[Lower limb paresis after total hip arthroplasty. A rare differential diagnosis].

We report on a case of complex nerve damage during total hip arthroplasty. The most severe clinical symptom was proximal leg paresis with diffuse sensory loss. There was an extensive causal Iliopsoas hematoma which developed during the postoperative rehabilitation under therapeutic anticoagulation for atrial fibrillation. An iliopsoas hematoma with subsequent neurological deficits are rare events in the field of hip arthroplasty and a literature review is provided. The treatment of retroperitoneal hemorrhage is controversial but in most instances a conservative approach is favored. The prognos

Meralgia paraesthetica: an unusual complication in peritoneal dialysis.

A 53-year-old woman with a history of end-stage renal disease on peritoneal dialysis (PD) presented with a 3-month history of intermittent numbness and paraesthesia over the anterior aspect of the right thigh. The patient noticed the pain was worse on walking and related to dialysis sessions. An examination revealed no neurovascular abnormalities or abdominal masses. However, there was subjective paraesthesia in the distribution of the right lateral cutaneous nerve. Subsequent nerve conduction studies revealed the cause of the patient's symptoms. She was diagnosed with meralgia paraesthetica.

Simvastatin-induced meralgia paresthetica.

3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) cause mainly muscular adverse effects. During the last 20 years there has been solid evidence of peripheral neuropathy caused by statins, with a risk of one in 10,000 patients treated for 1 year. Meralgia paresthetica is an entrapment neuropathy occasionally encountered by primary care physicians. To date there has been no report of entrapment neuropathy that could have been caused or aggravated by statins. This case report presents meralgia paresthetica aggravated by simvastatin use that disappeared after simvastatin cessati

Treatment of meralgia paresthetica with ultrasound-guided pulsed radiofrequency ablation of the lateral femoral cutaneous nerve.

A 23-year-old female with an 18-month history of left anterolateral thigh paresthesias and burning pain consistent with meralgia paresthetica was referred to our clinic after failing trials of physical therapy, nonsteroidal anti-inflammatories, gabapentin, and amitriptyline. We performed 3 lateral femoral cutaneous nerve blocks with corticosteroid over a 4-month period; however, each block provided only temporary relief. As this pain was limiting the patient's ability to perform her functions as an active duty service member, we elected to perform a pulsed radiofrequency treatment of the later

Percutaneous treatment of iatrogenic pseudoaneurysms by cyanoacrylate-based wall-gluing.

PURPOSE: Although the majority of iatrogenic pseudoaneurysms (PSAs) are amenable to ultrasound (US)-guided thrombin injection, patients with those causing neuropathy, claudication, significant venous compression, or soft tissue necrosis are considered poor candidates for this option and referred to surgery. We aimed to test the effectiveness and feasibility of a novel percutaneous cyanoacrylate glue (NBCA-MS)-based technique for treatment of symptomatic and asymptomatic iatrogenic PSA. MATERIAL AND METHODS: During a 3-year period, we prospectively enrolled 91 patients with iatrogenic PSA [tot

Long-term prognosis of nerve palsy after total hip arthroplasty: results of two-year-follow-ups and long-term results after a mean time of 8 years.

INTRODUCTION: Nerve damage is a rare but serious complication after THA. There exist only little data about the outcome of these patients particularly regarding the long-term results later than 2 years postoperatively. Aim of this study is to answer the following questions: Is the recovery to be expected for light nerve lesions different from the severe ones? Is there a possibility of nerve recovery more than 2 years after THA? Is the potential of nerve recovery depending on the affected nerve? MATERIALS AND METHODS: This study investigates 2,255 primary THA as well as revision surgeries perf

[The main tunnel syndromes].

Several tunnel syndromes are responsible for substantial functional impairment. The diagnosis has to be made and treatment is most often very simple--nerve decompression--with excellent results. Of these syndromes, the most common are median and ulnar tunnel syndromes of the wrist and ulnar tunnel syndrome of the elbow, but other syndromes must be identified at the risk of therapy failure due to poorly adapted treatment. Finally, good knowledge of this pathology must lead to prevention of the iatrogenic forms (sequelae of inguinal hernia treatment, ileac crest graft harvesting) by educating al

[Compartment syndrome of the thigh with sciatic nerve paralysis].

UNLABELLED: Acute compartment syndrome of the thigh has been infrequently reported in the literature. Closed femoral fractures and blunt soft tissue trauma are the main causes of this injury. The multiple injured patient in this case report developed a compartment syndrome of the thigh after intramedullary nailing of a comminuted fracture of the femur. Fasciotomy was performed two days after surgery because of extense swelling of the thigh in the ventilated and sedated patient. Sciatic and femoral nerve palsy was recognized after extubation of the patient nine days after the injury. During the

Laparoscopic plug removal for femoral nerve colic pain after mesh & plug hernioplasty.

BACKGROUND: Inguinal hernias account for 75% of abdominal wall hernias, with a lifetime risk of 27% in men and 3% in women. Major complications are recurrence, chronic pain, and surgical site infection, but their frequency is low. Few studies have reported a calcified mesh causing neuropathy by chronic compression of the femoral nerve after mesh & plug inguinal hernia repair. This is the first report of laparoscopic plug removal for femoral colic due to femoral nerve irritation cause by a calcified plug after mesh & plug inguinal hernia repair. CASE PRESENTATION: In July 2013, a 53-year-old m

Meralgia paresthetica occurring 40 years after iliac bone graft harvesting: case report.

OBJECTIVE AND IMPORTANCE: Meralgia paresthetica is an entrapment neuropathy involving the lateral femoral cutaneous nerve. We describe an unusual case in which meralgia paresthetica occurred many years after iliac bone graft harvesting. CLINICAL PRESENTATION: An 81-year-old man presented with a 1-year history of pain, dysesthesia, and hypesthesia in the anterolateral aspect of the right thigh. This patient had undergone iliac bone grafting when he sustained a calcaneal fracture 40 years previously. Radiographs and computed tomographic scans of the pelvis revealed a bony excrescence in the ant

Electromyographic evidence of inferior gluteal nerve compromise: an early representation of recurrent colorectal carcinoma.

Lumbosacral or primary buttock pain and cutaneous anesthesia in the distribution of the posterior femoral cutaneous nerve were the initial symptoms in 5 patients later proven to have extensions of previously resected colorectal malignancy. The initial electromyographic impression of an inferior gluteal nerve mononeuropathy associated with recurrent neoplasia was confirmed by a positive colon biopsy in 1 case and widespread pelvic metastasis demonstrated at laparotomy in the 4 remaining cases. Four of the 5 patients also had increased levels of carcinoembryonic antigen. Roentgenographs and othe

Cryoablation of an Inguinal Angiomyomatous Hamartoma: A Case Report.

Angiomyomatous hamartomas are mixed-tissue benign lymphatic tumors that typically occur in the inguinal or axillary lymph nodes. These lesions may cause local lymphedema and painful compression neuralgias. Ultrasound-guided percutaneous cryoablation is an interventional technique that has been shown to provide sustained pain relief for peripheral neuropathies and neuralgias. Previously, our group published a case report of a patient with an inguinal angiomyomatous hamartoma causing a compression neuralgia, whose pain was temporarily alleviated after blockade of the femoral branch of the genito

Bilateral thigh compartment syndromes from extended sitting with forward bending.

Alcohol consumption and subsequent sleeping in unusual positions often causes compression neuropathies. In this case, we experienced a unique case of bilateral leg palsy after sleeping in a forward bending position that was photographed by his colleague. We expected that prolonged blockage of the circulation of the femoral arteries resulted in bilateral thigh compartment syndromes (TCSs), leading to sciatic nerve damage. The muscle MRI and needle EMG support this hypothesis. A couple of similar cases have been reported, but the causes of TCS was undetermined due to lack of medical history. Thi

Giant iliopsoas bursitis: a complication of chronic arthritis.

Iliopsoas bursitis is a poorly recognized cause of hip pain that requires early recognition to avoid potentially serious complications caused by compression of adjacent structures. It can occur in the setting of trauma in athletes or those who engage in heavy labor and is also associated with acute or chronic arthritis. We describe the cases of 2 patients, one of whom developed a femoral neuropathy, while the other had marked venous compression of the lower limb resulting from enlargement of the iliopsoas bursa. Magnetic resonance imaging offers the most accurate information on the extent of t…

[A case of retroperitoneal schwannoma extracted by a nerve sparing procedure under microscopic surgery].

We report a case in which we extracted retroperitoneal schwannoma by a nerve sparing procedure under microscopic surgery. A 63-year-old male was diagnosed with left ureter stone and left hydronephrosis. A left retroperitoneal tumor was found by the abdominal ultrasound sonography. Abdominal computed tomography and magnetic resonance imaging revealed the mass 20 mm in diameter in the retroperitoneal cavity. We considered that the tumor arose from the left femoral nerve, and removed it under microscopic surgery. There was no malignancy. There was neither recurrence nor neuropathy after operation

Cementless total hip arthroplasty in patients with high congenital hip dislocation.

BACKGROUND: The optimal surgical treatment for patients with high congenital dislocation of the hip remains controversial. The purpose of our study was to evaluate the mid-term to long-term results of cementless total hip arthroplasty in such patients. METHODS: The study included sixty-eight total hip replacements performed between 1989 and 1994 in fifty-six consecutive patients with high congenital hip dislocation at our hospital. The cup was placed at the level of the true acetabulum, and a shortening osteotomy of the proximal part of the femur and distal advancement of the greater trochant

Brief transvertebral electrical stimulation of the spinal cord improves the specificity of femoral nerve reinnervation.

BACKGROUND: Functional outcomes are generally poor following peripheral nerve injury (PNI). The reason is multifactorial but includes the misdirection of regenerating axons to inappropriate end organs. It has been shown that brief electrical stimulation (Estim) of nerves has the potential to improve the accuracy and rate of peripheral axon regeneration. OBJECTIVE: The present study explores a novel percutaneous transvertebral approach to Estim, which was tested in the mouse femoral nerve model. METHODS: Inspired by the protocol of Gordon and colleagues (ie, 20 Hz, for 1 hour), we applied Est
