Spasticity is usually treated by rehabilitation, orthosis, chemical denervations, orthopaedic surgery and neurosurgery. Selective fascicular neurotomy is a neurosurgical procedure consisting in partial section of selected motor nerves innervating spastic muscles. Neurotomy is indicated in cases of localised disabling spasticity without musculotendinous shortening, resistant to chemical denervation and for which a motor nerve block with anaesthetic has given a good functional result. Neurotomy includes division of the afferent Ia and Ib fibers, unable to recover, leading to permanent disappeara

Metatarsal head resection for the symptomatic, deformed, rheumatic foot was performed in 83 feet of 49 patients via a modified plantar approach with functional after treatment, and in 64 feet of 41 patients via a dorsal approach, followed by placement in a plaster cast for 6 weeks. Both groups were comparable with regard to age, sex, and follow-up. From 1975 to 1985, 147 feet (90 patients) were operated in two different hospitals. Of these, 25 patients (39 feet) were lost to follow-up. The average follow-up was 83.3 mo. (range 33-136 mo.). Data were obtained by studying the medical records and

Bilateral absence of the ulna and ulnar rays in twin female infants are described as a manifestation of the split hand--split foot deformity (ectrodactyly). Family history revealed the father had a unilateral split hand. Given the wide variability of expression of this disorder and the availability of prenatal diagnosis, there is a need for obstetricians and pediatricians to recognize parents with this malformation and appreciate the potential for severely affected offspring.

The development of lateral tibial torsion in the paralysed lower limb is well documented, but its pathogenesis is poorly understood. This paper attempts to provide an explanation for its development when it is associated with a varus or equinovarus deformity of the hindfoot. Correction of the lateral tibial torsion by supramalleolar derotation tibial osteotomy and reorientation of the ankle mortise appear to unlock the talus from the laterally rotated position, correcting a mobile hindfoot varus deformity and altering soft-tissue tensions about the ankle so that the correction achieved is main

BACKGROUND: Progressive collapsing foot deformity (PCFD) has a variable course, with over 10% of patients progressing to surgery despite nonoperative treatment.
METHODS: Adults with PCFD initially treated nonoperatively and baseline weight-bearing CT were retrospectively stratified into nonoperative and surgical groups. Demographics, PROMIS, and WBCT alignment were compared.
RESULTS: Seventy-six feet were analyzed. Progression to surgery was associated with greater baseline hindfoot malalignment (Foot-Ankle Offset 9.0 ± 3.1 vs 6.6 ± 3.0, p = 0.046; Hindfoot Moment Arm p = 0.008), male sex (p

AIMS: Weightbearing CT (WBCT) has set a new standard for the assessment of foot and ankle alignment in patients with progressive collapsing foot deformity (PCFD) under physiological loading conditions compared with conventional CT. Principal component analysis (PCA) models are currently used for a detailed 3D shape analysis, but are not able to take into account non-linear (e.g. rotational) anatomical variance, which is particularly relevant in PCFD. Innovative advances in geometrical morphometrics by principal polynomial shape analysis (PPSA) are now able to overcome this challenge. Therefore

BACKGROUND: This study aimed to evaluate the outcomes of calcaneal lengthening osteotomy (CLO) and double arthrodesis of the talonavicular and calcaneocuboid joints (DA) for correcting planovalgus foot deformity exclusively in patients with generalised joint hypermobility.
METHODS: We retrospectively reviewed 29 feet in 17 consecutive patients who underwent either CLO or DA. The mean age at surgery was 11.3 ± 2.3 years, and the mean follow-up duration was 7.7 ± 3.2 years. Preoperative and final follow-up radiographs and dynamic foot-pressure measurements were analysed.
RESULTS: Both operatio

The objective of this study was to identify bone fractures and joint dislocations that have greater association with the severity of arch collapse in patients with Charcot foot involving the midfoot.A retrospective study in 28 (N = 29 feet) patients who had Charcot foot deformity of the midfoot. The study included stage III of Eichenholtz classification, and Schon classification types I to III. Talar-first metatarsal and calcaneal pitch angles and cuboid height were used to evaluate the severity of the midfoot deformity in a weightbearing lateral radiograph. Two investigators evaluated the bon

BACKGROUND: The objective of this study was to compare progressive collapsing foot deformity (PCFD) classifications performed using clinical and conventional radiographs (CR) with classifications established using clinical and weightbearing computed tomography (WBCT).
METHODS: This retrospective comparative study evaluated 89 consecutive PCFD feet (84 patients). Three readers performed chart reviews and CR evaluations, determining PCFD classifications that were previously published. After a washout period, the sequence was randomized, and a new classification was executed using clinical and W

BACKGROUND: Gastrocsoleus lengthening (GSL) is the most common surgical procedure to treat equinus deformity in ambulant children with cerebral palsy (CP). Foot drop, where the ankle remains in plantarflexion during swing phase, can persist in some children post-operatively. There is currently limited understanding of which children will demonstrate persistent foot drop after GSL.
RESEARCH QUESTION: Which children develop persistent foot drop after GSL surgery for equinus?
METHODS: We conducted a retrospective cohort study on ambulant children with CP who had GSL surgery for fixed equinus de

BACKGROUND: Middle facet subluxation (MFS) has been established as an early indicator of peritalar subluxation. However, when progressive collapsing foot deformity (PCFD) affects the ankle leading to a valgus talar tilt (Class E), structures and anatomic relationships distal to the ankle joint may be affected. Therefore, this study aimed to assess radiographic parameters of peritalar subluxation in patients with PCFD who either did or did not have a valgus ankle. Our hypothesis was that these parameters would differ in Class E patients, upsetting their capability to quantify deformity.

Over the last two decades there is a growing interest in the adult literature for subtalar joint arthroereisis. Parallel to this interest, there have been improvements in the design and biomechanics of the implant, although the main indication of subtalar joint arthroereisis in adults is not clear. Most studies show significant improvement in postoperative clinical scores and visual analog scores. Sinus tarsi pain, being the most common complication, is the main determinant of clinical satisfaction. This review focuses on the role and complications of subtalar joint arthroereisis in the adult

BACKGROUND: Semiautomatic 3-dimensional (3D) biometric weightbearing computed tomography (WBCT) tools have been shown to adequately demonstrate the relationship between the center of the ankle joint and the tripod of the foot. The measurement of the foot and ankle offset (FAO) represents an optimized biomechanical assessment of foot alignment. The objective of this study was to evaluate the correlation between FAO and traditional adult acquired flatfoot deformity (AAFD) markers, measured in different planes. We hypothesized that the FAO would significantly correlate with other radiographic mar

RECOMMENDATION: Progressive collapsing foot deformity (PCFD) is a complex 3D deformity with varying degrees of hindfoot valgus, forefoot abduction, and midfoot supination. Although a medial displacement calcaneal osteotomy can correct heel valgus, it has far less ability to correct forefoot abduction. More severe forefoot abduction, most frequently measured preoperatively by assessing talonavicular coverage on an anteroposterior (AP) weightbearing conventional radiographic view of the foot, can be more effectively corrected with a lateral column lengthening procedure than by other osteotomies
