Exploring the Physical and Mental Health Challenges of Teenage Pregnancy: A Qualitative Study in the Tamale Metropolis, Ghana.

OBJECTIVE: Teenage pregnancy poses significant public health challenges globally, particularly affecting the physical and mental well-being of adolescent girls. Despite a global decline in teenage birth rates, regions like sub-Saharan Africa continue to experience high prevalence. In Ghana's Tamale metropolis, teenage pregnancy rates have shown a concerning rise, underscoring the need for an in-depth exploration of its multifaceted impacts. This study aims to explore the physical and mental health effects of teenage pregnancy in the Tamale metropolis, capturing their personal experiences and c

Characteristics of chronic hepatitis B patients who underwent liver biopsies.

Significant liver disease has been reported in chronic hepatitis B patients with normal alanine aminotransferase (ALT) but most studies performed biopsies on selected patients only. The aims of this study were to determine the rate of liver biopsy, characteristics of patients who underwent a biopsy and factors associated with significant liver disease in a cohort of such patients. Records of patients with chronic hepatitis B during a 10-year period were reviewed. Significant liver disease was defined as Knodell HAI ≥ 7 and/or Ishak fibrosis ≥ 3. Of 743 patients, 55.7% were Asian, 56.4% were me

Paper versus electronic documentation in complex chronic illness: a comparison.

This paper reports an analysis and comparison of the electronic and paper medical records of 5 clinically complex persons with spina bifida, who were seen as in- and outpatients at a small Northeastern urban hospital. The combination of chronic illness, multiple medical and allied health specialties, and longer lifespan in this population ensures both a quantitative volume and qualitative complexity of medical event documentation. This ensures a rich field for research into the content and the nature of the fragmented data presently locked in the paper record.

Clinical characteristics and care pathways of patients with personality disorder who died by suicide.

BACKGROUND: It is estimated that 1 in 10 people have a personality disorder. People with emotionally unstable personality disorder are at high risk of suicide. Despite being frequent users of mental health services, there is often no clear pathway for patients to access effective treatments. AIMS: To describe the characteristics of patients with personality disorder who died by suicide, examine clinical care pathways and explore whether the care adhered to National Institute for Health and Care Excellence guidance. METHOD: National consecutive case series (1 January 2013 to 31 December 2013)

Acute health effects after exposure to chlorine gas released after a train derailment.

In January 2005, a train derailment on the premises of a textile mill in South Carolina released 42 to 60 tons of chlorine gas in the middle of a small town. Medical records and autopsy reports were reviewed to describe the clinical presentation, hospital course, and pathology observed in persons hospitalized or deceased as a result of chlorine gas exposure. Eight persons died before reaching medical care; of the 71 persons hospitalized for acute health effects as a result of chlorine exposure, 1 died in the hospital. The mean age of the hospitalized persons was 40 years (range, 4 months-76 ye

Adolescents' life between violence and discipline. Medical care in the "Jugendhäuser" juvenile detention centers in East Germany in the 1960s-1980s.

BACKGROUND: In 1952-1989, special juvenile detention centers, called Jugendhäuser, were established in the German Democratic Republic. There, juvenile delinquents had to not only complete their sentences, but they were also supposed to be re-educated into conscious socialist personalities through a system of collective education proposed by Soviet pedagogue Anton Makarenko. Among twelve Jugendhäuser in East Germany, the ones in Halle and Dessau were considered to have the most severe conditions due to the praxis of mental and physical violence. For the first time, based on the personal files o

Borderline personality features moderate trajectories of health and well-being around retirement.

With increasing percentages of the population in retirement, or approaching retirement, it is important to better understand how an individual's health and well-being may change during the retirement transition and for whom this transition may be particularly beneficial or problematic. Toward this end, the present study evaluated borderline personality disorder features as predictors of well-being and health trajectories from pre-to-post retirement, using up to seven waves of longitudinal data from the St. Louis Personality and Aging Network study. Participants were 590 older adults (aged 55-6

Linking the Nursing and Residential Care Facility Survey to tax and census data: A new data resource for health workforce research.

Calls for comprehensive data collection on the health workforce and long-term care sector have been voiced by policy makers, researchers, and public health experts following the aftermath of the COVID-19 pandemic and ongoing health care worker shortages. This paper introduces a novel linked data resource that fills these critical data gaps and establishes a foundation for evidence-informed policy making and workforce planning to strengthen the nursing and residential care system in Canada. This data resource integrates the Nursing and Residential Care Facility Survey, the Canadian Employer-Emp

World Sleep Society recommendations for the use of wearable consumer health trackers that monitor sleep.

Wearable consumer health trackers (CHTs) are increasingly used for sleep monitoring, yet their utility remains debated within the sleep community. To navigate these perspectives, we propose pragmatic, actionable recommendations for users, clinicians, researchers, and manufacturers to support CHT usage and development. We provide an overview of the evolution of multi-sensor CHTs, detailing common sensors and sleep-relevant metrics. We advocate for standardized 'fundamental sleep measures' across manufacturers, distinguishing these from proprietary exploratory metrics with future potential. We o

Perspectives of the person with mitral valve prolapse syndrome: a study of self-care needs derived from a health deviation.

Epidemiological studies in the United States indicate that 5% of the population or nearly 7 million people have Mitral Valve Prolapse. This incidence has also been confirmed by British physicians. Approximately half of these persons seek medical care for treatment of symptoms. Persons with symptoms are often said to have "Mitral Valve Prolapse Syndrome." The purpose of this study was to describe experiences and self-care needs of persons diagnosed with Mitral Valve Prolapse Syndrome (MVPS). In Phase I of the study, medical records of 124 subjects were reviewed to identify the medical experienc

A national evaluation of homeless and nonhomeless veterans' experiences with primary care.

Persons who are homeless, particularly those with mental health and/or substance use disorders (MHSUDs), often do not access or receive continuous primary care services. In addition, negative experiences with primary care might contribute to homeless persons' avoidance and early termination of MHSUD treatment. The patient-centered medical home (PCMH) model aims to address care fragmentation and improve patient experiences. How homeless persons with MHSUDs experience care within PCMHs is unknown. This study compared the primary care experiences of homeless and nonhomeless veterans with MHSUDs r

Walking the tightrope of dysphagia: insights from allied health professions working with children and young people in Australia.

PURPOSE: Dysphagia can negatively influence a child or young person's quality of life. There is currently limited in-depth qualitative data examining the impacts of dysphagia on children and young people from the perspectives the allied health professionals who provide supports that can shape their quality of life. This study examined the perspectives of allied health professionals on (a) the impact of dysphagia on quality of life and (b) barriers and facilitators to quality of life for children and young people (2-24 years) with dysphagia. METHODS: In this ethically approved research, 16 all

Expanding Race and Ethnicity Information Beyond Limited Preset Categories in Research and Clinical Care.

INTRODUCTION: Accurate and complete measurement of race and ethnicity is essential in clinical research and practice, but it is often undermined by inconsistent definitions and flawed data sources. Electronic health records (EHRs) regularly contain race and ethnicity information, yet some studies reveal errors, particularly for individuals from minority groups. Our study evaluates how data quality varies across three methods of collecting race and ethnicity information: EHR-extracted demographics, self-reported information via closed-ended questions, and self-reported information via open-ende

Association between primary heath care and dental service use among supportive housing tenants with behavioral health conditions.

OBJECTIVE: Homeless persons are at high risk for poor oral health. Supportive housing can improve housing stability for persons with behavioural health conditions, but its impact on dental service use has been little studied. METHODS: Data for this evaluation come from matched public service records for eligible applicants to a New York City supportive housing program (NY III) targeting chronic homelessness. NY III tenants (N = 5678) were compared with applicants not placed in housing (N = 6536) and applicants placed in other supportive housing programmes (N = 4823). Regression analysis was u

Hepatitis A Exposure Response and Outbreak Prevention in a Large Urban Jail - Los Angeles County, California, May-July 2023.

Correctional settings provide a high-risk environment for hepatitis A transmission because of the high proportion of homelessness and injection drug use among persons who are incarcerated. On May 30, 2023, Los Angeles County Department of Public Health informed the Communicable Disease Surveillance and Control (CDSC) unit of the Los Angeles County Jail system that a symptomatic incarcerated person had received a positive test result for acute hepatitis A. Upon learning the next day that the patient was a food handler, CDSC staff members identified 5,830 potential contacts of the index patient,

Matching ICD-11 personality status to clinical management in a community team-The Boston (UK) Personality Project: Study protocol.

Epidemiological studies show 30% to 50% of all patients in community mental health teams have personality disorders. These are normally comorbid with other psychiatric disorders, often as Galenic syndromes, and are seldom identified. In the Boston (UK) Personality Project all patients under a community health service in Boston in Lincolnshire will be asked to agree to have their personality status assessed using scales recording the new ICD-11 classification, together with clinical ratings, social function and satisfaction. A control group of 100 patients from an adjacent service of similar de

Relationship quality and mental health among sexual and gender minorities.

Sexual and gender minorities assigned female at birth (i.e., sexual minority women, transgender men, and gender diverse [SMW TGD] individuals) experience disproportionately high rates of anxiety, depression, and substance use problems. Romantic relationship involvement has been shown to be beneficial to mental health and substance use among sexual and gender minorities. However, few studies have explored the impact of relationship quality on mental health, or if high relationship quality can reduce the negative impact of minority stress on well-being in this population. The present study exami
