Understanding firearm-related suicide mortality in the national violent death reporting system: A study of circumstantial social determinants.

OBJECTIVE: To investigate the association between social determinants of health (SDoH), mental health history, and the propensity of firearm usage in suicide in the United States from 2003 to 2022. METHODS: Using retrospective data from the National Violent Death Reporting System, we analyzed 348,112 suicide deaths, identifying differences in circumstantial, sociodemographic and mental health characteristics across those who used firearms versus other lethal methods. RESULTS: Documentation of any adverse health care access SDoH or adverse social context-related SDoH was significantly associa…

A Twenty-Year Retrospective Cohort Study of Mortality and Morbidities in Adult Trauma Patients with Blunt, Sharp, and Firearm Injuries.

Background and Objectives: Traumatic injuries are a major public health issue, being the leading cause of death in the U.S. Advancements in medical care, injury prevention, and regional trauma systems have improved survival rates, but there is limited information on outcomes for survivors. Blunt, sharp, and firearm injuries are the primary mechanisms in trauma forensics. This study examines patient outcomes for blunt, sharp, and firearm injuries over 20 years. Materials and Methods: De-identified data were collected from the TriNetX Research network in June 2024. Patients aged 18-90 were categ

Shock index, hypotension, and blood product transfusion as predictors of post-traumatic stress disorder in firearm-related trauma.

BACKGROUND: Gunshot wound survivors are at especially high risk for post-traumatic stress disorder. Early identification of patients at increased risk remains a major clinical challenge. Physiologic markers of injury severity, such as shock index and hypotension, are readily available during initial resuscitation and may help identify patients who could benefit from targeted mental health interventions. This exploratory study evaluates the association between markers of physiologic stress and post-traumatic stress disorder screening outcomes following gunshot wound injury. METHODS: We retrosp

Concentrated Affluence and Deprivation and Spatial Distribution of Firearm Violence: Racial/Ethnic Disparities Across Urban Neighborhoods in the U.S.

INTRODUCTION: This study examined whether neighborhood-level concentration of deprivation relative to affluence is associated with spatial variation in shootings and whether this association differs by the racial/ethnic composition of neighborhoods. METHODS: Tract-level firearm violence data from 2020 to 2023 were obtained for 100 U.S. cities from the American Violence database. A modern measure of income inequality-Index of Concentration at the Extremes-and negative binomial regression models were used to estimate the marginal effects of concentration at the extremes on shootings across Blac

Messenger Acceptability for Suicide Means Safety Messages for Firearm Owners: Effects of Perceived Expertise, Trustworthiness, and Ideological Similarity.

Means safety, the process by which an individual's endorsed or expected method of suicide is made less available, is a widely used suicide prevention method. For firearm owners, this process involves delivering a means safety message, the acceptability of which may be influenced by the identity of the messenger. Previous research has indicated that the perceived expertise, trustworthiness, and ideological similarity of a messenger can influence the overall acceptability of a message, though to date there has not been research examining these constructs with regard to means safety messages for

Emergency Medical Services Encounters for Firearm Injuries - 858 Counties, United States, January 2019-September 2023.

Firearm-related deaths and injuries have increased in recent years. Comprehensive and timely information on firearm injuries and the communities and geographic locations most affected by firearm violence is crucial for guiding prevention activities. However, traditional surveillance systems for firearm injury, which are mostly based on hospital encounters and mortality-related data, often lack information on the location where the shooting occurred. This study examined annual and monthly rates of emergency medical services (EMS) encounters for firearm injury per 100,000 total EMS encounters du

Firearm Access and Socio-Structural Factors Related to Suicidality Among Youth With Diverse Sexual, Gender, and Racial Identities.

OBJECTIVE: Elevated rates of suicidal ideation (SI) and suicide attempts (SA) among youth, particularly multiply marginalized youth, are occurring in the context of youths' access to household firearms. Research examining how perceived access to firearms is related to SI and SA among youth with marginalized identities is limited and often neglects to consider intersectionality. This study explored how intersecting social identities and positions, access to firearms, and socio-structural factors were associated with SI and SA for youth. METHOD: The analytic sample (N = 17,794) included 7-12th …

Examining the Relation Between Early Violence Exposure and Firearm-Related Experiences in Emerging Adulthood: A Longitudinal Cohort Study.

Firearms are a leading cause of death among adolescents and young adults in the United States. Early exposure to violence, as a victim or witness, is associated with increased risk of firearm-related experiences, including carrying and threatening others with a gun. These experiences, in turn, increase the risk of both fatal and non-fatal firearm injuries. Using an ethnically diverse sample of emerging adults, we build on prior research by examining the link between early violence exposure at multiple contexts of the social-ecological model and multiple firearm-related experiences (i.e., firea…

Association of Community Vulnerability and State Gun Laws With Firearm Deaths in Children and Adolescents Aged 10 to 19 Years.

IMPORTANCE: Firearm-related injuries are the leading cause of death among children and adolescents in the US. For youths aged 10 to 19 years, 64% of firearm-related deaths are due to assault. Understanding the association between the rate of death due to assault-related firearm injury and both community-level vulnerability and state-level gun laws may inform prevention efforts and public health policy. OBJECTIVE: To assess the rate of death due to assault-related firearm injury stratified by community-level social vulnerability and state-level gun laws in a national cohort of youths aged 10 t

Change in suicide rates in Switzerland before and after firearm restriction resulting from the 2003 “Army XXI” reform.

OBJECTIVE: Firearms are the most common method of suicide among young men in Switzerland. From March 2003 through February 2004, the number of Swiss soldiers was halved as a result of an army reform (Army XXI), leading to a decrease in the availability of guns nationwide. The authors investigated the patterns of the overall suicide rate and the firearm suicide rate before and after the reform. METHOD: Using a naturalistic study design, the authors compared suicide rates before (1995–2003) and after the intervention (2004–2008) in the affected population (men ages 18–43) and in two comparison …

Circumstances Preceding Firearm Suicide Deaths in a Large Sample of Hispanic/Latino Adults: Findings From a Content Analysis.

INTRODUCTION: Rates of suicide deaths involving firearms have been increasing among Hispanic/Latino adults in the U.S. The high lethality of firearm suicide attempts and critical gaps in characterizing firearm suicide deaths among Hispanic/Latino adults may pose challenges to suicide prevention. However, there is little systematic knowledge about the circumstances that manifest among Hispanic/Latino suicide decedents who die by firearms or about how those circumstances may differ between female and male Hispanic/Latino firearm suicide decedents. This study aimed to descriptively characterize r…

An epidemiological evaluation of fatal and nonfatal assault related firearm injuries among youth aged 0-17 years.

BACKGROUND: Firearm-related injuries are a leading cause of death and disability among youth in the United States (US). Children are at risk for assault-related firearm injuries based on their race, ethnicity, age, and gender. The present study quantifies assault-related firearm morbidity and mortality among US children aged 0-17 years. METHODS: We utilized data from 41 states reporting to both the Nationwide Emergency Sample (NEDS) and the National Violent Death Reporting System (NVDRS) from 2019 to 2020 to calculate rates of nonfatal and fatal firearm-related assaults, respectively. We also

Violent Firearm Injury and Homelessness: Examining the Experiences of Black Men Participating in Hospital-Based Violence Intervention Programs.

Both experiencing homelessness and having survived a violent firearm injury are well-known risk factors for being a victim of violence. Despite there being significant racial disparities in these factors, there is no qualitative research examining the experiences of Black men who survive a gunshot wound and were homeless after their injury. Drawing on over a year of ethnographic research at the two busiest hospital-based violence intervention programs in the state of Maryland, this research centers the experiences of violently injured homeless Black men to understand how to best support their

Longitudinal Associations Between Peer Risk and Promotive Factors and Exposure to Community Firearm Violence Among Black Adolescents.

Firearm violence is pervasive for Black early adolescents in disinvested urban communities. The current study used a subsample of a longitudinal school-based dataset (N = 479, ages 10-17, 100% Black), which included middle school students from three urban communities who completed surveys at two time points (Fall, Spring). Cross-lagged panel models were run to specify the directionality of relations between five firearm exposure items and four peer factors; multiple group models tested differences in paths by sex and by grade. Results indicated that community firearm violence exposure was asso
