[Our clinical experience in the treatment of snakebites].

OBJECTIVES: We evaluated the results of medical and surgical treatment for venomous snakebites and reviewed current principles of first aid and therapy for affected patients. METHODS: Fourteen venomous snakebite victims (8 males, 6 females; mean age 22 years; range 7 to 75 years) were enrolled in the study. Six patients received medical treatment alone, while eight patients required both medical and surgical treatments. Injury was in the upper and lower extremities in nine and five patients, respectively. Fasciotomy was performed in seven patients due to ensuing compartment syndrome, which wa…

Harvesting of the radial artery: subfasciotomy or full skeletonization: a comparative study.

BACKGROUND: Long fasciotomy of the posterior aspect of the radial artery (RA) for the purpose of sympathectomy is one of the surgical techniques for the preparation of this graft. We compared the early results of this technique with those of the RA graft harvested as a pedicle in the conventional fashion. METHODS: The study patients have been selected from the patients who underwent coronary artery bypass grafting (CABG) using the RA graft harvested either with a long posterior fasciotomy (group 1) or as a whole with a pedicle in the classical manner (group 2). Only the patients with a flow s…

The "Jacobsen flap" technique: a safe, simple surgical procedure to treat Dupuytren disease of the little finger in advanced stage.

The surgery for advanced stages of Dupuytren disease of the little finger is controversial. In the literature, several techniques have been described with variable reported results and postoperative complications. Percutaneous needle fasciotomy, McCash technique, and dermofasciectomy are often performed for surgical treatment but they present significant complications and limits. This study reviews our experience of using the Jacobsen flap technique, a modification of the McCash procedure. We found that the Jacobsen flap technique for the significant correction of the contracture, the low rate…

Acute compartment syndrome from anterior thigh muscle contusion: a report of eight cases.

Eight patients with an anterior thigh muscle contusion or rupture developed an acute anterior compartment syndrome. All patients had an increased pressure in the quadriceps muscle, ranging between 41 and 80 mm Hg. At fasciotomy, three cases had a hematoma in the rectus femoris and four in the vastus intermedius, and in one case only, edema of the anterior compartment was found. Postoperatively, there was immediate pain relief in all cases, and after about 4 weeks, all patients had normal function. We recommend broad indications for fasciotomy and evacuation of hematoma after thigh muscle contu…

An 84-Year-Old Man with Acute Atraumatic Compartment Syndrome of the Upper Extremity Due to Streptococcus pyogenes Cellulitis.

BACKGROUND Acute compartment syndrome represents a surgical emergency to restore blood supply by fasciotomy and decompression. Compartment syndrome is most commonly associated with crush-related injuries and fractures; however, other non-traumatic etiologies can occur. This report illustrates a rare case of acute atraumatic compartment syndrome of the upper extremity due to Streptococcus pyogenes cellulitis in an 84-year-old man, presenting a challenging clinical scenario. CASE REPORT An 84-year-old man presented to the Emergency Department with 24 h of significant right-arm pain and swelling.…

Pigmenturia following direct muscle injury and delayed arterial reconstructive surgery. A case report.

A case is presented with severe muscular trauma to the lower leg and ischaemia of nine hours duration secondary to a totally disrupted popliteal artery and vein. Following successful revascularisation and fasciotomies, the patient developed an extreme local muscular oedma, pigmenturia and impaired renal function. It is concluded that the pigmenturia was a mixture of both myoglobinuria and haematuria, dependent on an acute toxic nephrosis, due in part to rhabdomyolysis and in part to the effect of delayed revascularisation. Extensive fasciotomies completed at the time of revascularisation almos

Use of Recombinant Factor VIIa for Bleeding Control in Treatment of Acute Extremity Compartment Syndrome Secondary to Primary Myelofibrosis: A Case Report.

CASE: A 40-year-old man was admitted to our emergency department with a painful and swollen calf. There was no history of significant trauma, and the physical examination revealed a pulseless, swollen left lower leg. Clinical history revealed a diagnosis of primary myelofibrosis, and magnetic resonance imaging showed a rupture of the gastrocnemius medial head. The diagnosis of spontaneous acute extremity compartment syndrome (AECS) secondary to myelofibrosis was established. An open fasciotomy procedure was performed, and recombinant factor VIIa treatment was applied to control bleeding. Posto…

Compartment syndrome after acute rupture of the peroneus longus in a high school football player: a case report.

We report the case of a young athlete who sustained an inversion injury to his ankle and presented several hours later with leg pain out of proportion to the injury. Pressure measurements suggested a lateral compartment syndrome and he underwent an emergency fasciotomy. The case is described and recommendations for diagnosis and treatment are given. Acute compartment syndrome of the lower extremity is an orthopedic emergency. Expedient release by emergent fasciotomy can minimize the likelihood of serious consequences such as myonecrosis and ischemic contracture. Most commonly, compartment synd…

Acute volar and dorsal compartment syndrome after a distal radius fracture: a case report.

We present a case of an acute volar and dorsal compartment syndrome of the forearm secondary to a distal radius fracture. The patient sustained a low-energy injury (fall) and presented within 12 hours with signs and symptoms of a compartment syndrome prior to any treatment. Emergent volar and dorsal fasciotomies were performed, and the fracture was reduced and pinned. The fracture and fasciotomies healed without complication, and the patient was doing well on the last follow-up examination. To our knowledge, this is the first report in the literature of a volar and dorsal compartment syndrome

A practical approach to compartmental syndrome. Part III. Management.

Early diagnosis and appropriate treatment of an established compartment syndrome usually results in a functional extremity for the patient. In my experience, the commonest causes for failure of a fasciotomy are (1) delay in initiation of the operative procedure and (2) an incomplete fasciotomy. Thorough knowledge of the anatomy of the upper and lower extremity should allow the surgeon to confidently decompress any compartment with relative ease and safety for the patient. Most of these extremities have already suffered closed blunt trauma, and many are associated with an underlying fracture. I…

Bilateral simultaneous traumatic upper arm compartment syndromes associated with anabolic steroids.

BACKGROUND: Acute compartment syndrome, a surgical emergency, is defined as increased pressure in an osseofascial space. The resulting reduction of capillary perfusion to that compartment requires prompt fasciotomy. Treatment delay has a poor prognosis, and is associated with muscle and nerve ischemia, resultant infarction, and late-onset contractures. OBJECTIVES: We report a case of traumatic bilateral upper limb acute compartment syndrome associated with anabolic steroids, requiring bilateral emergency fasciotomies. CASE REPORT: A 25-year-old male bodybuilder taking anabolic steroids, with…

Collagenase Treatment in Dupuytren Contractures: A Review of the Current State Versus Future Needs.

Dupuytren disease is highly prevalent and the finger contractures can be very extensile, compromising the patients' hand function. To restore full function, contractures have been addressed by cutting the causative strands for nearly 200 years, ever since Baron Guillaume Dupuytren demonstrated his technique at the beginning of the nineteenth century. Surgery can be minimal (fasciotomy) or quite invasive (fasciectomy and even skin replacement). However, in the last decade translational research has introduced the non-surgical technique of enzymatic fasciotomy with collagenase injections. Now, f…

Acute compartment syndrome following Bitis viper envenomation: a literature review with case reports.

BACKGROUND: Snakebites caused by Bitis nasicornis and Bitis gabonica are rare but can lead to severe systemic and local complications, including acute compartment syndrome (ACS). The role of surgical intervention in snakebite management remains controversial, with limited data available for snakebite envenomation. CASE PRESENTATION: Two cases of upper limb envenomation by Bitis nasicornis and Bitis gabonica were managed at the General University Hospital in Prague in year 2024. Both developed acute compartment syndrome requiring prompt antivenom therapy, fasciotomy, and intensive care. In the…
