Mitochondrial dysfunction, cerebral metabolic crisis, and exploratory bioenergetic biomarkers in a translational swine model of acute carbon monoxide poisoning.

Carbon monoxide (CO) poisoning remains a major cause of toxicologic morbidity and mortality and is a leading cause of acute neurologic injury among poisoned patients, yet the mechanisms underlying cerebral bioenergetic dysfunction remain incompletely understood. In addition to impaired oxygen delivery through carboxyhemoglobin (COHb) formation, CO poisoning is associated with mitochondrial respiratory dysfunction and cerebral metabolic injury. We characterized systemic physiology, cerebral metabolism, mitochondrial bioenergetics, and exploratory translational biomarkers in a swine model of acu

The effects of levamisole poisoning on the haematological and biochemical parameters in dogs.

This study was designed to evaluate possible organ and system disorders associated with experimentally induced levamisole poisoning in dogs. For this purpose, twelve clinically healthy dogs of different ages, sexes and breeds were used. They were divided into two equal groups (Group A and Group B) and given levamisole orally at a dose of 25 mg/kg of body weight daily for three days. The dogs in Group B were also injected with atropin sulphate (0.04 mg/kg of body weight) subcutaneously (sc) 1 hour after each administration of levamisole. Routine clinical examinations were made and some haematol

Carbon monoxide related deaths: A Verona case series. When cooperation becomes compulsory.

INTRODUCTION: Carbon monoxide (CO) poisoning is a significant concern in forensic medicine, as it often presents unique challenges in terms of diagnosis, investigation, and determination of the cause of death. CO is a colourless, odourless, and tasteless gas that can be lethal when inhaled in high concentrations. It binds strongly to haemoglobin, forming carboxyhaemoglobin (COHb), which reduces the oxygen-carrying capacity of the blood, leading to tissue hypoxia and ultimately death. MATERIALS AND METHODS: Circumstantial data, medical history information, autopsy findings, and toxicological a

Combined carbon monoxide and cyanide poisoning: a place for treatment.

During fires, victims can inhale significant carbon monoxide (CO) and cyanide (CN) gases, which may cause synergistic toxicity in humans. Oxygen therapy is the specific treatment for CO poisoning, but the treatment of CN toxicity is controversial. To examine the indication for treatment of CN toxicity, we have established a canine model to delineate the natural history of combined CO and CN poisoning. In seven dogs (24 +/- 3 kg), CO gas (201 +/- 43 mL) was administered by closed-circuit inhalation. Then, potassium CN was intravenously (i.v.) infused (0.072 mg.kg-1.min-1) for 17.5 +/- 3.0 min.

Acute methemoglobinemia- Diagnosing and managing in family practice.

Methemoglobinemia is a rare but serious disorder in which hemoglobin is oxidized to methemoglobin, impairing oxygen transport. This condition can present in primary care with cyanosis and oxygen desaturation and is potentially misdiagnosed as respiratory or cardiac failure. We present the case of a 38-year-old male driver who was brought to the emergency department unconscious, with cyanosis, and unresponsive to oxygen therapy. His oxygen saturation was 86% on pulse oximetry, although arterial blood gas analysis showed a normal partial pressure of oxygen. The source of poisoning was unknown at

Successful Rescue of Deep Coma, Shock, and Arrhythmia Caused by Bittern Poisoning.

Bittern has been used as the main ingredient of tofu coagulant, and poisoning events usually occurred in the rural of North China. Bittern intoxication may induce electrolyte imbalances, the most important of which is hypermagnesemia, and may be fatal if ingested. The patient without any medical history presented with sinoatrial block, atrioventricular block, paroxysmal atrial tachycardia, and atrial fibrillation on electrocardiography over a short period. The initial diagnosis was unclear and the treatment was difficult. Arterial blood gas analysis indicated severe metabolic acidosis. Her ser

Plasma metabolic profiling analysis of toxicity induced by brodifacoum using metabonomics coupled with multivariate data analysis.

Brodifacoum is one of the most widely used rodenticides for rodent control and eradication; however, human and animal poisoning due to primary and secondary exposure has been reported since its development. Although numerous studies have described brodifacoum induced toxicity, the precise mechanism still needs to be explored. Gas chromatography mass spectrometry (GC-MS) coupled with an ultra performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS) was applied to characterize the metabolic profile of brodifacoum induced toxicity and discover potential biomarkers in rat plasma. Paraquat lung: a reappraisal.

Smith, P. and Heath, D. (1974).Thorax, 29, 643-653. Paraquat lung: a reappraisal. The histopathology of the lungs from four human cases of paraquat poisoning is described. In two of them there was a dense mass of fibroblastic tissue which obliterated the lung architecture, and one of these showed an extensive deposit of collagen with honeycomb change. In a third case pulmonary fibrosis was less severe and could be demonstrated exclusively within the alveolar spaces. The fourth showed earlier lesions of capillary congestion, alveolar oedema, and hyaline membrane formation. We suggest that paraq

Indoor pollutant levels from the use of unvented natural gas fireplaces in Boulder, Colorado.

High CO and NO2 concentrations have been documented in homes with unvented combustion appliances, such as natural gas fireplaces. In addition, polycyclic aromatic hydrocarbons (PAH) are emitted from incomplete natural gas combustion. The acute health risks of CO and NO2 exposure have been well established for the general population and for certain high-risk groups, including infants, the elderly, and people with heart disease or asthma. Health effects from PAH exposure are less well known, but may include increased risk of cancer. We monitored CO emissions during the operation of unvented natu

[Case history of a gassed patient].

Ricardo T., aged 20 years, was a passenger in a Varig air-craft which suffered an accident on July 11th 1973. A fire developed on board whilst the plane was in the air, at about 3 p.m. After emergency landing, rescue workers (firemen and emergency ambulance teams) discovered only two survivors amongst the 122 passengers. One was in his forties and died a few moments later. The other (Ricardo) was rapidly transported to the surgical and traumatological intensive care unit of the Henri Mondor Hospital. At the time of arrival, he was in stage 1 coma with skin burns (3rd degree), burns of the airw

Nitric oxide: a greenhouse gas is used in the treatment of respiratory failure.

Medical science has long made the improbable probable, saving lives and improving quality of life. Upon the introduction of medical devices that can deliver safe quantities of the poisonous gas nitric oxide to help patients with respiratory and other illnesses, the FDA requested ASTM Committee F29 on Anesthetic and Respiratory Equipment to provide standards for these devices. Dr. Daniel Supkis and Mark Graber explain the delicate process of delivering NO to patients and how ASTM standards now in development will increase the safety of this procedure.

Type A fulminant Clostridium perfringens sepsis indicated RBC/Hb discrepancy; a case report.

BACKGROUND: Clostridium perfringens can cause various infections, including food poisoning, gas gangrene, cellulitis and fasciitis. C. perfringens septicemia is rare, but is a known cause of hemolysis by damaging red blood cell, and often proves rapidly fatal in emergency department (ED) situations. CASE PRESENTATION: A previously healthy 76-year-old man presented to the ED 8 h after onset of acute abdominal pain and diarrhea. Laboratory examination revealed a large discrepancy between the red blood cell count of 1.91 × 106/mm3 and the hemoglobin level of 10.3 g/dL, suggesting massive intrava

Characterization of two putative fibronectin-binding proteins of Clostridium perfringens.

Clostridium perfringens is a Gram-positive anaerobic pathogen that causes gas gangrene and food poisoning in humans and animals. Genomic analysis of C. perfringens strain 13 revealed that this bacterium contains two genes (CPE0737 and CPE1847) that encode putative fibronectin (Fn)-binding proteins (Fbps). These genes, named fbpA and fbpB, were found to be constitutively expressed in all three strains (13, NCTC8237, CPN50) of C. perfringens, isolated from gas gangrene of human, that were tested. Both fbpA and fbpB were cloned and His-tagged, recombinant FbpA (rFbpA) and recombinant FbpB (rFbpB)

A clinicopathological study on acute cutaneous lesions induced by sulfur mustard gas (yperite).

During the Iran-Iraq war (1980-1988), sulfur mustard (as a chemical warfare agent) was launched on several occasions. Thirty-two victims with acute mustard poisoning were referred to our dermatology department, and are basis of this clinicopathologic study. Clinical and laboratory findings of these 32 adult patients exposed to mustard gas were determined. Skin biopsies were obtained from all of the patients and studied after staining of the specimens with routine and special stains. Clinically the most frequently involved areas were genitalia, face and axilla. The most common cutaneous finding
