OBJECTIVE: To evaluate the utility of ultrasound (US) guidance in the percutaneous placement of gastric feeding tubes in patients in whom placement of a nasogastric tube is not possible.
PATIENTS AND METHODS: Records from feeding tube placements performed between January 1991 and August 1994 were reviewed. Of the 238 procedures performed, 27 cases (11%) involved initial US guidance, rather than nasogastric tube assistance, because of upper gastrointestinal obstruction.

A procedure of transcutaneous draining of bile ducts through a guided replaceable drain using the gallbladder and formation of cholecystostomy and cholecystohepatico-, gastro entero- and enteroenteroanastomoses is suggested for palliation of pancreatic cancer patients. The procedure offers a good clinical effect assuring a significantly longer survival as compared to standard palliative operations used for pancreatic cancer treatment.

During the last 100 years, since César Roux began utilizing his "Ansa-en-Y" procedure for gastric outlet obstruction, the procedure at first fell into disuse but was later adapted for other applications. This article discusses the background of the procedure and compares the original indications with the author's current experience. Current applications in this series were for chronic pancreatitis (26 patients), alkaline gastritis (22 patients), biliary duct obstruction (7 patients), pancreatic pseudocyst (3 patients), and gastric substitution (3 patients).

The incidence of duodenogastric reflux (DGR) was studied in 241 of 376 patients operated upon for duodenal ulcers. Rentgenological, endoscopic and laboratory examinations were performed prior to and after vagotomy and vagotomy with draining operations. A great number of DGR was established in all variants of operations. With the help of multifactorial analysis it was shown that the Finney pyloroplasty was followed by greater amount of postvagotomy complications than gastroduodenal anastomosis (GDA) after Jaboulay. The article deals with the problem of the functional status of the cardia in duodenal ulcer. The cardial function was studied in 27 patients with complicated duodenal ulcer. The results make it possible to judge about the degree of manifestation of cardia insufficiency. While tackling the problem of a character of surgical intervention upon the duodenal ulcer cases it is necessary, in author's opinion, to take into consideration the results of the functional examination of the cardia.

To assess whether or not a persistent antral response to meat extract stimulation is an indication of incomplete vagotomy, 29 patients with a duodenal ulcer who underwent operation had preoperative and postoperative acid studies as well as postoperative Hollander tests.
