Diaphragmatic Mitochondrial Myopathy in a Patient-Derived Mouse Model of Barth Syndrome.

Barth syndrome (BTHS) is a rare, X-linked genetic disorder caused by mutations in the enzyme TAFAZZIN (TAZ), resulting in insufficient cardiolipin (CL) remodeling and mitochondrial dysfunction. While BTHS respiratory distress and breathing difficulties are commonly reported, the precise role of intrinsic respiratory tissue vulnerabilities has only recently begun to be appreciated. Historically, BTHS respiratory distress is frequently attributed to secondary consequences like cardiomyopathy or generalized skeletal myopathy, leaving the intrinsic vulnerability of vital respiratory muscles poorly

Biological responses to 30 mT static magnetic field in young and 36-month-old rats.

This study aimed to investigate the effects of subchronic exposure to a 30 mT static magnetic field (SMF) on hematological parameters, splee … 36-month-old rats, SMF may act as a nonspecific physiological stressor, potentially triggering the General Adaptation Syndrome (three-stage stress response), leading to corticosterone-mediated immunosuppression and cell redistribution. To our knowledge, this is the first study demonstrating age-dependent differential modulation of NLR and PLR by subchronic SMF exposure, lin …

Successful Closure of Multiple Post-bariatric Surgery Fistulas Using Modified Internal and External Endoscopic Vacuum Therapy: A Case Report.

Leaks and fistulas following bariatric surgery are serious complications associated with sepsis, malnutrition, and high morbidity. Endoscopic vacuum therapy (EVT) has emerged as an effective minimally invasive alternative for the management of transmural defects of the upper gastrointestinal tract. Recently, low-cost modifications using conventional hospital supplies have been developed to expand their applicability in resource-limited settings. We present the case of a 46-year-old female patient with a history of multiple bariatric surgeries: sleeve gastrectomy (2017), Roux-en-Y gastric bypas

Health State Utility Associated with Parenteral Nutrition Requirement in Patients with Short Bowel Syndrome and Intestinal Failure in Korea: A Vignette-Based Approach.

Background/Objectives: Short bowel syndrome (SBS) is the leading cause of intestinal failure (IF) that often requires long-term parenteral nutrition (PN). Extended PN results in severe complications and reduced quality of life (QoL). This study aimed to evaluate the QoL utility weights associated with PN duration using vignettes. Methods: We developed detailed scenarios and descriptions to represent eight hypothetical health states, reflecting variations in PN frequency in both pediatric and adult patients. A cross-sectional survey was conducted among 359 Korean adults (aged 19-59 years) from

Lateral Atrial Tunnel Fontan Operation Predisposes to the Junctional Rhythm.

The goal of the study was to compare the early postoperative course after fenestrated lateral atrial tunnel (LT) and non-fenestrated extracardiac conduit (EC) Fontan operation (FO) in a single center where both techniques were parallelly used. Between 2004 and 2012, 56(32.7%) children underwent LT and 113(66.1%) EC FO. The mean age was 3.7 ± 2.9 years and mean weight was 14.6 ± 7.3 kg. The most common malformation was hypoplastic left heart syndrome (50.9%). The general approach was to perform LT in children after hemi-Fontan operation and EC in patients after Glenn anastomosis. Influence of stress in acute pancreatitis and correlation with stress-induced gastric ulcer.

BACKGROUND AND AIMS: In the general adaptation syndrome, gastric lesions are the first manifestation of stress. We hypothesized that acute pancreatitis (AP), an inflammatory acute disease, will be exacerbated if unchained following stress. Visceral hypersensitivity will be enhanced due to catecholaminergic discharges leading to an over-induction of the intrapancreatic cholinergic tone with increased response of the pancreocyte to cholecystokinin (CCK). Our aim was to investigate the influence of stress before AP on the later AP, and the effect of AP on underlying diseases such as gastric ulcer…

Renal aspects of the term and preterm infant: a selective update.

This review discusses new aspects of normal and abnormal renal development that expand insight into the adaptation of the neonatal kidneys to the stress of extrauterine life. Highlighted are some pitfalls in measuring glomerular filtration rate in the neonate mainly caused by postnatal fluctuations in serum creatinine levels. Serum creatinine levels are correlated with the authors' recent finding of tubular reabsorption of creatinine in the immature neonatal kidney. Renal maldevelopment in premature and small-for-date babies has been shown related to serious medical problems in adult life, inc

Intravenous nutritional support and the surgeon: where next?

Over the past twenty five years the development of total parenteral nutrition has in many ways revolutionised the practice of surgery. It has enhanced survival in otherwise high mortality operations such as oesophageal surgery, especially with anastomotic complications. It has changed significantly the management of fistulae, either post operative or associated with diseases such as Crohn's enteritis. Here a basic general principle is applied--that a fistula will close if there is no distal obstruction and the throughput can be diminished. This can be achieved by withholding oral feeding and u

Generation of a new infectious recombinant prion: a model to understand Gerstmann-Sträussler-Scheinker syndrome.

Human transmissible spongiform encephalopathies (TSEs) or prion diseases are a group of fatal neurodegenerative disorders that include Kuru, Creutzfeldt-Jakob disease, Gerstmann-Sträussler-Scheinker syndrome (GSS), and fatal familial insomnia. GSS is a genetically determined TSE caused by a range of mutations within the prion protein (PrP) gene. Several animal models, based on the expression of PrPs carrying mutations analogous to human heritable prion diseases, support that mutations might predispose PrP to spontaneously misfold. An adapted Protein Misfolding Cyclic Amplification methodology

Critical Literature Review on Clinical Presentation of Oncocytic Thyroid Carcinoma with Immunoendocrine Complications and Unpredictable Outcome: Myths, Facts, and Their Overinterpretation.

Objectives: Endocrine neoplasms, as a general rule, show systemic, neuro-inflammatory and metabolic consequences, known as paraneoplastic syndrome. The comorbidity of thyroid tumors with neurological and autoimmune diseases prompt looking for common neuro-immuno-endocrine mechanisms of these disorders. While most TCs are well described, there is a gap in the literature after the isolation of oncocytic/Hürthle cell carcinoma (HCC), as a unique type due to immunoendocrine and metabolic features (low TSH-receptor expression and radioiodine avidity). The aim of this study was to collect clearly de

Early-life stress and HPA axis trigger recurrent adulthood depression.

It is now broadly accepted that psychological stress may change the internal homeostatic state of an individual. During acute stress, adaptive physiological responses occur, which include hyperactivity of the HPA axis. Whenever there is an acute interruption of this balance, illness may result. The social and physical environments have an enormous impact on our physiology and behavior, and they influence the process of adaptation or 'allostasis'. It is correct to state that at the same time that our experiences change our brain and thoughts, namely, changing our mind, we are changing our neuro

[Nutrition of diabetic children in 1989].

Diabetic children should consume the number of calories normal for their age, with an ideal % nutrient distribution: +/- 55% carbohydrate (especially delayed-resorption carbohydrates), +/- 30% lipid and +/- 15% protein. In some countries e.g. Belgium, the excessive proportion of fat in the diet must be reduced. Calorie requirements may vary a lot from one day to the next, but nutrient fluctuations do not entail proportional changes in metabolic control, as long as insulin adaptation is adequate. It is essential that diabetic children adapt their food intake to their appetite; they do not have

Immune Toxicities in AAV Gene Therapy: Overview for Clinicians.

Gene therapy using recombinant adeno-associated virus (rAAV) vectors has emerged as a transformative therapeutic modality for genetic disorders, demonstrating high transduction efficiency and a generally favorable safety profile during pre-clinical development. However, serious adverse events, including thrombotic microangiopathy, acute respiratory distress syndrome, hepatotoxicity, myocarditis, cytokine storm, and hemophagocytic lymphohistiocytosis, have been observed across multiple gene therapy clinical trials. Significant efforts have been made to understand the toxicities that cause these

Transplant Trial Watch.

Cancer cachexia represents a debilitating syndrome that diminishes quality of life and augments the toxicities of conventional treatments. Cancer cachexia is particularly debilitating in patients with pancreatic cancer (PC). Mechanisms responsible for cancer cachexia are under investigation and are largely derived from observations in syngeneic murine models of cancer which are limited in PC. We evaluate the effect of human PC cells on both muscle wasting and the systemic inflammatory milieu potentially contributing to PC-associated cachexia. Specifically, human PC xenografts were generated by

The Facial Osteoplasty for Polyostotic Fibrous Dysplasia in a Patient With McCune-Albright Syndrome: A Case Report.

Fibrous dysplasia is a developmental anomaly that affects bone maturation and remodeling, generating replacement of medullary and cortical bone by a disorganized and immature fibro-osseous matrix, which makes the sufferer susceptible to bone pain, skeletal deformities, and pathological fractures. This is a condition that, when associated with cutaneous hyperpigmentation and endocrinological disorders, forms the classic triad of McCune-Albright syndrome, a rare multisystemic pathology formed by postzygotic somatic mutations of the GNAS gene. Fibrous dysplasia can even cause loss of vision, hear

Visualizing a role for the actin cytoskeleton in the regulation of B-cell activation.

Appropriate activation of B cells is required for mounting protective humoral immune responses. B-cell activation is initiated following specific recognition of antigen by the B-cell receptor (BCR) and results in the generation of antibody-secreting plasma cells and long-lived memory cells. Initial imaging approaches revealed that B cells undergo dramatic molecular and morphological reorganizations following recognition of antigen. A number of these studies pointed to a role for the underlying cytoskeleton in regulating early events of B-cell activation. More recently, groundbreaking advances

Role of the central nervous system in the adult respiratory distress syndrome.

The CNS might play an important pathophysiologic role in the induction of the adult respiratory distress syndrome (ARDS). Post-traumatic development of cerebral microthromboses seems to be a major feature. To test this hypothesis, we subjected Wistar rats with postburn ARDS to the following microneurosurgical procedures in an attempt to morphologically detect modifications in the evolving pattern: a) anterior (AH) or posterior (PH) hypothalamus nuclei stereotaxic electrolytic lesion; b) intracerebroventricular microinjections of phenoxybenzamine (PB)-50, micrograms propranolol-0.5 micrograms,
