[Hollow foot in adults].

Contrary to the general impression, the hollow foot is much more common than the flat foot. We distinguish three categories of hollow foot: varus hollow (clinical form I), valgus hollow (clinical form II), and pseudo-hollow (clinical form III). These three clinical forms have a common deviation dynamics, the consequence of shortening of the sural-Achilles-calcaneoplantar system. Metatarsalgia often precedes deformation of the front of the foot. Talalgia is linked to excess traction or to a conflict between the heel and the back of the shoe. Common therapy consists of plantar orthoses, eventual

Problems in polydactyly of the foot.

1. Polydactyly should be treated early. 2. Adequate bone and soft tissue should be removed. 3. One arm of the Y should be removed surgically … metatarsals remain abnormal. 7. Polydactyly associated with a short first metatarsal and congenital hallux varus carriers a poorer treatment prognosis and may require continuing treatment. 8. Postoperative casting and taping should be utilized to prevent angular deformities and encourage normal forefoot contour.

[Surgical management of hallux valgus by techniques preserving the first metatarsophalangeal joint: long-term results].

PURPOSE OF THE STUDY: Hallux valgus is a frequent static deformity of feet in shoe-wearing populations. Lasting problems usually require sur … st, i. e., 17.9 and 4.5 degrees for HVA and IMA, respectively. Two patients of this group developed hallux varus, but their HVA and IMA values were not included in the assessment because they would adversely affect the objective evaluation of all the patients. However, in the subjective evaluation of the whole group, these two unsatisfied patients were include …

Prospective, randomized comparison of proximal crescentic and proximal chevron osteotomies for correction of hallux valgus deformity.

In this study, intermetatarsal angle (IMA) correction, functional outcome, and healing time for the proximal crescentic and proximal chevron osteotomies in moderate to severe hallux valgus deformity were prospectively compared. Seventy-five patients (97 feet) were prospectively randomized to either a proximal crescentic or proximal chevron osteotomy for the correction of moderate to severe hallux valgus deformity with associated metatarsus primus varus. Criteria for study entry included age (adult patients), IMA greater than or equal to 13 degrees, persistent symptoms despite nonoperative trea

Revisiting the radiological signs for the first metatarsal pronation assessment.

AIMS: The first metatarsal pronation deformity of hallux valgus feet is widely recognized. However, its assessment relies mostly on 3D standing CT scans. Two radiological signs, the first metatarsal round head (RH) and inferior tuberosity position (ITP), have been described, but are seldom used to aid in diagnosis. This study was undertaken to determine the reliability and validity of these two signs for a more convenient and affordable preoperative assessment and postoperative comparison. METHODS: A total of 200 feet were randomly selected from the radiograph archives of a foot and ankle cli

Screw versus plate fixation of proximal first metatarsal crescentic osteotomy.

BACKGROUND: Hallux valgus associated with metatarus primus varus is a deformity that disrupts normal foot function. Standard treatment has often included distal or proximal metatarsal osteotomy with or without a distal soft tissue procedure. The intrinsically unstable proximal crescentic osteotomy relies on operative fixation for stability. This study examined the strength of fixation of a single screw versus a plate for stabilization of proximal first metatarsal crescentic osteotomies for correction of hallux valgus. MATERIALS AND METHODS: A crescentic osteotomy was performed on nine pairs o

Correction of hallux valgus. Metatarsal osteotomy versus excision arthroplasty.

The long-term retrospective results (followup range, 10-22 years) of an uncontrolled series of basal metatarsal closing wedge osteotomies and Keller's excision arthroplasties performed in patients 14 to 40 years of age are analyzed. In the osteotomy group, 34 patients (50 feet) were available for clinical review and 26 patients (37 feet) were available for radiologic review. In the Keller group, 24 patients (37 feet) were reviewed clinically and 23 patients (34 feet) were reviewed radiologically. Patients were assessed using the Hallux Metatarsophalangeal Interphalangeal Scale of the American

[Complications of first ray osteotomies: a consecutive series of 475 feet with first metatarsal Scarf osteotomy and first phalanx osteotomy].

PURPOSE OF THE STUDY: Available studies on Scarf osteotomies of the first metatarsal (M1) and first phalange (P1) shortening and varus displacement have reported good results, but have have not focused on complications. We reviewed a consecutive series of 475 feet operated on over a five year period. Our goal was to determine the incidence of complications and to compare our data with reports in the literature. We wanted to know if association with Weil osteotomy on the lateral metatarsals affects the rate of complications. MATERIAL AND METHODS: We used the following inclusion criteria: hallu

Correlation Between Hallux Valgus Severity and the Prevalence of Metatarsus Adductus in Hallux Valgus.

INTRODUCTION: Hallux valgus (HV) is a common foot deformity, with metatarsus adductus (MA) identified as a potential predisposing factor. MA has been shown to negatively affect surgical outcomes for HV, particularly in severe cases. This study aims to clarify the prevalence of MA in the HV population using different metatarsus adductus angle (MAA) measurement methods and assess whether MAAs are influenced by HV severity. MATERIALS AND METHODS: This retrospective study included 294 feet from 147 participants. Patients were classified into non-HV (normal) and HV subgroups, with HV severity grad

Axial rotation of the first metatarsal head in a normal population and hallux valgus patients.

HYPOTHESIS: Does metatarsal pronation exist and, if so, what is its impact? INTRODUCTION: Hallux valgus is a deformity associating angulation and a rotational component. The present study sought to investigate the nature and origin of the coronal plane displacement. MATERIALS AND METHODS: A prospective single-center radiological and anatomic study was conducted on 100 feet operated on for hallux valgus. Baseline X-ray determined the preoperative position of the 1st metatarsal head in the coronal plane. The range of motion (ROM) of the cuneometatarsal joint in pronation-supination was measure

Revisional surgery for hallux valgus with serial osteotomies at two levels.

The aetiology and form of hallux valgus (HV) is varied with many corrective procedures described. We report a 39-year-old woman, previously treated with a Chevron osteotomy, who presented with recurrent right HV, metatarsus primus varus, and associated bunion. Osteotomies were performed at two levels as a revisional procedure. This report highlights (1) limitations of the Chevron osteotomy and (2) the revisional procedure of the two level osteotomies: (i) proximal opening-wedge basal osteotomy and (ii) distal short Scarf with medial closing wedges. If a Chevron osteotomy is used inappropriatel
