Familial malignant osteopetrosis in children: a case report.

The clinical, radiological, pathological and laboratory findings of two brothers with autosomal recessive malignant osteopetrosis are presented. Our findings are similar to characteristics previously reported in the literature about patients with osteopetrosis. The 6-year-old male patient was pale and had petechiae on his arms and legs. He also had macrocephalia, splenomegaly, severe pancytopenia, genu valgus, hypocalcemia, amaurosis, cessation of physical development, generalized bone sclerosis and recurrent infections with a history of multiple incidences of acute otitis media. Generalized b

Paraneoplastic epidermolysis bullosa acquisita associated with multiple myeloma.

Epidermolysis bullosa acquisita is a rare acquired autoimmune subepidermal blistering disease that clinically resembles other vesiculobullous lesions such as pemphigus vulgaris and cicatricial pemphigoid. Multiple myeloma is the most common plasma cell malignant disorder characterized by a single clonal expansion and increased level of a single immunoglobulin. Epidermolysis bullosa acquisita has been reported with other systemic diseases such as lymphoma. In this case report, we present a patient with paraneoplastic epidermolysis bullosa acquisita associated with multiple myeloma.

Thalidomide in the management of recurrent aphthous ulcerations in patients who are HIV-positive: a review and case reports.

Recurrent Aphthous Ulceration (RAU) is an inflammatory ulcerative disease of unknown etiology. Although RAU are common in the general population, people with HIV disease may have ulcers that are large and painful and take a long time to heal. Local and systemic steroid therapy has been the traditional method of treatment for recurrent lesions. Recent studies have shown that systemic thalidomide therapy has the best efficacy in the treatment of major RAU in HIV-seropositive patients. Two case histories of complete resolution of the RAU are presented. We have included recommended guidelines for

A systematic review of oral health assessment by nurses and carers for residents with dementia in residential care facilities.

This paper presents systematic review findings to best summarize the assessment of oral health and the use of oral assessment tools by nurses and carers for adults with dementia living in residential aged care facilities. The systematic review searched electronic databases for articles in English (1980 to 2002) and supplemented these with a secondary search of references cited in articles meeting the review inclusion criteria. Delineation is needed between a comprehensive dental examination conducted by a qualified dentist and a dental assessment screening by a carer, nurse, allied health prof

Effect of reinforced laryngeal mask airway or endotracheal intubation anesthesia on adverse events: quality of life issues for dental patients requiring general sedation.

This study tested whether the use of a reinforced laryngeal mask airway (LMA) compared to endotracheal intubation (El) anesthesia diminished adverse events and improved the post-operative quality of life for 146 dental patients who required general anesthesia due to developmental disabilities. In this randomized, crossover trial, patients received either reinforced LMA or El anesthesia during an initial dental treatment visit; the other anesthetic method was used during a second visit scheduled after six months or more. Data were assessed with Student's t-test, chi-square test and McNemar's te

The perceptions of homeless people in Stockholm concerning oral health and consequences of dental treatment: a qualitative study.

This study investigated the perceptions of Swedish homeless people concerning their oral health and perceived consequences of dental treatment. Candid, tape-recorded interviews were conducted in a conversational style. A phenomenological-hermeneutical method was used to analyze the subjects' stories. New participants were recruited into the study, until the interviews provided no additional new information, which occurred after eight interviews. All narratives revealed expressions of loss as well as recovery in the informants' life. Both aspects highlighted the fact that homelessness equated t

Xylitol in the prevention of oral diseases.

Special needs populations such as the mentally disabled or developmentally disabled present a unique challenge for the dental professionals providing dental treatment. Among those challenges is oral health promotion through primary prevention of dental caries and periodontal disease. There are many barriers to adequate oral care for these special needs patients that include cost, physical limitations, low levels of perceived need and lack of dental knowledge by residents and staff, if the person resides in an institution. Primary prevention to maintain oral comfort and function for these patie

Dental hygienists as providers in long-term care facilities.

The authors surveyed all practicing dental hygienists (n = 870, response rate = 69%) in Kansas about providing preventive dental care to elderly residents of long-term care (LTC) facilities. Maximum likelihood estimates stepwise logistic regressions were used to predict the likelihood of dental hygienists filling the need for a specialty workforce. Few respondents wanted to work exclusively with elderly patients. Lack of interest in LTC practice was related to the number of children living at home or discomfort with patients who had limited abilities to communicate. Interest in part-time geria

Development of a device for removing a partial denture in a patient with scleroderma.

Scleroderma is a chronic disease that has been associated with immune dysfunction. One of the oral manifestations is microsomia, a result of collagen deposition in the perioral issues. The complexity of treating these patients includes limited mouth opening ability, and difficulty inserting and removing dentures due to finger deformity. This article will describe an appliance specially designed especially for scleroderma patients, which facilitates treatment of the patient with removable partial dentures (RPD).

Prosthodontic management of older severe hypodontia patients in general dental practice: a case series.

Although a detailed understanding of the etiology of hypodontia is lacking, there is a need for awareness among dental professionals of the social and psychological consequences of severe hypodontia, as well as knowledge of those affected by hypodontia have ongoing dental treatment needs. Although, there may be a need for immediate referral of children to different specialists for optimal multidisciplinary treatment planning, most older patients with severe hypodontia can be treated by general practitioners. Practitioners therefore need to be aware of the ramifications of the condition and be

Granulocytic sarcoma: an atypical presentation in the oral cavity.

Acute myelogenous leukemia (AML) is a hematologic disorder that is characterized by an abnormal proliferation of immature myeloid cells. Granulocytic sarcomas are clusters of leukemic myeloid cells that may develop as a result of AML. Oral manifestations of AML are common and often involve enlargements of the gingiva and/or mucosal tissue from direct leukemia cell infiltration. We describe the case history of a 50-year-old man who had an ulcerative lesion of the oral mucosa that was determined to be a granulocytic sarcoma of AML-MO subtype. The combination of both the subtype and clinical pres

Oral health for people with intellectual disability and high support needs: positive outcomes.

This qualitative study, based on phenomenological approaches, explored and documented four situations in which positive oral health outcomes occurred for people with mental retardation and moderate to high support needs. Strategies and environmental factors that contributed to these oral health outcomes were identified through 10 semistructured interviews conducted with 'key-players' supporting the oral health of the people with disabilities. Participants included dental professionals, direct support workers, and other professionals who cared for their four people with disabilities. Common str

Practical protocols for the prevention of dental disease in community settings for people with special needs: the protocols.

People with special needs have more dental disease and more missing teeth than the general population. They also have reduced access to oral health diagnostic, preventive, interceptive and treatment services. If services are available, they can be complicated and costly. It is critical to prevent dental diseases in these individuals. This article presents a set of practical protocols for preventing dental disease in people with special needs. These protocols are designed to be used in community settings outside of a dental office.

Leukemic gingival infiltrate as an indicator of chemotherapeutic failure following monoclonal antibody therapy: a case report.

Treatment options are limited for patients with relapsed acute myelogenous leukemia (AML), particularly when the disease is refractory to standard cytotoxic chemotherapy. Targeted drug therapy offers the advantage of delivering higher doses of non-cross resistant chemotherapy with potentially less systemic toxicity. Gemtuzumab ozogamicin (Mylotarg, Wyeth-Ayerst Laboratories) is an immunoconjugate that consists of humanized anti-CD 33 antibody linked to the potent anti-tumor antibiotic calicheamicin and has been an effective therapy for some patients with relapsed AML. However, the overall util

Irrigational therapy of a dentigerous cyst in a geriatric patient: a case report.

Dentigerous cysts are recognized as one of the most common lesions of the jaws. Although surgical enucleation is usually the preferred treatment method, it is not always the best choice for elderly patients who have other medical complications. With cyst irrigational therapy, repeated irrigation allows bone regeneration around the cyst to progress more rapidly than that around a cyst treated operatively with marsupialization. We used irrigational therapy to treat a dentigerous cyst in a 72-year-old man who did not agree to surgery because he had no symptoms associated with the cyst, and he had

Stimulated parotid salivary flow rate in patients with Down syndrome.

Saliva is essential for oral defense against infections. Decreased salivary secretion may result in increased dental caries, oral mucosal changes, an altered sense of taste, difficulty in swallowing, and oral pain. A review of the literature reveals sporadic and contradictory reports on the use of sialometry and sialochemistry to explain the role of saliva in the oral health and well-being of subjects with Down syndrome. The present study documents parotid gland saliva secretion at different ages in a group of subjects with Down syndrome. Saliva was collected from 39 patients 11 to 62 years ol

Diet medications and valvular heart disease: the current evidence.

Since 1997, the appetite suppressant drugs fenfluramine-phentermine (fen-phen) and dexfenfluramine have been associated with the development of valvular heart disease. As a result, the drugs have been withdrawn from the market and interim recommendations for evaluation and treatment of exposed patients were issued. However, subsequent studies showed lower prevalence rates for valve abnormalities than were initially reported, and the possibility of such drug-induced lesions resolving has been raised. This article reviews relevant evidence and discusses concerns for the dental practitioner.

Use of dental implants in patients with Down syndrome: a case report.

Down syndrome is caused by trisomy of the 21st chromosome and is associated with well-described physical and systemic problems. Most people with Down syndrome have some degree of mental retardation as well as malformation of head and neck. Oral structures that are commonly affected include the tongue (macroglossia), abnormalities in the number and shape of teeth, and poor quality (osteoporotic-like) of alveolar bone and jaw. These oral malformations as well as a tendency toward poor cooperation in the dental office contribute to the belief among dentists that people with Down syndrome are not

The oral health of psychiatric in-patients in South Wales.

Little information is available regarding the oral health of hospitalized psychiatric patients. The aims of the study were: to quantify the oral health status and identify dental treatment needs of hospitalized psychiatric patients in South Wales; and to compare the oral health of subgroups within the population by their age, psychiatric diagnosis, psychotropic medication use, and length of stay. The total patient population of the hospitals involved in the study was 469, and 326 subjects (70%) took part in the study. The mean age of the subjects was 71.1 years, with 265 long-stay and 61 short
