Management of MACROGLOSSIA: Case Series and Suggested Algorithm.

PURPOSE: Idiopathic macroglossia is a rare entity of true tongue enlargement without an underlying etiology. There are only a few case repor … with tracheostomy and percutaneous endoscopic gastrostomy (PEG) tube placement followed by partial glossectomy (n = 5, 100%). We defined successful outcomes as 1) modifying the tongue to a functional, nonprotruding form, 2) tracheostomy decannulation and 3) PEG tube removal. Tracheostomy decannulation and PEG tube removal were achieved in 80% of the patients …

Beyond the boundaries of compartmental hemiglossectomy: a proposal for an anatomically based classification of surgical approaches to advanced oral tongue squamous cell carcinoma.

INTRODUCTION: In the last decade, compartmental surgery (CTS) has been the surgical approach of choice for advanced tumors of the tongue and oral floor. METHODS: Advanced tumors (cT3-T4) oral tongue squamous cell carcinoma (OTSCC) may extend beyond the lingual septum and involve the contralateral hemitongue, developing along the intrinsic transverse muscle. The disease may then involve the genioglossus muscle and, more laterally, the hyoglossus muscle. RESULTS: The surgical approach to the contralateral tongue must be guided by anatomic and anatomopathological principles to achieve a safe on

Surgical treatment for obstructive sleep apnoea.

Surgical therapy of the sleep apnoea/hypopnoea syndrome is an attractive option to many patients as it avoids using therapy nightly for the remainder of one's life. This article reviews the outcomes of the surgical options available. The role of uvulopalatopharyngoplasty is not yet clear. It may be useful second line therapy for some patients but cannot at present be advocated as first-line therapy. Mandibular maxillary osteotomy and advancement is an effective therapy of value in a minority of patients. Other procedures such as partial glossectomy and genioglossal advancement may also be of v…

Acoustic signal intensity analysis in patients with dysphonia rehabilitated with two different designs for tongue prostheses: A cross-over study.

STATEMENT OF PROBLEM: Studies on the significance of intensity on speech production with a tongue prosthesis are lacking, despite this being an important criterion in speech production in patients with total glossectomy. PURPOSE: The purpose of this cross-over clinical study was to assess acoustic signal intensity and frequency in patients with total glossectomy rehabilitated with mandibular dentures with 2 different attachment designs for the tongue prosthesis. MATERIAL AND METHODS: Seven participants initially received a mandibular denture with a tongue prosthesis attached using an acrylic…

Renal cell carcinoma metastatic to the tongue: a case report and review of the literature.

Renal cell carcinoma (RCC) metastatic to the head and neck region is quite rare. This report describes a case of RCC metastatic to the oral tongue presenting initially with a renal mass that evaded diagnosis by biopsy examination of the primary lesion and was eventually established as a papillary type RCC by lingual biopsy examination. The tongue mass progressed rapidly despite chemotherapy with interferon-α2b, caused difficulties with oral food intake, and thus necessitated removal by partial glossectomy. Treatment alternatives for lingual RCC metastasis include surgical resection for major f…

The efficacy of tongue resection in treatment of symptomatic macroglossia in the child.

Nine patients with symptomatic macroglossia, 3 with Beckwith-Wiedemann syndrome, 3 with Down's syndrome, and 3 with lymphatic malformation of the tongue, were evaluated for postoperative improvement following partial glossectomy. Symptoms evaluated were speech intelligibility, oral competence at rest, drooling, and normal deglutition. All patients with Down's syndrome and Beckwith-Wiedemann syndrome were improved postoperatively. All patients with Down's syndrome exhibited improved deglutition and reduced drooling. Only 1 patient (33%), however, achieved oral competence. Speech was improved po…

Periapical Cyst and Odontogenic Remnants on Osseous Mandibulectomy Margins: Two Diagnostic Pitfalls.

In this article, we describe 2 high-stake diagnostic pitfalls that occurred in a single patient having undergone partial glossectomy and marginal mandibulectomy for invasive squamous cell carcinoma. Margins were grossly uninvolved. Histopathology showed a moderately differentiated squamous cell carcinoma. Microscopic evaluation of margins was challenging, as the inferior osseous margin submitted en face showed spongiotic epithelium with mixed chronic and acute inflammation, representing a periapical cyst after correlation with the gross examination. Another section of the osseous margin showed…

Second primary squamous cell carcinoma arising in the skin of a pectoralis major myocutaneous flap 12 years after floor of mouth reconstruction.

Second primary squamous cell carcinoma (SCC) in the skin of a flap used for reconstruction of floor of mouth is rare. A case of SCC arising in the pectoralis major myocutaneous flap that was used to reconstruct the floor of mouth 12 years after subtotal glossectomy and resection of the floor of mouth is reported. The tumour is a second primary SCC arises in the skin of a myocutaneous flap without any obvious risk factors. Review of the literature reveals four previous similar reports and the findings are summarized. In summary, squamous epithelium of the skin flap after prolonged exposure to n…

[Influence of mandibulotomy approaches on oral function following radical resection of tongue carcinoma].

OBJECTIVE: This study aimed to compare the influences of postoperative oral function in patients with median or paramedian mandibulotomy during the radical resection of tongue carcinoma and to provide evidence for the choice of osteotomy location for mandibulotomy. METHODS: The clinical data of 126 patients who underwent combined radical neck dissection with mandibulectomy and glossectomy followed by simultaneous reconstruction were analyzed retrospectively. The patients were divided into two groups according to the position of mandibulotomy: median mandibulotomy group (median group, n=60) an…

Clear cell carcinoma of the base of the tongue: case report and literature review.

OBJECTIVE: Clear cell carcinoma (CCC) is a rare, low-grade malignant neoplasm of minor salivary glands. Clear cell carcinoma is composed of monomorphic clear cells that contain glycogen within a hyalinized stroma. The microscopic diagnosis can be challenging, and the treatment strategy has not been established. METHODS: We report a case of a 56-year-old woman with CCC in the base of the tongue. A literature review, including 22 published cases and our case, was performed to clarify the clinical characteristics of CCC in the base of the tongue. RESULTS: The exophytic tumor showed no infiltrat

[Indications for the surgical management of macroglossia in children].

Macroglossia means long or large tongue. Since it is difficult to establish clinical criteria for assessment of the size of the tongue, usually it is related to the size of the mandible. We present the indications for the surgical management of macroglossia in children and we discuss the results after the Becker operation. We also present the results in relation to the preservation of taste, the mobility of the tongue, the restoration of normal speech, disturbances of sensitivity, as well as the timing for the operation.

[Wide excision in cancer of the tongue: the choice of approach, modalities of reconstruction].

Surgical treatment of extensive carcinoma of the tongue requires reconsideration at the present time: firstly, because the efficacy of other therapeutic measures is rapidly suppressed by the size of the tumors, and there is not theoretical reason for excluding extensive surgery, and secondly, because esthetic and functional sequelae of these bloody treatments have been totally modified by the introduction of new reconstructive techniques. Based on a series of 16 cases treated during a one-year period, the conditions for surgery of vast malignant tongue tumors (T3, T4, N indifferent) are analyz

A Systematic Review and Meta-Analysis of Functional Tongue Reconstruction Using Dynamic Free Flap Designs.

BACKGROUND: The treatment of tongue malignancies frequently produces deficits in feeding, swallowing, and speech intelligibility. These have a significant impact on patients' quality-of-life. Free tissue transfer is the preferred method for reconstructing larger, complex defects, and aims to restore both mechanical and sensory function while protecting the airway. The role of dynamic free flap designs incorporating motor reinnervation remains unclear, particularly in cases with more extensive glossectomy defects. The objective of this systematic review is to evaluate the speech and swallow out…

Combined surgery and postoperative radiotherapy for carcinoma of the base of radiotherapy for carcinoma of the base of tongue: analysis of treatment outcome and prognostic value of margin status.

BACKGROUND: Choice of treatment for base of tongue carcinoma is controversial, with options including surgery alone, radiotherapy alone, or multimodality treatment. Given the highly aggressive nature of these tumors, it has been our institutional policy to manage this disease with combined partial glossectomy (with attempt to avoid laryngectomy if possible) with planned postoperative radiotherapy (RT). We reported on our institutional experience with this approach. METHODS: A retrospective review of the charts of 17 patients with primary base of tongue squamous cell carcinoma treated with sur…
