Adjuncts to annuloplasty in atrial functional mitral regurgitation.

Atrial fibrillation is the most common cardiac arrhythmia, leading to progressive dilation of cardiac chambers, abnormal contraction patterns of the atria and ventricles and, potentially, atrioventricular valvular insufficiency. Moreover, heart failure with preserved ejection fraction is often present and closely intertwined with disease initiation and progression. Surgical valve repair with a true-sized ring annuloplasty is a well-established treatment option in atrial functional mitral regurgitation. While early results are good, recent studies have brought the durability of this repair appr

Mechanisms of atrial structural changes caused by stretch occurring before and during early atrial fibrillation.

Structural remodelling occurring before, due to the underlying heart disease, and during atrial fibrillation (AF) sets the stage for permanent AF. Current therapy in AF aims to maintain sinus rhythm in symptomatic patients, but outcome is unfortunately poor. Stretch of the atria is a main contributor to atrial remodelling. In this review, we describe different aspects of structural remodelling as seen in animal models and in patients with AF, including atrial enlargement, cellular hypertrophy, dedifferentiation, fibrosis, apoptosis, and loss of contractile elements. In the second part, we desc

Disappearing intracardiac thrombi in both atria after mumps in a patient with Turner syndrome.

A girl with Turner syndrome was admitted with an acute cerebrovascular occlusive disease 15 days after mumps infection. Imaging techniques such as Doppler echocardiography, computed tomography and angiography of the heart revealed the existence of masses in both atria. Eight days after the last radiologic study the patient had an operation, but no masses were found in either atrium. It was thought that atrial thrombi, probably formed after viral infection, had broken down to form emboli and disappeared. It is proposed that the patients with congenital cardiopathy should be regularly examined a

An alternative technique for orthotopic cardiac transplantation, with preservation of the normal anatomy of the right atrium.

The standard technique for orthotopic cardiac transplantation implies large atrial anastomoses which do not preserve the anatomical integrity of the donor atria. This may become a potential source of electrophysiological and mechanical atrial dysfunction, especially in the right atrium with the sinus node and the sensitive low-pressure atrioventricular valve. As an improvement we suggest an alternative technique which we have recently developed for orthotopic cardiac transplantation; it combines the simple, convenient left atrial connection of the standard technique with individual anastomoses

Carcinogenicity of heptachlor and heptachlor epoxide.

Heptachlor and its metabolite heptachlor epoxide are unequivocally carcinogenic in rats and mice. The chemicals induced carcinomas of the liver, which were highly significant. There were neoplasms at other sites in rats. Neoplasms at all sites, as well as malignant tumors, were increased in heptachlor-treated male rats. There were similar increases in benign and malignant neoplasms of endocrine organs, particularly in female rats. Neoplasms of the thyroid and pituitary were increased in male rats and neoplasms of the reproductive system, including the ovary and uterus, in female rats given hep

Fatal Case of Hypertrophic Cardiomyopathy in a Donor Heart: A Case Report.

Hypertrophic cardiomyopathy is a genetically determined heart muscle disease, and patients are at an increased risk for sudden cardiac death. We report the case of a 39-year-old White man who was found dead at home unexpectedly. He had a cardiac transplant for congenital heart disease at the age of 12 and his condition was maintained with immunosuppression ever since with good cardiac function and right bundle branch block. At autopsy, the heart was enlarged with a weight of 591 g and had fibrous adhesions of the pericardium with endothelialized sutures in the atria and great vessels in keepin

Cardiac transplantation in infants and children.

Cardiac transplantation has become a more frequently used therapeutic modality for select cardiac pathology in infants and children. Since June 1986, 30 pediatric patients (19 male and 11 female) ranging in age from 4 days to 15 years (11 < or = 1 month old) have undergone orthotopic cardiac transplantation at our institution. Indications included idiopathic cardiomyopathy (n = 8), hypoplastic left heart syndrome (n = 13), and other forms of complex congenital heart disease (n = 9). There have been four operative and three late deaths only in the groups with hypoplastic left heart syndrome and

Electropathological substrate of longstanding persistent atrial fibrillation in patients with structural heart disease: epicardial breakthrough.

BACKGROUND: During persistent atrial fibrillation (AF), waves with a focal spread of activation are frequently observed. The origin of these waves and their relevance for the persistence of AF are unknown. METHODS AND RESULTS: In 24 patients with longstanding persistent AF and structural heart disease, high-density mapping of the right and left atria was performed during cardiac surgery. In a reference group of 25 patients, AF was induced by rapid pacing. For data analysis, a mapping algorithm was developed that separated the fibrillatory process into its individual wavelets and identified wa

Heart transplantation: operative techniques and postoperative management.

Heart transplantation has become an established therapeutic option for some patients with end stage heart failure. The technique of heart transplantation involves excision of the diseased heart leaving behind cuffs of the right and left atria, pulmonary artery and aorta, which are subsequently attached to their counterparts in the procured donor heart. In most cases donor and recipient operations are carried out in two different hospitals which can be hundreds of miles apart. Careful coordination and timing of the donor and recipient operations is fundamental to keep the ischemic time (ie, the

The surgical anatomy of ventricular septal defects with univentricular atrioventricular connection.

Hearts that do not possess one-to-one connections at the segmental junctions almost always produce a univentricular atrioventricular connection. One ventricle is usually large and dominant and the other small, lacking one or two of its components. The ventricular septal deficiency forms part of the circulatory pathway. We take the stance that only hearts that possess a truly solitary ventricular chamber are univentricular. They cannot have a ventricular septal defect, and so are excluded from this study. This review, therefore, is concerned with the morphology of septal defects in hearts in wh

Atrial cardiomyopathy in endurance athletes.

Atrial cardiomyopathy is characterized by electrical and structural remodeling of the atria, which can predispose to arrhythmias and thromboembolic stroke. Changes in atrial size and function are frequently observed in athletes engaged in endurance sports, a phenomenon known as "athlete's heart." Common left atrial observations in athletes may include larger left atrial volumes but lower left atrioventricular volume ratios, mildly reduced left atrial strain, possible mild left atrial fibrosis, longer P-wave duration, and greater atrial ectopic activity. However, it remains unclear whether thes

Congenital abnormalities of the conduction system in sudden death in young adults.

The sudden death of four apparently healthy young people, three men aged 25, 24 and 21 years, respectively, and one woman aged 20 years, was investigated. The heart was of normal size in two and was slightly enlarged in two. The conduction system in all hearts was serially sectioned. In all hearts the His bundle showed loop formation. In two, the branching bundle was left-sided with the right bundle branch intramyocardial. In the other two, the His bundle was markedly fragmented. Two had myocarditis, and three had arteriolosclerosis of the summit of the ventricular septum. These findings sugge

In-situ optical assessment of rat epicardial kinematic parameters reveals frequency-dependent mechanic heterogeneity related to gender.

BACKGROUND: Gender-related cardiac mechanics following the electrical activity has been investigated from basic to clinical research, but results are still controversial. The aim of this work is to study the gender related cardiac mechanics and to focus on its heart rate dependency. METHODS: We employed 12 Sprague Dawley rats (5 males and 7 females) of the same age and, through a novel high resolution artificial vision contactless approach, we evaluated in-situ cardiac kinematic. The hearts were paced on the right atria appendage via cathodal stimuli at rising frequency. RESULTS: Kinematic d

Excess ROS induced by AAPH causes myocardial hypertrophy in the developing chick embryo.

BACKGROUND: The developing embryo is very sensitive to oxidative stress and excess reactive oxygen species (ROS) generation is often associated with cardiovascular malformation. However, little is known about the adverse effects of ROS during heart morphogenesis, especially during the formation of the atria and ventricles. METHODS AND RESULTS: We have treated early chick embryos with 2,2-azobis (2-amidinopropane) dihydrochloride (AAPH) to generate free radicals in the developing heart. We established that excess ROS induced by AAPH caused cardiomegaly to develop in 4-, 14- and 17-day-old embr

Immunohistochemical and biochemical evidence for a cardiovascular mineralocorticoid receptor.

The presence of mineralocorticoid receptors (MRs) and their physicochemical characteristics were investigated in the heart and blood vessels of rabbits. Immunohistochemical methods using the monoclonal anti-idiotypic antibody H10E, which interacts with the steroid binding domain of MRs, revealed the presence of immunoreactive material in the heart and large blood vessels. In the heart, a positive staining was observed in myocytes and endothelial cells of atria and ventricles. In vessels, MRs were detected in the aorta and pulmonary artery. They were localized in endothelial and vascular smooth

Recellularized Humanized Bioengineered Biatrial Model for Arrhythmia, Biological Pacemakers, and Optogenetic Studies.

BACKGROUND: The study of atrial arrhythmias has been hampered by the lack of anatomically relevant human cardiac tissue models and by the inability to perform targeted, functional perturbations in such models. METHODS: To engineer anatomically relevant light-sensitive atrial chambers, we combined human pluripotent stem cells, differentiation protocols yielding atrial and sinoatrial nodal cells, rat heart decellularization/recellularization processes, and optogenetics tools. RESULTS: Immunostaining for chamber-specific cardiomyocyte markers, optical action potential recordings, and the respon

Robustness of imageless electrocardiographic imaging against uncertainty in atrial morphology and location.

INTRODUCTION: Electrocardiographic Imaging is a non-invasive technique that requires cardiac Imaging for the reconstruction of cardiac electrical activity. In this study, we explored imageless ECGI by quantifying the errors of using heart meshes with either an inaccurate location inside the thorax or an inaccurate geometry. METHODS: Multiple‑lead body surface recordings of 25 atrial fibrillation (AF) patients were recorded. Cardiac atrial meshes were obtained by segmentation of medical images obtained for each patient. ECGI was computed with each patient's segmented atrial mesh and compared w

Cardiac mucormycosis: a case report.

BACKGROUND: Mucormycosis is an invasive fungal infection (IFI) most commonly seen in immunocompromised patients. Diabetic ketoacidosis, haematopoietic transplantation, iron overload states, and deferoxamine therapy are considered to be some of the classical risk factors. While cutaneous and rhino-sinusoidal forms may be seen in immunocompetent (IC) individuals, cardiac and mediastinal involvement is rare. In this report, we describe a young patient without predisposing factors who presented as mediastinal mucormycosis with extensive cardiac involvement. CASE SUMMARY: A 19-year-old male presen
