these cases they made up between 12 and 16% of the whole medical record. Intravenous infusion and urinary catheterization accounted for almost 70% of th

Pigtail catheters used in the treatment of pneumothoraces in the neonate.

Air leak in the neonatal population can be a deadly situation. Neonates have many risk factors that can contribute to air leak. These include, but are not limited to, respiratory distress syndrome; mechanical ventilation; sepsis; pneumonia; aspiration of meconium, blood, or amniotic fluid; and congenital malformations. In the NICU, the staff must be prepared to diagnose and treat pneumothoraces in a timely manner. Pathophysiology of air leaks in the neonate including the anatomy of the chest and diagnosis, indications, and common methods for the treatment of a pneumothorax in an infant is expl

Long term urinary catheter problems: a flow chart to aid management.

Pressure ulcers cause a great deal of distress, loss of dignity for the patient and can be problematic in terms of management. Pressure ulceration in the sacral area may lead to added complications if the patient has problems with urinary and faecal incontinence. Maintenance of the patient's skin integrity is vital to avoid added discomfort and distress due to moisture lesions. Prevention of wound and dressing contamination with urine and faeces is difficult and can lead to increases in clinical risk (infection) and cost if not managed successfully.
