A unique method for closed reduction of lumbosacral fracture dislocations that uses a circle-electric bed is described. Fine radiological description of the injury with computed tomography scanning allows the application of appropriate forces to facilitate the reduction, while having the patient awake during the procedure greatly reduces the chance of introducing further neurological deficit.

A review of 61 patients with dislocation of the lunate (some with and some without fracture of the scaphoid) showed that the majority had satisfactory results at an average follow-up of three and a half years. Most patients with a simple dislocation had a good or satisfactory result; radiological instability was noted in a quarter of the wrists but was not often associated with symptoms. Two-thirds of the patients with an associated fracture of the scaphoid had a good or satisfactory result. Immediate percutaneous wire fixation of the reduced scaphoid, whether it is fractured or not, is the be

A consecutive series of 2579 patients with 2571 fractures and 189 dislocations was studied prospectively. Males and children, especially under 10 years of age, predominated. The majority of the injuries resulted from falls either in the home or outdoors and from road traffic accidents. Fractures occurred more commonly in the upper limb than in the lower limb or the axial skeleton. Fractures of the shaft of the femur were three times more common, while fractures of the neck of the femur were less common by the same factor, in Africans than in Caucasians. Dislocations were commonest at the shoul

Correct diagnosis of fractures and dislocations of the cervical spine in children requires an awareness of the pseudosubluxation and other normal variants noted in x-ray films of patients in this age group. Fractures of the odontoid process occur frequently and almost always can be treated by closed reduction and external fixation until union occurs. Fractures of the lower cervical spine are difficult to detect roentgenographically, and x-ray changes can belie the severity of soft tissue injury and cord trauma. Instability tends to persist in adolescent patients after cervical spine injury bec

OBJECTIVES: Posterior malleolar fractures (PM) have been linked to inferior outcome in malleolar fractures. This study aims to analyze the prevalence and pathoanatomy of PM fractures in Bosworth fracture-dislocations (BF).
MATERIALS & METHODS: Radiographs and computed tomography (CT) scans of 13 patients treated at our institution and 97 cases published between 1947 and 2018, identified in a systematic literature search, were evaluated with respect to the pathoanatomy of BF. In all 13 cases from the present study and in 10 cases from the literature, axial CT scans were performed.
RESULTS: Al

Fractures and dislocations of the carpal bones are more common in young active patients. These injuries can lead to pain, dysfunction, and loss of productivity. Conventional radiography remains the primary imaging modality for evaluation of suspected carpal fractures and dislocations. However, multidetector computed tomography (CT) is playing an increasingly important role, especially in the following situations: (a) when results from initial radiographs are negative in patients with suspected carpal fractures, (b) when initial radiographic findings are indeterminate, and (c) when knowledge of

Thirty-eight patients with unstable thoracolumbar and lumbar (T12-L5) spine fractures were treated with Cotrel-Dubousset pedicle screw instrumentation. They were followed for an average of 22.73 months (range 12-39). Measurements of canal compromise, wedge index, and kyphosis correction at follow-up examination were made. Thirty-three (86%) patients responded to a questionnaire concerning overall satisfaction, use of pain medication and return to work. There was an overall correction of kyphosis at follow-up of only one degree after a loss of six degrees from operative correction. There were n

Open reduction and internal fixation was employed in the treatment of 25 severely displaced fractures and fracture-dislocations of the proximal humerus. Our aims were accurate reduction and stable fixation to allow early mobilisation and to achieve full functional recovery. In 15 fractures an AO T-plate was used and in 10 a bent semitubular plate was employed as a blade plate. Excellent or satisfactory results were obtained in all six patients with two-part fractures involving the surgical neck; in four of the five patients with three-part fractures involving the surgical neck and tuberosities

41 cases of posterior sprain fracture are analysed by the authors. 4--10 years after the injury 31 patients appeared at the check-inspection. On the basis of the late results it is stated by the authors that the immediate diagnosis and the reposition of the luxation are of the greatest importance. Conservative treatment yields generally satisfactory results. In the authors' opinion, osteosynthesis is indicated but in fractures concomitant with the fracture and dislocation of great piece of the acetabulum and in juvenile patients.

Sixty patients have undergone surgical reconstruction of the injured thoraco-lumbar spine during the period 1968 to 1978 at the Spinal Unit of the National Murayama Hospital. The aim of treatment of any fracture, dislocation, or fracture-dislocation of the spine is to render it stable and painfree. This is best achieved by reduction and stabilisation of the involved segment of the spinal column. Where the posterior ligaments and intervertebral disc are ruptured healing of these structures with return to normal stability cannot be anticipated. In order to obtain permanent stability at the affec

Because of its flexibility and structure, the cervical spine is disposed to various mechanisms of injury: although not so common as injuries caused by head impacts, cervical fractures and/or fracture-dislocations have been reported without direct impact to the head. Some cervical injuries reported have been sustained by wearers of lap and shoulder belts in auto accidents; however, we do not consider belt use a potential hazard because ample evidence has accrued in the medical and engineering literature to document general injury and fatality reduction by use of seatbelts. We believe that in ma

We describe two patients with an Essex-Lopresti fracture dislocation in association with a dislocation of the elbow. This combination of injuries has not been previously reported. The Essex-Lopresti fracture is a rare injury, and the associated distal radioulnar dislocation is often missed. Meticulous radiographic evaluation of the wrist, including dynamic stress radiographs of the forearm, can lead to an earlier diagnosis and improved care of these patients.
