[Transport of finger parts using an abdominal skin flap for finger and thumb reconstructive plastic surgery].

1st. With the aid of an abdominal or an inguinal flap it is possible to reconstruct a finger with sensibility by transporting islands of palmar skin with the corresponding nerve to any amputation stump of the hand. At least three operations are necessary. Parts of bones, joints and tendons may be included in the transported part from the hand. 2nd. These digits are rather bulky. Mobility depends on the preserved joints. 3rd. This method should only be used for thumb reconstruction or to achieve a simple two digit grip. 4th. In children growth of transported bones remains undisturbed.

[Plastic surgery of the thumb, using the epiploon. The problem of integrating mechanical hand prostheses into the body].

The techinque using epiloon tissue in hand surgery was introduced by the author in 1974. The efficiency of the method was checked in several cases of radionecrosis of the hand, deep burns with complete destruction of the tissues on the back of the hand, severe trauma with tearing of the teguments and uncovering of the bones. Epiploonoplasty of the hand achieves a massive transfer of thick epiplooic tissue which is well vascularized, rapidly integrated in the circulatory system of the hand. An extensive description of the surgical technique is presented. A case of severe trauma with amputation

Oligodactyly with Thumb.

BACKGROUND: Oligodactyly of the hand is one of the rarest congenital anomalies of upper extremities and defined as the presence of fewer than five fingers on a hand. Although it usually occurs in association with hypoplasia or absence of ulna, it can occur without abnormality of the forearm bones. The purpose of this study is to present clinical features and radiographic characteristics of hand oligodactyly with thumb. METHODS: Five patients of oligodactyly with thumb who showed normal forearm bones, were evaluated. Two patients had threefingered hand with thumb, and three had two-fingered ha

Reconstruction of a complex defect of the hand with two distinct segments of the scapula and a scapular fascial flap transferred as a single transplant.

To repair multiple defects of the metacarpal bones in combination with skin defects, a combined scapular free flap on a single vascular pedicle--the subscapular artery and the accompanying veins--can be used successfully. This flap includes two distinct bone segments of the lateral border of the scapula and a scapular fascial flap. Because of its long and mobile vascular pedicle, such a transplant can be used for one-stage reconstruction of the first and other metacarpal bones of the long fingers of the hand. The scapular fascial flap included in the transplant provides good functional and aes

Desmoplastic Fibroma of the Phalanx of the Hand: A Rare Case Report.

Desmoplastic fibroma is a rare primary benign bone tumour that typically affects the long bones, mandible, and pelvis. It has a similar local aggressiveness to soft tissue fibromatosis. It rarely involves the small bones of the hand. We describe an extremely rare case of desmoplastic fibroma of the proximal phalanx of the hand in a patient who presented with an aggressively enlarging but painless mass on the left ring finger. Radiological features suggested malignancy; however, an initial biopsy revealed fibrotic tissue. Trans-metacarpal amputation of the ring and little fingers and soft tissu

Polyostotic Paget disease involving bones of the upper extremity.

Paget disease of bone (PD) is a common skeletal disease. It is usually polyostotic and most frequently involves the pelvis, femur, spine, skull, and tibia. The bones of the upper extremity in poly- or monostotic PD are rarely affected. A patient with PD involving the third left metacarpal bone and carpal bones of the right hand is described.

Possible case of spondylo-epiphyseal dysplasia in a medieval skeleton.

Multiple anomalies of vertebral and long bone growth were observed in the incomplete skeleton of an adult male from a collection of 693 medieval skeletons excavated at Abingdon, Oxfordshire. The skeleton consists of a fragmented skull, right scapula and humerus, left humerus, radius, ulna and partial hand, vertebrae from C1 to T11, and fragments of the ribs and sternum. The bones were examined, measured, and radiographed. The humeri are abnormally short and thick, with exaggerated muscle attachments. Both shoulder joints show extensive osteoarthritic destruction. Some vertebrae show slight fla

Osteochondroma of the phalanx: a late Roman case.

Accounting for 20-50% of all benign forms, solitary osteochondromae are the most common kind of bone tumour. The long bones of the lower extremity are the most frequently affected; the small bones of the hands and feet, the pelvis, the scapula, and the spine are less usual locations. This paper describes an osteochondroma in a proximal phalanx of the hand of a female aged between 17 and 25 years buried in a Late Roman necropolis from SE Spain. The bone displayed a solitary osteochondroma, which was confirmed by macroscopic and radiographic examination. This is the first palaeopathological exam

Metacarpophalangeal pattern profile and bone age in Rett syndrome: further radiological clues to the diagnosis.

Hand radiographs of 100 girls representing 73% of the known Australian population of girls with Rett syndrome, age 20 years or less, were available for this study. Control radiographs were matched for age, sex, and laterality. Bone age was assessed against standard radiographs in Greulich and Pyle [1959: Radiographic Atlas of Skeletal Development of the Hands and Wrist, 2nd ed.]. A metacarpophalangeal pattern (MCPP) profile comparing the relative lengths of the hand bones with mean population norms by age was produced by converting the length of each of the 19 metacarpal and phalangeal bones i

Vascularized Medial Femoral Condyle Flap Reconstruction for Osseous Defects of the Hand and Wrist.

Vascularized bone flaps from the descending genicular artery system are versatile and effective for the use of recalcitrant nonunions from the tubular bones of the hand to the long bones of the upper extremity. Familiarity with the vascular pedicle, various techniques of harvest and inset, and skin paddle harvest and application are essential for the reconstructive surgeon.

Twenty classic hand radiographs that lead to diagnosis.

Most of the common skeletal dysplasias have some manifestation in the hand. Many have characteristic findings in the hand that lead to the diagnosis. Hand bones are also affected in many systemic hematologic and metabolic conditions. The diagnosis can be clinched on a single hand radiograph if characteristic findings are present. This pictorial essay illustrates characteristic findings of 20 common conditions including bone dysplasias and metabolic and hematologic abnormalities on a single hand radiograph. It also includes some common hand abnormalities without systemic skeletal abnormalities.

Osteochondromas at Unusual Sites- Case Series with Review of Literature.

INTRODUCTION: Osteochondromas are benign tumours of the skeletal system. Their commonplace of occurrence is around growing ends of long bones like lower end of femur and upper end of tibia, but literature describing their incidence around flat bones of body like pelvis, scapula and small bones of hand, foot is rare. CASE REPORT: We describe two cases of osteochondromas at unusual sites, one on the dorsal aspect of scapula and other at the base of first metatarsal. Patient with scapular osteochondroma had difficulty in sleeping in supine position while that with metatarsal osteochondroma had d

Fractures of the Carpal Bones.

Diagnosis and proper initial management of acute fractures of the carpal bones is critical because of the limited blood supply of many bones of the wrist and the role of the carpus in optimizing hand function. Pathology is correctly diagnosed by a focused history and examination. Injuries may be missed with a cursory examination and routine wrist radiographs. Together, fractures of the scaphoid and triquetrum make up nearly 90% of carpal bone fractures. Relative frequency, mechanism of injury, diagnosis, and management principles are covered for each of the bones of the wrist.

The use of latissimus dorsi muscle flap in the aesthetical reconstruction of heat-press injury of the hand.

We present a successful case of aesthetic reconstruction utilizing free latissimus dorsi muscle flap transfer. A large quantity of skin of the dorsum of hand and finger was lost. The dorsum of the index, long and ring fingers was severely damaged, such that extensor tendons were necrotic and all digital bones and the second metatarsal bone were exposed with partial necrosis. In addition, the proximal interphalangeal joints (PIP) were also exposed. To cover exposed bones and the tendons of dorsum of the hand, a free latissimus dorsi muscle flap was transferred, and then meshed skin covered the

The role of metacarpophalangeal pattern (MCPP) profile analysis in the treatment of triphalangeal thumbs. Description of a method and a case report.

The metacarpophalangeal pattern (MCPP) profile analysis is a method of comparing the length of each of the 19 tubular bones of the hand on the X-ray with the standard length in the normal population according to age and sex. An MCPP plot is a graphic illustration of the MCPP analysis. It is not the exact height of the curve on the MCPP plot which is most important, but the profile which occurs because of the individual lengthening or shortening of the bones. This pattern profile appears to be specific for several congenital malformation syndromes. We have recently used MCPP analysis in plannin

Giant Cell Tumor of the Metacarpal Treated with Non-Vascularized Bone Graft: A Case Report.

INTRODUCTION: Giant cell tumor (GCT) of bone is a benign but locally aggressive neoplasm, most commonly affecting the epiphyseal region of long bones. Involvement of the hand bones is rare and is known to exhibit more aggressive behavior with higher recurrence rates compared to conventional sites. Optimal reconstruction following tumor excision in the hand remains challenging, as, along with the reconstruction of the bone defect, the function of the hand should also be addressed. CASE REPORT: A 19-year-old right-hand-dominant female presented with a progressively enlarging, painful swelling o
