The ultrasound aspect of the skin and subcutaneous fat layer in various benign and malignant breast conditions.

In a series of 260 consecutive X-ray mammography examinations in symptomatic patients, 25 patients proved to have breast cancer, and 41 had various benign breast conditions. In the 66 patients with abnormal mammograms a real-time ultrasound examination with a 7.5 MHz linear array transducer and a fluid offset pad was performed. This paper focuses on the ultrasound aspect of the skin and the subcutaneous fat layer in various benign and malignant breast conditions. In most of the malignant tumors studied as well as in cases of fat necrosis and breast abscesses, the following ultrasound signs wer…

Local application of disc-related cytokines on spinal nerve roots.

STUDY DESIGN: To analyze the effects of tumor necrosis factor-alpha, interleukin-1beta, and interferon-gamma on cauda equina function and to define if any of these cytokines could induce nerve root dysfunction comparable with the situation with application of nucleus pulposus. SUMMARY OF BACKGROUND DATA: Proinflammatory cytokines derived from the intervertebral disc have been suggested to mediate the nucleus pulposus-induced nerve root injury following local application of nucleus pulposus. However, it is not known if such cytokines may induce similar injury if applied separately. METHODS: A

Sonographic findings in ischemic colitis in 58 patients.

OBJECTIVE: The purpose of this study was to describe the sonographic findings of 58 patients with proven ischemic colitis and to evaluate whether any of the findings are related to the presence or development of transmural necrosis. MATERIALS AND METHODS: We reviewed the histories of patients diagnosed with ischemic colitis over a period of 5.5 years. Sixty-two patients had undergone sonographic examinations. The spectrum of sonographic findings in ischemic colitis was based on the original imaging report, with an analysis of the presence of colonic abnormalities and their associated alterati

Arterial Fat Embolism in a Pediatric Patient with Femur Fracture: A Case Report.

CASE: Here, we present the case of a pediatric polytrauma patient found to have lower extremity intra-arterial fat embolism causing ischemic necrosis and ultimately necessitating below-the-knee amputation. CONCLUSION: Fat embolism, a common complication of long bone fractures in adults, can be associated with significant morbidity. Although rare, it should be considered among the possible etiologies for a pulseless limb after trauma. Early fracture stabilization may prevent fat embolism and fat embolism syndrome; however, there is no known definitive treatment, and management is supportive.

Growing Fat Tissue After Grafting for Dural Sealing.

We report on a young patient with a growing retroauricular benign fat tissue tumor after juvenile fat grafting for dural sealing of a placed ventriculoperitoneal shunt. The clinical images indicate fat tissue rather than a cerebrospinal fluid leak due to potential shunt malfunction suspected on plain radiography. Human adipose tissue is a source of stem cells that can replicate rather than undergoing necrosis, in particular when transplanted during development.

Partial breast irradiation in breast conserving therapy by way of intersitial brachytherapy.

OBJECTIVE: To examine early and late toxicities, evaluate cosmetic results, and determine the need for reoperation or additional diagnostic procedures in patients treated with accelerated partial breast irradiation (APBI) delivered by way of an interstitial implant in breast-conserving therapy. METHODS: A total of 199 patients with stage I or II breast cancer were managed with lumpectomy followed by radiation restricted to the tumor bed using an interstitial implant (APBI). Retrospective analyses were performed for early and late toxicities (infection, fat necrosis, breast pain, edema, erythe…

Acquired idiopathic lipoatrophic panniculitis in a 12-month-old infant.

Lipoatrophic panniculitis is a rare condition affecting mainly children, often associated with connective tissue disease. We report a healthy 12-month-old girl with no clinical or laboratory features of connective tissue disease who presented with the progressive appearance of annular atrophic plaques beginning at the left arm. A histopathological analysis revealed lobular panniculitis, with fat necrosis and an associated inflammatory infiltrate supporting the diagnosis of lipoatrophic panniculitis. Lipoatrophic panniculitis should be considered in infants and young children with clinical feat…

The deep inferior epigastric perforator flap for breast reconstruction in overweight and obese patients.

The authors retrospectively reviewed the computerized records of 71 women undergoing 80 deep inferior epigastric perforator (DIEP) flap reconstructions after mastectomy over a 1-year period. There were 33 normal, 26 overweight, and 12 obese patients. No statistically significant difference in flap complications was found between groups. Overall fat necrosis rates were 11.4 percent for the normal-weight patients, 6.7 percent for the overweight patients, and 6.7 percent for the obese patients. Postoperative hospital time was similar for all groups. The occurrence of abdominal wall fascial laxity…

[Subcutaneous adiponecrosis in newborn infants].

A case of subcutaneous fat necrosis of the newborn gave us the opportunity of re-examining this disease in the light of recent reports. Clinical, histological and laboratorial data do not show any specific pattern, although, seen as whole, they seem to form a picture distinguishable from sclerema neonatorum.

Nicolau syndrome after intramuscular injection of non-steroidal anti-inflammatory drugs (NSAID).

Nicolau syndrome is a rare complication of intramuscular injection that leads to local ischemic necrosis of the skin and adipose tissue. In this paper, we discuss etiologies, risk factors, and treatment options for gluteal Nicolau syndrome referring to patients treated in our hospital. Our study includes 17 women who visited our clinic with symptoms of gluteal necrosis secondary to intramuscular injection. The following variables were taken into account: injection site, drug administered, frequency of injections, the person who administered the injections, needle size, and needle tip color. Ma

Crossed fat embolism in a cow with tetralogy of Fallot.

A 10-month-old, female Holstein-Friesian cow with tetralogy of Fallot died during surgery for ameliorating pulmonary stenosis. Necropsy revealed no fracture of bones. Microscopically, fat emboli in the capillaries were detected in the brain, kidney, lung, adrenal gland and pituitary. In the kidney, many fat emboli were present in the glomeruli. No significant changes of the neurons were found throughout the brain, and perivascular hemorrhage and/or necrosis were rarely seen accompanying with fat emboli in the cerebral cortex. The trigger for fat embolism might be costectomy, and right-to-left

3T-MRI, elastography, digital mammography, and FDG-PET CT findings of subcutaneous panniculitis-like T-cell lymphoma (SPTCL) of the breast.

Subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is a rare subtype of cutaneous lymphoma, which is characterized by infiltration of ne … ermetabolism in areas corresponding to other imaging techniques. MRI can be useful for diagnosis of fat necrosis, which is a primary radiologic feature of SPTCL. However, fat necrosis has multitude of appearances by various imaging techniques, which typically indicate a benign disease, but may indicate a malignancy. Therefore, an ultrasonographically guided co …

Melatonin promotes survival of nonvascularized fat grafts and enhances the viability and migration of human adipose-derived stem cells via down-regulation of acute inflammatory cytokines.

Nonvascularized fat grafting is a valuable technique for soft tissue reconstruction but poor survival of fat in the host environment remains a problem. A process known as cell-assisted transfer is used to enhance fat graft retention by adding stromal vascular fraction, an adipose-derived stem cell (ASC) rich content to lipoaspirate. We have recently shown that the use of melatonin, a reactive oxygen species scavenger, protects human ASCs from hydrogen peroxide-induced oxidative stress and cell death in vitro but its role as a pharmacological adjunct in clinical fat grafting has not been studie

Panniculitis associated with a pancreas divisum.

A case is presented of a 67-year-old man with chronic abdominal pain thought to be due to peptic ulcer disease. He developed nodules of panniculitis (nodular fat necrosis), resulting in extensive investigations for pancreatic disease. He was ultimately found to have low-grade pancreatitis associated with a pancreas divisum. Surgical treatment of this congenital anomaly resulted in complete resolution of the panniculitis and abdominal pain. Panniculitis as a manifestation of pancreatitis is well documented. However, panniculitis leading to the discovery of chronic pancreatitis with a surgically…
